As Homeless Pathway Coordinator, you’ll be a consistent and trusted presence for patients experiencing homelessness, helping them navigate what can feel like an overwhelming mix of health, housing and support systems. You’ll spend time with patients on wards and in A&E, listening to what matters to them, understanding the barriers they face and making sure their voice remains at the heart of every decision.
Day to day, you’ll manage a caseload, complete assessments, develop clear discharge and support plans and make referrals to housing, healthcare, social care, mental health and substance use services. You’ll follow up actions, keep plans moving and advocate firmly when someone is at risk of being discharged without the right accommodation or support in place.
You’ll work closely with the Homeless Pathway Lead, hospital teams, Newham Council and SHP’s wider Health Team, bringing professionals together and helping everyone stay focused on the person behind the case. You’ll also build links with community services, identify gaps in existing pathways and share learning that helps the hospital respond more effectively to people experiencing homelessness.
This is a role where strong relationships can change the course of someone’s recovery. You’ll join an experienced and supportive team, with opportunities to deepen your knowledge of inclusion health, homelessness and psychologically informed practice through training, reflective practice and shared learning.

Interviewing ex-offenders, offering one-to-one job coaching to help them into work and providing ongoing mentoring during their first 12 months of employment.
Ours is a face-to-face, holistic and wraparound service that culminates in our candidates finding good jobs that last, with a wide array of socially inclusive employers. Our job finder support can begin in custody and continue through the gate into the community, or start within the community. Typically, we work with an individual for between one and three months to establish work readiness and continue supporting them for up to a year after they secure employment. Many people remain in touch and return to us for subsequent jobs over a period of years, and we consider our door to always be open.

This is an important new role reporting to the Student Support Lead (Disability and Inclusion). The post-holder will provide support and advice to trainees with a wide range of needs, including disabilities such as SpLD, autism and ADHD, as well as mental health conditions. They will write Reasonable Adjustment Plans (RAPs), work with partners to ensure agreed support is implemented effectively, escalate cases where appropriate, and contribute insight to help strengthen student support, inclusion and wellbeing practice across NIoT. Above all, the role is focused on enabling an outstanding student experience.

Islington Refuge Service provides emergency accommodation to women and children fleeing domestic abuse and sexual violence. Sessional play workers deliver high-quality safe fun activities that include a combination of enrichment activities, physical games and sports and nutritional education to children up to the age of 14.
About the Role
As a Children Sessional Play Worker, you will be involved in the planning and delivery of a variety of sessions and activities for children and young people at the refuge. This includes listening to children’s ideas to design services around their needs, choices, and aspirations. You will support and involve children in activities which are playful, stimulating, and enjoyable.
You will work alongside the Family Support Worker, encouraging all children to take part and have as much fun as possible whilst helping to develop children's learning, well-being, and social skills.
About You
You will have experience of working with children and you are passionate about play as well as dedicated to providing a welcoming and safe space for all children - supervising the children and ensuring their safety at all times.
The ideal candidate will have in-depth knowledge of domestic abuse and its impact on children as well as practical experience of working with children and within a team. You will also need to demonstrate a proven ability to work on your own initiative and under supervision. A minimum NVQ Level 2 or equivalent in child-care /play/ youthwork /sports/coaching or creative arts is desirable. Responsibilities of our Legacy Assistant:
As the Legacy Assistant you will be part of the Legacy Admin Team which is responsible for the administration of all legacies left to Cats Protection, ensuring that our supporters’ final wishes are fulfilled and their legacies are able to help as many cats as possible.
The role will involve providing administrative assistance to the Legacy Admin Team, including the accurate and timely recording of all legacy notifications, being the main point of contact for all queries legacy related and administering our Branch legacy allocation process. The successful candidate will also manage their own case of pecuniary legacies, undertaking regular reviews to ensure payment is received.

What we’re looking for in our Legacy Assistant:
Experience of using and managing/maintaining a database
Experience in a public facing/customer service role
Experience of admin activities such as preparing reports, writing letters, managing and triaging email inbox, tracking responses and maintaining records
Excellent verbal and written communication skills and able to communicate well with a wide range of people
Good attention to detail, organised and methodical
Able to work as part of a team and build relationships with relevant teams and stakeholders both internally and externally.

We are looking for an effective and compassionate Children's Independent Sexual Violence Advisor (CHISVA) to support children and young people, and their families, affected by sexual violence. You'll help victims and survivors to cope and recover, as well as understand the criminal justice process and act as their advocate where needed. The Bounce Back service forms an integral part of Hertfordshire Mind Network’s Complex Needs Community Services. The service provides one-to-one outreach support to facilitate a smooth, timely and supportive discharge from hospital back in to the community.
About the Project
Herts Mind Network (HMN) has a partnership with Hertfordshire Partnership Foundation Trust (HPFT) to deliver a one-to-one outreach service for people who are in hospitals across Hertfordshire.
There is an emergency department arm of the project and will be based at Watford General Hospital working in A&E departments, with the Mental Health Liaison Team, and hospital wards facilitating hospital discharge for people with mental health need.
About the Role:
The Bounce Back Worker will be based at Watford General Hospital and HMN Centres, promoting joint working between Watford A&E, creating a seamless pathway into community support for clients recovering from mental ill health.
The aims of the Bounce Back service are to provide up to 16 sessions of emotional and practical support and advice & information to patients. We will visit patients on the ward and continue the sessions in the local community and in patient’s homes once discharged. We will facilitate a smooth and timely hospital discharge.
Key Responsibilities
The Bounce Back worker will use a recovery orientated approach working with patients on the ward, and in the local community, including in people’s homes. The recovery approach includes empathy, warmth, acceptance, authenticity, compassion and humanity. The Crisis & Resilience Fund Team Leader will lead the delivery of the Exeter Crisis & Resilience Service, providing operational leadership to a multidisciplinary team delivering high-quality, person-centred support to residents experiencing financial hardship, crisis and multiple disadvantage. The postholder will line manage Navigate's Client Liaison Officers and the Money Advice Caseworkers employed by partners Churches Housing Action Team (CHAT) and Encompass Southwest to ensure a seamless, coordinated service that supports individuals from immediate financial crisis through to long term resilience. As Navigate's operational lead for the contract, the Team Leader will foster strong partnership working, maintain effective communication across organisations and ensure clients experience a consistent 'No Wrong Door' approach.
