Living Well Health and Wellbeing Coordinator Role:

Referrals and assessment

• Triage and co-ordinate client referrals from a range of sources, primarily health and care, in conjunction with the Living Well Manager.  These can be for the Living Well service alone, or for “Living Well Plus” where additional support is given to those clients who may require hoarding or de-cluttering support
• Communicate with referral partners to understand the trigger for referral and liaise with clients to build trust and encourage initial assessment
• Work closely with Adult Social Care, regularly hot desking at Merton Civic centre and joining GP surgery Multi-disciplinary meetings to accumulate more referrals and build professional relationships
• Actively engage in taking part in community events and meetings to promote the Living Well service and increase referrals/referral pathways
• Hold ‘guided conversations’ to assess clients’ needs and home risk, these will be conducted on telephone and through home visits

Action planning and support provision

• Create person-centred action plans to enable clients to set realistic goals to improve their overall health and wellbeing.
• Connect individuals to support services catered to their needs at both Age UK Merton and external partners
• Provide ongoing support and co-ordination for clients to ensure that action plans are progressing, including regular liaison with partners across health, care and voluntary sector to achieve successful client outcomes
• Work with partners across health, care and voluntary sectors to build strong understanding of the service offer for older adults in the London Borough of Merton.
• Receive and respond to enquiries related to the programme by phone, in person and by e-mail
• Work closely within Community Support Service, carrying out joint home visits with appropriate staff to ensure better outcomes for clients as well as ensure streamlined/triaged support.

General

• Record information on the CRM database, Charity Log, including service monitoring data and provide accurate and timely monitoring reports as required for funders
• Engage in exporting personal monthly KPIs as required by the Living Well Manager
• Cover the Community Navigator Role when needed with the Community Support service team on a rota basis
• Create two Case Studies quarterly, reflecting on successful outcomes for clients
• Carry out reviews with clients to capture the client’s journey from assessment to closing the case; showcasing how the client has improved their health and wellbeing outcomes
• Understand the process for raising safeguarding concerns and the route of accountability
• Comply at all times with the policies and procedures of Age UK Merton 
• Ensure that Age UK Merton’s Equal Opportunities policies, principles and practices are observed and implemented throughout service delivery
• Attend staff meetings, personal supervision and appraisal meetings
• Be aware of own training needs and participate in training/education to improve performance considered relevant to the post and to achieve agreed targets

St Giles is expanding its delivery of hospital-based projects; this service is aimed at vulnerable 12 to 25-year-olds who attend the adult or paediatric emergency department or Urgent Care Centre at Northwick Park Hospital. We will work alongside hospital staff to proactively identify vulnerable young people and help them to access and engage the support they need to prevent any potential escalation of violence and reduce the risk of repeat victimisation or exploitation.

About this key role

You will be embedded in the hospital, providing support for those young people admitted to emergency departments right through to their discharge back into the community. You will have experience of working with ‘high risk,’ vulnerable young people, be part of a multidisciplinary team, and have a flexible, empathetic approach to your work.   This support extends post-discharge from the ED or Hospital with our Caseworkers working with the CYP and the professional networks to ensure the CYP voice is front and centre, this is to support the betterment of the CYP and their future, as well as reducing risks. This is done through providing 121 support in formulating the support plan with the CYP. This can also include supporting the family and advocating for them and CYP within the wider professional network.

We actively encourage people with personal experience of the criminal justice system or lived experience of the issues facing this client group to apply for this role.

You will provide appropriate emotional and practical 1-to-1 support to (those who identify as) women and non-binary people involved in sex work and those who are sexually exploited, who often experience stigma and face barriers to accessing services. You will be enabling people to access support and navigate services,  You will be working in partnership with other agencies and making appropriate referrals for people to access other services to support their wider needs. You will contribute to wider team responsibilities including outreach, drop in and duty. 

To provide appropriate emotional and practical support to women in the sex industry and/or those who are being sexually exploited with a specific focus on those who are homeless and experiencing domestic violence and abuse. The women we support require intensive practical and emotional support as well as support navigating health and other social care or criminal justice systems. The women will have poor mental or physical health, experience with homelessness and of the criminal justice system, are often victims of sexual and/or domestic abuse and many are homeless. This post specifically will include a focus on domestic violence, sexual exploitation, sexual violence and housing. Working within a harm reduction ethos prioritising women’s safety and wellbeing, the intensive personalized supported will help women to access other services making appropriate referrals for women who wish to make changes, including exiting/transitioning out of sex work. You will be responsible for ensuring that women with varying needs are supported effectively throughout their pathway of support with Basis and partners, advocating for and with the women you are working with including where necessary challenging stigma and judgement women face.

As Counselling Coordinator, you will support the smooth and effective delivery of Woman’s Trust’s specialist counselling services for women affected by domestic abuse, with particular responsibility for supporting crisis and short-term therapeutic provision in East London.

You will coordinate referrals, assessments, caseload allocation, liaison with counsellors, and ensure accurate administrative and clinical record systems. Barnardo's is seeking an empathetic and child led individual who can work within a dynamic and fast-paced environment using their strong organisational, communication and time management skills to support children in the secure estate.

This part-time position (Independent Children's Rights and Advocacy Worker – Project Worker 2) is based within HMYOI Parc, which accommodates children aged between 15-18 years, who are in custody, either sentenced or on remand.  Barnardo's refers to Young Offender Institutions (YOI) and Secure Training Centres (STC) as the ‘Secure Estate'.

Barnardo's is commissioned by the Ministry of Justice (MoJ) to provide an Independent Children's Rights and Advocacy (ICRAS) Service to children accommodated in a secure setting. The service is known to children as Barnardo's: Your Rights, Your Voice, and currently works within  four Young Offender Institutes and  one Secure Training Centre. The ICRAS service is child led and independent of the secure estate; our service is delivered within HMYOI Parc to ensure children can freely access support for a range of issues linked to their needs, rights & experiences of custody, resettlement, and safeguarding.  As such this is a child-facing service, and at times involves lone working in the establishments, so we are seeking someone who can see the child, not the offence.

We hold ‘voice' at the heart of all we do, therefore we feel the role is best described by someone who is currently working in this sector: “The role is an Independent Children's Rights and Advocacy role, which means it is our job to empower the children we work with and help them to understand that what they think, what they feel and what they want, really matters. The position (Independent Children's Rights and Advocacy Worker – Project Worker 2) is line managed by a Team Manager, reporting to an off-site manager. The post holder will need to be able to work autonomously, working to the requirements of the contract and the regime of the YOI. The secure estate is a highly structured environment; as a Barnardo's service we deliver independent advocacy and support for a range of issues, whilst still having to follow and adhere to this structure.

This role includes lone working in this challenging secure environment. It is, therefore, critical that the successful candidate can follow guidance and policy and is able to take proactive and individual responsibility to understand and access the service support mechanisms.  This role requires the worker to be onsite for their contracted hours, working remotely only for occasional training or meetings. The advocacy team work on a rota system with set hours each week, which includes weekends and bank holidays. The West Sussex Parent Carer Forum (WSPCF) is an independent organisation for the parent carers of children and young people aged 0-25 with special educational needs and/or disabilities (SEND). We support parent carers by providing information, signposting, and training that equips them in their lifelong caring role and empowers them to participate in shaping services for their children and young people.

This is a great opportunity for a proactive and passionate person who is committed to collaborating with education, health and care partners, to strengthen co production and drive service and system change in West Sussex. You will be an excellent, persuasive communicator and have had demonstrable success at partnership working. You will be confident in strategic leadership, skilled at amplifying parent carer voice and knowledgeable and organised in managing a wide brief which includes overseeing delivery. You will lead a dedicated team motivated by improving outcomes for children and young people with SEND and be supported by Amaze who host and provide back-office support to WSPCF. Skills and experience required

• Parent carer of a child or young person aged 0-25 years old, living in West Sussex with a special educational need or disability (SEND) • Strong understanding of the SEND landscape, including the needs and experiences of parent carers and children/young people with SEND • Experience of overseeing delivery of multiple projects or workstreams • Experience using monitoring, evaluation and feedback to shape priorities, demonstrate outcomes and assess impact • Experience in strategic leadership • Experience of partnership working with senior leaders across local authorities, health, education, VCSE sectors • Excellent communication and interpersonal skills • Strong organisational skills • Confidence in IT and financial oversight • Commitment to equality, diversity and inclusion, and to empowering parent carers • Ability to lead staff and volunteers, including providing supervision and support • Creative, innovative and reflective approach to problem solving

This is initially a fixed-term role, with the possibility of extension subject to the continued development, funding and success of the Land Management Advice Service

We have secured funding from Natural England to explore how private finance and new farm‑based business opportunities can help reduce environmental pressures on the River Clun Special Area of Conservation (SAC) and inform a possible future Protected Site Strategy (PSS) for the area. This project is known as FINCH (Facilitating Investment in Natural Clun Habitats).

What you will be doing:

This exciting role involves managing the day-to-day delivery of the FINCH project, including developing and maintaining strong relationships with participating farmers and external consultants, and ensuring all work is delivered to agreed timescales, budgets and reporting standards.

You will play a key role in facilitating learning and deepening understanding of emerging nature markets, engaging a wide range of audiences including farmers, landowners, colleagues and partners. A central aspect of the role is building and maintaining positive relationships with farmers and landowners to support wildlife friendly farming, habitat creation and maintenance and to encourage land use change where appropriate. You will also provide land management and farm advice, taking practical action across a range of settings to benefit different habitats including riparian corridors and river systems.

Working collaboratively with colleagues, you will contribute to the ongoing development of Land Management Advisory Service, help identify and engage new clients and strengthen support for nature-friendly farming practices. You will also support the development and implementation of systems for evaluation and feedback from landowners and farm managers.

In addition, the role involves delivering practical on-farm measures through third-party funding, supporting farm businesses to access government and environmental assurance schemes and undertaking farm surveys and whole-farm planning focused on the delivery of environmental goods.

What you will bring:

• A degree in Agriculture or a related countryside discipline, or equivalent professional experience.
• Project management experience, with a track record of delivering projects to agreed timescales, budgets and outcomes.
• Experience providing agricultural advice and management support, including knowledge of Government grant schemes and emerging Nature Finance Markets.
• Experience in delivering environmental and wildlife sensitive farm surveys and whole farm management plans.
• Willingness to travel across the county to engage with farmers, landowners and colleagues as required.
• Excellent communication skills, supported by strong IT skills, including proficiency in Microsoft Office functions such as Word, Excel, Outlook, email and the internet.
• Full current UK driving licence.

We combine projects across Shropshire (including Telford & Wrekin) with advocacy and campaigning to restore nature and to engage people. We manage over 40 nature reserves and have almost 50 staff, 300 volunteers, and over 9000 members. Barnardo's is seeking an empathetic and child led individual who can work within a dynamic and fast-paced environment using their strong organisational, communication and time management skills to support children in the secure estate.

This part-time position (Independent Children's Rights and Advocacy Worker – Project Worker 2) is based within HMYOI Werrington, which accommodates children aged between 15-18 years, who are in custody, either sentenced or on remand.  Barnardo's refers to Young Offender Institutions (YOI) and Secure Training Centres (STC) as the ‘Secure Estate'.

Barnardo's is commissioned by the Ministry of Justice (MoJ) to provide an Independent Children's Rights and Advocacy (ICRAS) Service to children accommodated in a secure setting. The service is known to children as Barnardo's: Your Rights, Your Voice, and currently works within  four Young Offender Institutes,  one Secure Training Centre.  The ICRAS service is child led and independent of the secure estate; our service is delivered within HMYOI Werrington to ensure children can freely access support for a range of issues linked to their needs, rights & experiences of custody, resettlement, and safeguarding.  As such this is a child-facing service, and at times involves lone working in the establishments, so we are seeking someone who can see the child, not the offence.

We hold ‘voice' at the heart of all we do, therefore we feel the role is best described by someone who is currently working in this sector: “The role is a Children's Rights and Advocacy role, which means it is our job to empower the children we work with and help them to understand that what they think, what they feel and what they want, really matters. We can speak on behalf of children to ensure their voice is heard and we also have the opportunity to help them to find the tools and confidence to raise their voices for themselves.  Advocacy and Children's Rights support is particularly crucial in the secure estate because children are away from home, family and natural advocates, and also because children in secure estate are some of the most vulnerable children in society; they have often faced considerable adversity, disadvantage and discrimination prior to arriving into custody and they might not, therefore, be equipped with the skills needed to articulate their concerns. Through the work you do with a child such as  simply helping them make contact with friends or family on the outside, to helping them with concerns they may have in relation todiscrimination, resettlement or safeguarding issues, you  may be the one person telling them that  they matter for the very first time.”

The position (Independant Children's Rights and Advocacy Worker – Project Worker 2) is line managed by a Team Manager, reporting to an off-site manager. The post holder will need to be able to work autonomously, working to the requirements of the contract and the regime of the establishment. The secure estate is a highly structured environment; as a Barnardo's service we deliver independent advocacy and support for a range of issues, whilst still having to follow and adhere to this structure.

This role includes lone working in this challenging secure environment. It is, therefore, critical that the successful candidate can follow guidance and policy and is able to take proactive and individual responsibility to understand and access the service support mechanisms.  This role requires the worker to be onsite for their contracted hours, working remotely only for occasional training or meetings. The advocacy team work on a rota system with set hours each week, which includes weekends and bank holidays. Applicants should also be aware, that due to the nature of working within the secure estate, the vetting and induction process can take several months to complete.
