The postholder will lead, develop and motivate a team of support practitioners to ensure that immediate pastoral resettlement support packages are developed and delivered to meet the specific individual needs of each tenant, which ultimately facilitates their moving towards living with a greater independence. 

To take overall responsibility for the successful running of the support service as agreed. In line with the requirements of our regulatory bodies and company policies and procedures. To promote and ensure high standards of support and involvement opportunities for our tenants. 

Key Responsibilities 
 

• Provide support, guidance and training to support team to ensure they are equipped with the tools and skills to deliver high quality support and involvement programmes in their area. This includes ensuring that ACH’s internal quality framework for Involvement best practice, and relevant policies and procedures are followed.Train support team in Support Processes to ensure that Support Plans, Risk Assessments, Reviews, Exit Interviews are carried out within the required time frames.
   

• Carry out monthly one to one’s with staff and coordinate weekly or bi weekly staff meetings. 
   

• To line manage staff team and act as the first point of contact and to effectively manage, supervise and develop the support staff and any apprentices or volunteers. 
   

• Support the Housing Team in receiving new referrals and these being processed to ensure empty rooms are let. Work with the Housing Team to ensure that residents are aware of their responsibilities regarding rent payments and licence agreements. 
   

• Work with other ACH teams to maintain effective internal referral procedures for residents to engage with wider ACH services such as Training and Employability Support 

• To maintain an excellent standard of safeguarding practice and awareness throughout the service and staff team and to support staff on safeguarding issues. Working in line with ACH’s Safeguarding Policy to follow up any safeguarding related issues and ensure they are dealt with effectively with appropriate referrals to safeguarding boards, local authorities social services and where necessary the Police. 
   

• To provide and seek training, support and guidance for staff and to implement staff training programmes. Ensuring all staff have completed any mandatory training and any training specific to service delivery.

• Ensure that staff maintain Continuing Professional Development logs that record trainings undertaken. 
   

• Monitor support service ensuring that support plans and risk assessments are reviewed, evaluated and updated regularly. To ensure all paperwork is completed to a high standard and clear and concise records are maintained.
   

• Regular monitoring of CRM, making use of data reports to ensure that support outcomes are being reached.

• Carrying out quality audits of support service and report findings to management and make recommendations for service improvements. To ensure best practice through internal auditing and self-monitoring. 
   

• Provide regular information and reports for Committee and other stakeholders to demonstrate the impact of activities and outcomes achieved by the support team.

• Complete monthly Support Service KPI Reports in line with ACH reporting procedure.
   

• Recruit, induct, manage and motivate employees within the team, ensuring they are trained in the performance of their duties and appraised of their performance; organise, supervise and monitor their performance, thereby ensuring the provision of efficient, effective and high quality services. Dealing with all personnel issues in conjunction with the HR Manager as necessary.Maintain personal records on the HR database, manage holiday requests. 
   

• Represent ACH professionally to external stakeholders. 
   

• To work with the Support Service Manager and Head of Support and Housing to ensure effective administrative systems and procedures relating to the effective running of the support service are in place.
   

• Where needed to cover support delivery for a small, designated caseload of residents. 
   
   

• If department is IAA registered maintain high-quality case management system in line with IAA Code of Standards.

Role overview 

Shine supports a community of over 15,000 members living with spina bifida and/or hydrocephalus, including 10,000 adults (25+).

You will be delivering high-quality support and creating opportunities for adults living with spina bifida and/or hydrocephalus and their carers, supporting Shine members to lead healthy, independent, and fulfilling lives by improving condition management and fostering connections within the Shine community. 

This role will focus on adults (25+), you will be primarily working within the Adult Services Team. However, there will be occasions when you will work across age groups to ensure the best outcomes for our members. 

The role is home-based and ideally you will live within an hour commute of London, within Essex, Kent or Sussex as you will be attending clinics in London and the Southeast as required. About you: We are looking for someone with at least two years’ experience working with diverse groups of young people, with excellent communication, teamwork and organisational skills. You will be able to build strong relationships with young people and adults alike across a range of settings, influence and motivate others, and contribute positively to the wider vision and goals of the charity.

You will have knowledge of gardening or horticulture and the confidence to plan and deliver engaging practical sessions with young people. About the role:

We are looking to appoint a Family Support Worker on a full-time basis (35 hours, 5 days per week), to deliver a high-quality family support service as part of our North East Care Team to families in the local area, at a time where they need it most.

Reporting to the Family Support Manager of the Southampton Care Team and working in partnership with health, education and social care professionals, you will take responsibility for providing needs-led emotional, social and practical support to families where a child/young person has a life threatening or terminal illness. You will enjoy responsibility for managing your own schedule, remaining flexible to the needs of the families on your caseload.

Having worked in a stressful and emotional environment you have a genuine interest in building supportive relationships and helping people; and having provided bereavement support to families, you understand processes of grief, loss and change - and how best to help others deal with its impact.

·An experienced child health, education or social care professional - applications will be particularly welcome from those who have worked in a community environment and those with a recognised qualification in education, health or social care.

We're hiring a Complex Needs Recovery Worker to join our team based in Penrose Focus. In this role, you will support residents who have a history of forensic mental health issues with co morbidity, often alongside co-occurring substance misuse disorders. The residents range from high risk to low support and our aim is to provide tailored support to help them reintegrate into the community.

• Provide person-centered support to residents, helping them rebuild their lives and transition back into the community.
• Support residents to build skills and confidence and signpost residents to external organisations and services that can aid their recovery, such as drug rehabilitation programs, psychological therapy and social services.
• Monitor residents’ wellbeing, carrying out regular welfare checks, and responding to any incidents or emergencies.
• Build good relationships with residents, offering emotional support and fostering a safe, non judgmental environment.
• Confidently report issues to the police when necessary and work closely with probation officers to ensure residents meet their legal and rehabilitation requirements.
• Create an empowering and enabling environment where individuals are supported to make informed decisions and take control of their own lives.
• Support residents in accessing primary healthcare services, including arranging GP appointments and liaising with mental health professionals. 

About You

We're looking for a genuine passion for supporting people who may have complex needs which includes forensic mental health. You will be a self motivated individual who is able to be proactive in managing your own time during busy or quiet periods to ensure productivity. Experience in mental health or support work is desirable but not essential, as full training will be provided. You will be able to work collaboratively as part of a team, and will have alignment with our SIG values. You will be keen to support our residents to achieve their individual goals and support them to be the best versions of themselves. 

• Previous professional or lived experience supporting people with forensic mental health and complex needs, ideally in mental health and residential setting
• Experience working with people with complex backgrounds, including ex-offenders, mental health, substance misuse, or challenging behaviour and an understanding of housing and social needs
• Ability to engage with people from different backgrounds, build rapport, and empower them to achieve their personal goals
• Ability to show empathy and compassion and the different challenges people face; ability to come up with creative solutions to support them in their journey
• Ability to use, learn and adapt to IT at an intermediate level, including Microsoft and other software programs
• Ability and willingness to show flexibility of working patterns, responding to the needs of the service and residents
• Ability to promote the service and provide outreach based provision, with an ability to liaise and work effectively in partnership with stakeholders
• Understanding of the different needs people with multiple and complex needs have and be aware of the social marginalisation that can be attached to people who face personal challenges

The service provides low, medium, and high level support to women who have experienced homelessness, criminal justice history, substance use, mental health, and domestic abuse. As a Recovery Worker, you will be at the forefront of providing person centred support, to help our residents overcome personal challenges, and meet their desired goals/outcomes. You will do this through various support plans and interventions, running and participation of activities, and working alongside various internal and external partners. 

Key Responsibilities include: 

• Initiate appropriate interventions to prevent crisis or other risks.
• Ensure service responsibilities and requirements are carried out effectively.
• Develop, participate in, and encourage residents to participate in the running and development of various projects and activities.
• Empower residents to make decisions to take control over their lives, by creating an enabling environment.
• Provide advice, information, guidance, and life skills training to residents as required.
• Recognise signs of deteriorating mental health, physical health, or other health implications.

As our Café Team Member, you'll play a key role in helping people take their next steps into work in the hospitality industry. Reporting to the Fruitful Café Project Manager, you'll support our trainees development, work on the day-to-day Cafe operations, and ensure our customers feel welcome. 

You'll work closely with the Fruitful Development Worker and our café crew to help trainees build their skills, confidence and experience in a real working café. Every day, you'll support up to four trainees as they gain paid work experience in hospitality while serving around 50 customers.
