 Mind in Tower Hamlets, Newham and Redbridge are working in partnership with Stepney and Wapping Primary Care Network’s Neighbourhood Team to support the Transformation of Mental Health Services across Tower Hamlets.

Tower Hamlets is one of the pilot Boroughs to implement the Community Mental Health Framework which aim is to redesign the offer to support people with serious mental illness (SMI) to not just get back on track with their lives but to be happier and more fulfilled.

The programme is grounded in a population health approach to SMI, delivered through and with Primary Care Networks in Tower Hamlets, with an ambitious vision to support more patients closer to home and reduce the stigma and fragmentation in existing services.

Purpose of the role

 The Community Connector will be responsible for working within the Neighbourhood Team within Stepney and Wapping PCN to ensure that people with SMI are able to live fulfilled lives through offering of one-to-one support to set goals towards their aspirations, needs and to offer hope and choice using a range of skill, knowledge and tools to achieve this. The community connector is seen as valuable asset to our neighbourhoods and act us a local expert, gathering and sharing information about local services.

The post holder will work collaborative and in partnership with the Multi-Disciplinary Teams and local Services and Community Groups to create pathways of support for people and to will act as the ‘glue’, linking people in with experts and local assets, and undertaking support work in partnership with external stakeholders to complement their interventions. The post holder will support people to identify the wider issues that impact on their health and wellbeing, such as debt, poor housing, employment circumstances and unemployment, loneliness, isolation and caring responsibilities.

Key Responsibilities

 Case Management

 · To support and offer opportunities for the involvement of people with lived experience of mental health in the design, delivery and monitoring of services.

· To conduct regular innovative and engaging sessions (virtually or in person) in order to work towards support plan goals.

· To use a range of tools and skills such as health coaching and motivational interviewing techniques, identify barriers to people accessing services, and work with service users to overcome these.

Community Development

 · Develop productive relationships with local partner organisations to improve service outcomes, and involve service users and carers in the design, development, and delivery of the service.

· Triage referrals and signpost to specialist support quickly where necessary, and deal with general queries to contribute to the overall smooth running of the neighbourhood team.

Monitoring and Compliance

 · Ensure accurate and timely record keeping and data collection in line with MindTHNR’s and ELFT’s policies and procedures

· obtain and collate feedback from services user and their carers and ensure that these are recorded and shared with Neighbourhood Team and MindTHNR

· Maintain confidentiality and ensure that information is stored according to Mind in Tower Hamlets, Newham and Redbridge policies and service guidelines, GDPR and data compliance.

 General

 · Act as an ambassador and represent Mind in Tower Hamlets, Newham and Redbridge at external functions, events, giving talks and presentations to inform and inspire.

· Comply with, promote, and contribute to the development of MindTHNR’s: Aims and Values, Equality, Diversity and Inclusive principles and all organisational policies.

You must have:

· To have attained a minimum Level 3 in Health and Social Care or equivalent

· Demonstrable and substantial experience of providing 1:1 support in mental health within voluntary or statutory settings

· Good knowledge and understanding of the diverse population in East London, their needs, complexities and the inequalities they may experience

· Experience of setting up and facilitating wellbeing groups and activities and delivering monitoring and evaluation activities in a 1:1 or group setting

· Experience of working collaboratively with local people, community, and statutory organisations with a working knowledge of the principles of co-production, involvement, and peer work within mental health services

· Experience of using skills and tools such as coaching or motivational interviewing techniques

· Demonstrable ability to engage with people from all backgrounds and a commitment to Equality, Diversity, and Inclusion practices.

Support Coordinator

We’re looking for an innovative, passionate, and professional individual with excellent communication and organisational skills to join our Stroke Recovery Services based in the Dorset East area.

There will be opportunities to hot desk within Bournemouth and Christchurch, as well as Poole NHS sites.

This is an exciting opportunity to work with stroke survivors and their families to provide Key Worker support following a stroke.

Position: S11166 Stroke Association Support Coordinator (x2 posts)

Location: Home-based, Dorset East however, Frequent travel will be required as part of this role (May include team meetings or other work-related meetings)

Hours: Part-time, 2 x 28 hours per week

Salary: Circa £21,391 per annum

Contract: This is a fixed-term contract until 30 June 2026. We have funding for this role until 30 June 2026.

The Role

The service aims to identify and address the needs of stroke survivors and carers across the stroke pathway, by providing a range of innovative support solutions, supporting them to meet their desired outcomes.

The Stroke Association Support Coordinator will:

· Support new stroke survivors and their carers from hospital discharge into the community.

· Provide personalised information, advice and support.

· Support stroke survivors to make informed lifestyle changes which will help them to prevent further strokes.

The Role

As a Donkey Welfare Adviser, you will promote good welfare for donkeys and mules by working proactively to support, guide and educate key audiences and by sharing best practice.

You will provide support to our Cost-of-Living Crisis Clinic Consultant to co-ordinate the efficient service delivery of our busy Cost-of-Living Crisis Clinics, used by clients who need assistance to make successful applications for: welfare benefits; social housing; lower utility and broadband tariffs; and crisis and general grants.   You will work with a wide range of people internally and externally, providing support and supervision to volunteer advisers and helping clients.

Key responsibilities of the role include but not limited to:

General Duties

• Being the initial point of contact; receiving calls and emails from clients and referring agencies, , and booking client appointments in an efficient and timely manner
• Setting up and clearing away after Cost-of-Living Crisis Clinic sessions, (currently, Tuesdays, at St Luke’s Community Centre, 90 Central Street, Islington, EC1, and Wednesdays at Westway Trust, 1 Thorpe Close, Ladbroke Grove, W10).  Sessions start at 9.00am and finish around 1.00pm
• Meeting, greeting and orienting (showing where the facilities are and H&S guidance) clients and volunteers
• Triaging clients on to our database and referring clients who are out of scope to other relevant organisations
• Assisting clients to complete online and paper forms both face-to-face and remotely
• Liaising in a positive manner with our stakeholders, eg. clients, other teams within Westway Trust, our funder, a City Law firm, and our hosts, Westway Trust and St Luke’s Community Centre
• Responsibility for monitoring our work, completing simple reports, writing case studies, making sure volunteers complete their data entry after advising clients, and running case management statistics using AdvicePro, our case management system) (training on AdvicePro will be provided)
• Recruiting, inducting, training and supervising volunteers both corporate (from the City) and lay (students and local residents)
• Casework; writing up case notes about clients helped
• Attending relevant meetings and training and keeping resources up-to-date
• General administration including: photocopying, scanning and record keeping, and coordinating the general day-to-day activities of the Cost-of-Living Crisis Clinics
• Effectively contributing to make our Cost-of-Living Crisis Clinics high performing in line with the values of Westway Trust

Appeals & Representation Duties

Responsibility for our legal appeals representation service, including:

• Triaging clients, including reading appellant’s hearing bundles to assess the merits of their case
• Case management of appellants’' appeal cases, (ie, smooth management of appellants' cases from inception to post-hearing)
• Attending hearings
• Guiding and case managing lawyers/law students on applications for leave to appeal/set aside to the Upper Tribunal/Judiciary, (from the Lower Tribunal).  Full training will be given

Qualifications: You will have or be working towards a law degree or equivalent.

• A good understanding of the law in any jurisdiction

To be successful as Specialist Housing Advocate you will need the below experience and skills:

You will have an excellent understanding of domestic abuse and its effects on survivors and their children, and of best practice within the domestic abuse and housing sectors. You will be skilled in risk management and safety planning, remaining calm in a crisis and in handling sensitive information on a daily basis. The post holder will hold a caseload of survivors and will also be required to support Advance’s duty team on a rota basis which will involve completing intake assessments and providing crisis intervention support.

Wellbeing Coach

The Role:
We’re looking for a bold and caring professional to join us as a Wellbeing Coach. If you have experience of supporting people with their mental health and wellbeing, this could be the job for you!
 
You will deliver predominantly one to one wellbeing coaching with some group support to both adults and young people. Ideally you will have some experience of using coaching and CBT techniques such as guided self-help, psychoeducation and thought challenging.

Job Purpose

Increasing numbers of Disabled people are becoming aware of the fact that CYCLING might be possible for them. However, information about how/where to access cycling, the variety of possible equipment, regulations around cycling a non-standard cycle, funding avenues, etc. can be difficult to find. As the Wheels4MeLondon loan scheme starts to be marketed in Spring/Summer 2024, we anticipate increasing numbers of enquiries to reach us, including from around the UK.

With the support of the rest of the Wheels for Wellbeing team, our Specialist Advisor will become very knowledgeable about all aspects of Inclusive Cycling and will support enquirers with information and advice about the most appropriate next steps with regards to their cycling ambitions. They will provide information and advice, based on where enquirers live, their level of experience with cycling, their goals (whether they wish to try out cycling, to borrow, hire or to purchase a cycle, etc.).

Where a caller wishes to attend one of our own cycling sessions, the Advisor will support them with using our booking system. Where a caller is ready to borrow a cycle through Wheels4MeLondon, the Specialist Advisor will support callers to request a loan via the Peddle My Wheels website. Peddle My Wheels staff will then deliver the cycle to them at the agreed place and time, provide cycle training where required, and collect the cycle at the end of the loan period. For other enquiries, the Advisor will signpost callers to relevant external organisations.

To be successful in this role you need excellent listening and communication, interpersonal and organisational skills. You will love acquiring knowledge (including through online research), storing information and sharing it. You need to be confident using record keeping systems, within our data protection & privacy policy and working with people from diverse backgrounds and with a range of accessibility needs.

Job Description

1. Answering enquirers’ queries and providing information. This will include:

a. Being first point of contact for enquiries into Wheels for Wellbeing’s phone / email.

b. Actively listening to understand the detailed nature of the query and responding through the most appropriate and accessible mode of communication, whether contact is made by phone, email, instant messaging or other methods of communication.

c. Sourcing information from the rest of the Wheels for Wellbeing team and/or researching specific information for callers (where appropriate, redirecting the query to colleagues within the organisation or signposting them to other organisations).

d. Ensuring that new, useful information is stored/posted to our website for use by future enquirers.

e. Supporting people with queries about how to access Wheels for Wellbeing’s services, including support with our booking system

2. Building accessible information resources (eg: website pages, briefings, FAQ page, etc.) about Inclusive cycling, such as:

a. Inclusive cycling opportunities providers in London and around the UK

b. Types & models of non-standard cycles and specialist adaptions retailers around the UK and overseas

c. Details of the Wheels4Me London’s scheme and other inclusive cycle hire/loan schemes around the UK.

d. Funding options for Disabled people wishing to purchase their own cycles

3. Effective liaison with all Wheels4MeLondonPlus partners for successful cycle loan experience. This will include:

a. Regular communication with the Peddle My Wheels team to ensure most effective referral methods (to be developed with your input, as the scheme gets off the ground during Spring/Summer 2024)

b. Take part in project team meetings with all partners (Sustrans London; Peddle My Wheels and the rest of the WfW team).

c. Collect and store data such as numbers & types of enquiries, case studies, etc., as required for reporting to the funder.

d. Contribute to short articles, social media posts, etc. for the promotion of Wheels for Me London and Inclusive Cycling in general.

Other:

-  Contribute to the ongoing development of Wheels for Wellbeing, team meetings and organisational priorities

-Be proactive in keeping up to date with developments affecting the role

-Abide by and promote organisational policies and practices, and charity’s values

-  Support diversity and equality of opportunity in the workplace

-Occasionally, with notice, work evenings / weekends

- Work at least once a quarter in an appropriate role at a Wheels for Wellbeing session
