What you’ll be doing?
You will be part of the MYcommunity Linkworkers team. You’ll be responsible for:

Provide personalised brief intervention and social prescribing support to individuals, their families and carers.

Meet clients on a one-to-one basis, making home visits where appropriate and allowing clients the time and space to discuss ‘what matters to me’.

Provide person-centred support to enable clients to identify their health and wellbeing needs and goals. Co-produce support plans with clients to discuss what’s important, where they would like to be in the future and how we can support them to achieve this.

Manage and prioritise a caseload to ensure clients' needs, priorities and urgent support are met.

MYcommunity Lambeth works in partnership with a large number of statutory and community services and organisations across the borough. You will link clients with appropriate services to meet their needs. It is vital to have a strong understanding of when it is necessary to refer someone back to health care professionals/ agencies when the client’s needs are beyond the scope of the MYcommunity team.

We are seeking a sessional Project Worker to facilitate our structured, psychoeducational groups alongside our peer support volunteers. We are seeking a Sessional Project Worker to join our Perinatal Outreach service, to co-facilitate group support for perinatal clients (pregnant women and birthing people of children less than 2 years old) within the London Boroughs of Bromley and Greenwich.
You will help to support women under the care of the Oxleas Perinatal Mental Health Team with moderate to severe mental health challenges who have been identified as requiring additional practical and social support.
You will attend a regular programme of perinatal support groups and activities that encourage women to access outreach support within the local community, develop social networks and reduce isolation as part of their recovery plan. Current activities include weekly play sessions, walking group, soft play, picnics and monthly outings.
Applicants should have a good understanding of mental health challenges in the perinatal period alongside experience of responding to emotional crisis and managing risk. An ability to adapt to multiple and diverse needs, to be proactive and work independently but collaboratively is essential for this role.

Main purposes of job
To provide a supportive role to service users and ensure the security and health and safety of our accommodation services. Staff must stay awake stay awake for the duration of the shift.

The Role
The purpose of this role is to support and empower Stroke Association Support Coordinators to deliver high quality stroke support services, ensuring stroke survivors and their carers receive the best possible support to rebuild their lives after stroke.

The Role
Reporting to the Service Delivery Coach, the Stroke Support Coordinator will deliver high-quality, person-centred support to stroke survivors and their carers/family members across Sheffield.
You will have the ability to support stroke survivors to identify their goals. Using strong assessment skills and a person-centred approach enabling them to maximise their recovery and improve communication.
Key responsibilities will include:
· Supporting newly diagnosed stroke survivors and their carers from hospital discharge into the community.
· Supporting a diverse caseload including anyone experiencing communication or cognitive difficulties
· Providing support via a combination of face-to-face visits, telephone calls, emails or letters and digital methods (such as video calls).
· Using a person-centred and person first approach to provide specific, tailored information, advice and support to stroke survivors and their carers.
· Empowering stroke survivors to make informed lifestyle changes which will help them to live life well after stroke.
· Working with other health and social care professionals across the stroke pathway to ensure high quality support.
· Working from home but as part of a team of coordinators.
· Delivering stroke support groups in the area.
· Visiting stroke survivors in hospital to introduce the Stroke Recovery Service and provide information and support.
About You
You will have:
· Experience working with people within a health and/or social care profession or other caring capacity.
· Experience of maintaining accurate records using IT skills and database systems.

The Role

As a Trainee Project Worker/Young Person's Worker, you will be based within a project team, working directly with clients or young people to build trust, create safety and support meaningful progress. You will help people settle into the service, engage with support, develop practical life skills, manage day-to-day challenges and take steps towards greater independence. Whether you are supporting adults or young people, you will play an important role in creating a consistent, encouraging environment where people feel listened to, respected and able to move forward.
As a Trainee Floating Support Worker, you will travel across London to meet clients in their homes and communities, tackling housing issues, supporting health and wellbeing, and helping them achieve personal goals. The work is varied, fast-paced and deeply rewarding, with every shift – whether early mornings, late evenings, weekends or bank holidays – bringing the chance to see real change happen.
In return, you will be part of a comprehensive and engaging Trainee Programme, with support around you from day one. You will learn through trainee-specific workshops, formal training, reflective practice, service visits and hands-on experience alongside knowledgeable colleagues across SHP. Over the course of the traineeship, which usually runs for up to 9 months with the possibility of fast-track at 6 months, you will build skills in areas such as housing, benefits, mental health, safeguarding and recovery-focused support. As your confidence and practice grows, your salary will increase at six and nine months, recognising your development and achievements as you build a meaningful career helping people move from homelessness and crisis towards safety, stability and independence.
