Advisor and Caseworker - Cardiovascular Social Prescribing 

Location: London 

Contract Type: Permanent/Part time 

Salary: £32,082 per annum (pro rata to 0.6 FTE) 

Hours: 21

Our client is collaborating with the East London Cardiovascular Prevention Group (ELoPE) to integrate social prescribing into secondary care for patients of Barts Health. This initiative seeks to link individuals with non-medical support to enhance their physical and mental wellbeing, fostering connections with their local communities and empowering them to take control of their health. 

Project Objectives: 

This project is designed to tackle health inequities and support individuals who could benefit from: 

  • Assistance with social issues and the social determinants of health, including debt, housing, employability, and food insecurity. 

  • Greater community connections through engagement in physical, social, and creative activities. 

  • Accessing and engaging with statutory and voluntary sector services. 

Role Description: 

The Advisor and Caseworker will offer social welfare advice to individuals referred via the Barts Health Cardiovascular Team. The role will focus on what matters to the individual and take a holistic approach, providing advice on welfare benefits, housing, debt, and energy. They will refer individuals to specialist agencies for issues such as immigration, 

legal advice, and debt, as well as support them in accessing various community services and activities.
