Ideally, you will have previous experience of working with birth relatives whose children have been placed for adoption and you will have a good understanding of the adoption process.  You will work with relatives from a range of diverse backgrounds, lived experiences and those who may be neuro diverse and you will need to demonstrate understanding, respectfulness and adaptability to your social work practice.

ABOUT THE ROLE

We have a new opportunity for a specialist Dual Diagnosis Worker to join the team in Croydon on a 12 month fixed term contract. This is a new role to the team and you will deliver evidence-based interventions based on  holistic assessments to support personalised recovery, integration, and promotion of independent living skills. You will work to the long term support needs of our residents and work towards facilitating improved outcomes. 

Our service has 27 residents across 4 sites, which specialise in Forensic Mental Health aiming to enable and empower our residents to achieve their maximum levels of functional independence and enhance their sense of wellbeing.

Shift Pattern:  This role is full time 37.5 hours per week. Hours to be discussed, you will be based in service.

OVERVIEW OF KEY RESPONSIBILITIES

• Set up realistic and flexible strengths-based support plans, working closely with colleagues and support teams to support and advise on interventions and approaches to meet resident/participant needs.
  Include appropriate substance misuse interventions within support plans for those with co-occurring substance dependence and mental health need devising systematic plans for care; implementing this plan; coordinating the input of others and evaluating effectiveness.
• Undertake joint holistic assessments and risk assessments with residents/participants to help determine service and support needs and identify where these can be met. Provide outcome-based support measured against goals, recovery, and achievements.
• Encourage residents/participants to identify and define their own goals and work towards being resilient and independent as possible, support with reducing reliance on clinical interventions and hospitalisations. 
• Promote active involvement of those with dual diagnosis, assist residents/participants to identify their own strengths, personal interests and goals and support them to access and participate in activities of their choice.
• Undertake time-limited, SMART, and a range of evidence-based interventions such as 1-1’s, including harm minimisation, increasing awareness and motivation through stages of change and relapse prevention.
• Support in creating and maintaining a welcoming, psychologically informed home environment for residents/participants, ensuring a continuous focus on safety, health, and wellbeing.
• Positively promote and support connections with local communities and boroughs of SIG’s services and liaise with other teams to maintain and share knowledge and links to support people to access resources which include print, personal and digital.
• Develop community links and liaise regularly with a range of agencies and individuals across voluntary and third sector services in the community including but not limited to; drug and alcohol services, supported employment schemes, gyms and healthy living programmes, recovery colleges, and benefits and advice agencies.
• Act as a role model for staff of all disciplines and grades working with people with co-occurring mental health, alcohol and/or drug problems. 
• Share knowledge and skills to help enhance awareness around dual diagnosis and best practice, challenges, and services available with other services of Social Interest Group.

Other responsibilities than those described above may be required to be undertaken from time to time and will be expected to be performed to as long as it is within the capability and level of the position. 

KEY CRITERIA

What we are looking for:

• Knowledge, understanding and experience of working with people of complex backgrounds. Ideally ex-offenders, those with enduring mental health, substance misuse and challenging behaviours
• Experience of assessing needs and strengths, and supporting people with dual diagnosis and complex needs
• Previous experience in a similar role, providing holistic assessments and support for people experiencing mental illness who use substances
• IT Proficiency, including Microsoft Office, and the ability to navigate and learn new case management systems and other types of organisational software
• Able to recognise ill health and additional support and need for interventions
• Knowledge of the interactions between mental health problems and drug and alcohol use and the issues facing people trying to stabilise their situation
• Applied knowledge of the principles of risk assessment and risk management, particularly in relation to harm minimisation
• Ability to engage those who may be hard to reach, able to influence and negotiate positive outcomes with others

What we would like, but not essential:

• Experience in delivering training in motivational interviewing, Cognitive Behavioural Theory informed approaches and other relevant recovery models

Additionally, UK Reads works to facilitate a global pen pal project, inspiring young writers and offering them a platform to connect, fostering cultural awareness and enhancing writing skills.

About the Project

UK Reads: Literacy Impact East London specifically  focus on children and young people aged 11-18 years. The project is designed to reach young people from diverse, low-income backgrounds and ethnic minorities. We have observed that approximately 85% of our beneficiaries come from low-income families, 10% of our users are students with Special Educational Needs and Disabilities (SEND), reflecting the diverse range of learners we aim to support. Additionally, 45% of our beneficiaries come from Black minority communities, highlighting the importance of promoting inclusivity and addressing potential disparities in literacy outcomes.

We acknowledge that low literacy rates often vary through boroughs and are committed to reaching the most underserved, underrepresented, and marginalised young people across East London. We collaborate with schools and community organisations to implement a tailored intervention that addresses the specific needs of each group, offering a diverse range of services to expedite the development of foundational literacy skills. 

Key Responsibilities

About the Service

Our refuges provide emergency accommodation to women and children fleeing domestic abuse and sexual violence, last year nearly 1000 women and children found a safe place to stay in one of our refuges.  Refuge Case workers provide emotional and practical support this can include housing support, legal options, reporting to the police, benefits, financial matters and accessing therapeutic support services.

About the Role

You will be a keyworker for a caseload of (on average 6-10 women) living in or moving on from the refuge.  You will be with them from the start of their journey to the end, welcoming and helping them settle when they arrive to when they are ready to move on to their next home.  This includes carrying out risk and needs assessments and developing support plans.  Your work with service users will ensure their practical needs as well as emotional needs are taken care of. You also provide a vital role in keeping the service a safe and secure environment for the women and children who now call the refuge home. It is a face paced, target driven role.

About You

Key to this role is experience of working/volunteering with anyone who has experienced any form of social disadvantage.  A knowledge of domestic abuse and its impact on women and their children is important as well as an ability to build supportive and empowering relationships.  You must be open and willing to learn new skills which will build on your working practice with service users to co-develop and deliver support planning, risk assessment & linking in with other agencies where needed. 

Knowledge and experience of risk assessing, case management and legal remedies, housing and welfare benefits is ideal however not required as we are confident that we can train around these.

About the role:

As a Resettlement Worker and Domestic Abuse Specialist, you will work in person and remotely with clients who have support needs, in particular, those who are survivors of domestic abuse. In conjunction with other members of the Partnership, you will support our clients to engage with services and find accommodation solutions. About the Service

Ella House is an Accomodation Service that provides support to women who have experience Rough Sleeping, Trauma and Multiple Disadvantage.  The aim of the project is to provide support to women for around 18 months to build skills to be able to live independently. 

The service is a 24 hour service that runs on a shift pattern over the week and has a concierge service covering nights.  Staff will provide holistic support to service users looking at all their needs and putting a robust plan in place.

About the Role

The Activities and Wellbeing worker will provide both 121 and group sessions. Speaking with women to understand their needs and wants and developing appropriate session plans.  These can be wellbeing activities, skills based  or creative depending on what the women require.  There will be a small budget to accommodate activities.

About You

The ideal candidate will be passionate about working with vulnerable people from a strengths based approach.  An understanding of Trauma and how to work in psychologiacally informed environments is essential in this role.  Someone who is motivated and creative and able to build relationships with women and get them to engage in a variety of activities.

About the role:

The post holder will work within The Passage’s Community Services as a Complex Case Outreach Worker and will lead on case management for people experiencing homelessness who are presenting in the footprints in which the team operate and/or who are accessing the Resource Centre. The post-holder will support individuals with substance use support needs and/or mental health issues and will lead on work with clients who are experiencing alcohol and/or drug dependencies and who may also be experiencing mental health problems or other health or social care needs.

Main duties:

• To manage a caseload of clients presenting within the footprints in which the Engagement & Outreach service operate and individuals who are accessing The Passage Resource Centre. To support clients with complex needs, supporting them to identify and work towards recovery and away from homelessness.
• Carry out assessments, case management, support planning and the delivery of interventions for clients including those experiencing mental health and/or substance misuse issues who are multiply-disadvantaged in their homelessness.
• Provide assertive outreach and engagement with street-based service users presenting with complex needs and barriers to engagement. Carry out monthly street-based audits to inform reporting and analysis of service outcomes.
• Support clients who are rough sleeping or involved in day time activity in the footprints in which the team operate to end their homelessness by linking them in with appropriate internal and external support services relevant to their needs. To provide support to clients who are accommodated, but may require ongoing support in order to maintain this accommodation, linking them in to appropriate local services.

General responsibilities:

• In conjunction with your line manager to continuously develop the role to ensure that all tasks are being undertaken in an effective and appropriate manner which meets the strategic aims and objectives of The Passage.
• To participate in internal/external meetings as required, attend training events, conferences and other functions as necessary.
• To participate in regular supervision and annual appraisal, and help in identifying your own job-related development and training needs.
• To ensure that all The Passage policies and procedures are being adhered to, particularly those relating to Code of Practice and Confidentiality.
• To contribute to the effective implementation of The Passage’s Diversity and Equality Policy as it affects both The Passage and its work with vulnerable adults.

Desired experience:

• Significant experience of delivering support services to socially excluded clients in a street based/day centre or similar setting.
• Significant experience of working in a mental health and/or substance misuse setting, working collaboratively to achieve positive outcomes for clients.
• Experience of providing a fully sensitive and responsive service to clients in a psychologically informed manner.
• Experience of working cooperatively with external partner agencies, e.g. Police, Social Services, NHS, as well as housing providers and voluntary agencies.
• Experience of assessing client needs and supporting clients to address their needs through assertive outreach and case management.
• Experience of working in a client-centered recovery focused approach and the necessary professional social skills to initiate and maintain constructive and appropriate relations with clients and agencies.

• Understanding the support needs of the street populations with complex health, and housing needs.
• Knowledge of services and legislative environment regarding housing, health, work and immigration as it relates to people experiencing homelessness.
• Ability to communicate, both verbally and in writing and collate and evidence work and outcomes using case management databases.
• Knowledge of Psychologically Informed Frameworks.
• An understanding of the causes of homelessness and, in particular, the needs of people who are multiply-disadvantaged in their homelessness.
• Knowledge of housing pathways available to dual diagnosis clients and how to access them.
• A working knowledge of how to support people with either substance or mental health needs.

We have an exciting new opportunity for a Social Prescribing Link Worker to join our team in Merton.

The Merton Social Prescribing service is one of the most established and mature services in the UK and is celebrating its 7th anniversary this year. We are seeking an individual to join our successful service in Southwest London. This role will help to strengthen the links between Primary Care, voluntary and public services to improve patients’ health and wellbeing. 

You will support patients within Primary Care in Merton, and you will support each patient to access appropriate services provided by voluntary and community organisations and other agencies. We expand the options available in Primary Care by creating a ‘managed’ way for referrals of patients with social, emotional and/or practical needs to a variety of local non-clinical services in the community. These include leisure, social activities, education, welfare, housing and employment advice and support.

About the role

We are now looking for an experienced and innovative individual to support the engagement and outreach of our Young Women’s Service.

The successful candidate will be experienced in developing and delivering creative Gender Based Violence (GBV) prevention programs and confident to successfully engage young women in a wide range of settings including schools, colleges, youth programs & pupil referral units to raise awareness, challenge and positively influence the attitudes and behaviours that normalise GBV in this peer group. You will receive training on delivering GBV Resilience Programs that build psychological and emotional resilience in young people (YP) to strengthen protective factors to promote positive mental and emotional well-being in YP. You will co-produce and deliver training and awareness raising work for professionals, including first responders and managers in GBV related services / mental health provisions, educational centres and non-abusing family members and carers to facilitate appropriate response towards YP at risk. 

About you

You will therefore be skilled in nurturing and developing YP voices and active engagement within such forums. You will be skilled at working alongside young women with an acute awareness of how to successfully co-create and co-deliver training packages to professionals and the wider community, thus centralising YP as the leaders and change agents in the project.

Regional Youth Worker - London

Location: Flexible, based at our office in Aldgate East, with travel throughout the London region and occasional remote working.

We are looking for an experienced Youth Worker, to deliver training sessions and projects to young people through youth centres, community groups, schools and alternative provisions across the London Region, and to ensure our programmes are fit for the future.
 

We are an award-winning national charity which trains over 9,500 young people affected by street violence each year in emergency first-aid through a team of 350 healthcare volunteers and youth workers. We empower young people with the skills to become part of the solution to violence by developing the skills and confidence to save lives and increasing their understanding of the medical and psychological consequences of violence.

As the StreetDoctors Youth Worker in the Region, you will deliver high quality training, develop new partnerships and maintain positive relationships.
