Health & Wellbeing Coach - Adults

Post holders will work in partnership with clinical and non-clinical, social care and voluntary sector colleagues to ensure the best possible outcomes for patients. The role is varied, and will include multi-disciplinary working, case management, self-management education through 1-2-1 consultation, group facilitation and facilitation of peer support.
