Job Purpose:
To provide specialist support to individuals affected by neuroendocrine cancer—including patients, families, carers, and health and social care professionals—through expert nursing advice, psychosocial support, and the provision of dedicated advocacy and self-management services.

• To deliver accurate, evidence-based information and guidance on neuroendocrine cancer.
• To advise on self-care and promote physical and mental well-being, including appropriate escalation pathways (e.g., GP, CNS/CPN, clinical team, emergency services).
• To contribute to the delivery of direct care and support services, including the national helpline, support groups, and access to counselling and therapy.

Key Responsibilities

Requirements

• Must have a current NMC registration with no restrictions on their practice or caution order attachment.
• Min 5-year post reg. experience, within a clinically relevant field: including caseload management
• Be able to provide at least two professional references from your last place of work as a nurse or midwife.
• Please note that a Disclosure and Barring Service (DBS) check will be required.
• Demonstrate a willingness to attend all mandatory training relevant to their role
• Must be flexible to work locally or remotely, and willing to travel to attend support groups, conferences, and events.
• Minimum 1 day/week helpline +/- cover as needed.
• Mandatory staff meeting days in the office (Leamington Spa) 4 times a year.

Administrative Skills

• Proficient in Microsoft 365 Office applications
• Proficient in accurately entering complex data into secure electronic systems.
• Be able to demonstrate full awareness and compliance with legal and professional requirements (e.g., GDPR, NMC), reporting appropriate concerns through line management to the Data Protection Officer.

Professional Experience & Knowledge

• Demonstrable knowledge and experience within relevant clinical speciality.

Main Purpose of the Role: 

To provide proactive, emotional, and practical support to families and individuals affected by Duchenne Muscular Dystrophy (DMD) across England. The role will focus on early engagement, wellbeing assessment, community building, and delivery of workshops and events, while ensuring accurate tracking of support outcomes and data.   
  
The successful candidate will also lead on support for our End of Life and Bereavement projects, working collaboratively with healthcare professionals, palliative care teams, and hospices to ensure families have access to appropriate services and resources. 

  

Specific Tasks: 

1. Proactive Family Support  

• Initiate contact with newly registered families within agreed timeframes (email within 3 working days, call within 7–10 working days)  

• Contact to be made to all registered Action Duchenne members, knows to us in the Regions you will be covering, to ensure the current support offer is clear   

• Provide ongoing support tailored to individual needs, including emotional wellbeing, physical health, housing, financial security, self-esteem and respect, decision-making, social engagement, quality of life, and access to care  

• Use the Action Duchenne Assessment Form and Action Plan to identify and respond to areas of concern  

2. Advanced and End-of-Life Care Support 

• Provide compassionate, practical, and emotional support to children, young people, and adults living with Duchenne, and their families, as they navigate advanced stages of the condition and end-of-life care. 

• Work collaboratively with healthcare professionals, palliative care teams, and hospices to ensure families have access to appropriate services and resources. 

• Develop and share guidance, resources, and workshops to help families prepare for and manage advanced care planning, symptom management, and emotional wellbeing. 

• Support families in understanding available options for care settings (home, hospice, hospital) and facilitate informed decision-making. 

• Ensure continuity of care and communication between multidisciplinary teams, while respecting family preferences and cultural considerations. 

• Offer signposting to bereavement support and follow-up services for families after loss. 

3. Wellbeing Tracking and Outcome Measurement  

• Administer wellbeing questionnaires and record scores across key domains (e.g. physical health, emotional wellbeing, financial security)  

• Collaborate with families to co-create action plans and track progress  

• Ensure all data is entered into CRM (E-Tapestry or similar) within the allotted timeframe, i.e. immediately after or during the call.  

4. Community Engagement and Event Delivery  

• Organise and deliver regional meetups (minimum one per quarter)  

• Facilitate support groups (virtual and in-person) for parents, young people, and extended family (minimum one per quarter)  

• Support delivery of workshops and events aligned with programme schedule (e.g. music, life skills, employability)  

• Actively participate in networking events to promote Action Duchenne within our communities, build relationships, and drive growth in membership by encouraging registration of families affected by Duchenne 

5. Stakeholder Collaboration  

• Liaise and develop relationships with external organisations including NHS care advisors and clinics, local authorities, counselling services, and other charities  

• Represent Action Duchenne in across your regions and build relationships with local networks  

6. Administration and Reporting  

• Maintain accurate records of all interactions and support provided  

• Contribute to quarterly reporting on activity delivery, capacity utilisation, and family impact  

• Support development of CRM processes and service delivery improvements  

7. Key Performance Indicators (KPIs):  

• New contact acknowledgement email: within 3 working days

• New contact follow-up call: within 7–10 working days 

• Families contacted per week: 12–15 hours of direct contact

• Regional meetups: 1 per quarter

About the Role

We're looking for a compassionate and driven Service Manager to join our residential service based in Havering. This role supports residents in temporary housing who are facing challenges with substance use. As a Service Manager, you’ll lead a dedicated team, making sure we deliver high quality support, performance and improvements across our service whilst representing values of the organisation. You'll play a hands on role in supporting individuals facing addiction, homelessness, and complex needs guiding them towards stability, independence, and brighter futures.

The service focuses on providing a 24 hour, 25 bed accommodation service to residents who are deemed to be high risk. They support those who are experiencing homelessness. All admissions are jointly agreed, and placements reviewed at a weekly Complex Needs Panel chaired by the local authority. Residents are agreed on the basis that they are:

• Overseeing support related to substance misuse, ensuring effective interventions and pathways for service users
• Line Management and Leadership, supporting your team providing advice, guidance and support throughout their employment lifecycle
• Support the Service Manager in leading the day to day operational delivery of the service
• Risk Management ownership, ensuring processes and policies are followed
• Property and Housing Management, ensuring the accommodation meets our requirements, and repairs and maintenance are regularly reported and completed

About You

We are seeking a passionate, driven and motivated colleague to lead this service in a trauma informed and inclusive way. Someone who is knowledgeable in the needs of our residents, particularly substance misuse and can support the team in delivering excellence. You will be driven with passion for what we do, and be able to work flexibly to meet service needs, providing new ideas and opportunities to develop the service further to continue to achieve excellence!

• Understanding and/or practical knowledge of the social and societal marginalisation that can be attached to people with substance abuse issues, addiction, exploitation, homelessness and within the criminal justice system

• Experience of working with confidently and ability to provide advice, lead a team on all aspects of the service such as resident related enquiries which can include housing, substance abuse support, signposting

This role does not involve coding - instead you’ll develop and design clear ideas and plans with Unifrog’s two co-founders, and with other colleagues involved in platform development, and then you’ll work with our lead programmer to make them happen.

You will become an expert in:

• The needs of all our different user types, including students, teachers, school groups, universities, employers, and parents

• How to navigate the platform as a user of each type

• How the Unifrog platform works behind the scenes

You’ll be part of the team that:

• Collates feedback on the platform, deciding which changes to move ahead with, and how
   

• Comes up with and designs ideas for platform improvements

• Develops and designs completely new tools

• Investigates potential bugs and comes up with suggested solutions

• Communicates platform changes to the Unifrog team

• Answers queries about the platform from colleagues

 

Working together

You’ll regularly be working with:

• Unifrog’s two co-founders and other colleagues involved in maintaining and developing the platform – in particular our data and content teams – to come up with improvements to the platform and to develop new projects

• Our lead programmer to implement ideas

• User-facing colleagues to understand what our users are asking for, as well as communicate to them what things have changed on the platform

• (More occasionally, but especially at the beginning) our users, when you need to have a greater understanding of a particular topic

You will be line-managed by Unifrog’s two co-founders.

 

Skills and characteristics

We are looking for an independent worker with design skills, who is a great problem solver and is motivated to find an elegant solution, whatever the issue.

Design

You’ll be producing the designs that go to our lead programmer, and which he’ll use as the basis for his work. You should have experience with UX design, and we can accommodate whichever application you prefer to work with, eg Figma or Sketch.

We’ve developed and plan to keep developing a large range of tools on the platform, like:

• Quizzes

• Tools to make large banks of information easy to search and understand

• Tools to simplify complex workflows

• Integrations with other platforms

• Document creation that involves input from multiple people

• Data visualisation

The problems we come up against can involve:

• Different users handling the same processes in different ways from each other

• Different groups of users having competing wants

• A data provider changing the way they structure their data

• Users not completing an important action on the platform

• A change on one tool having a knock-on effect on several other tools

• Users expressing their needs and ideas in uncertain or ambiguous ways
   

Your challenge is to design concrete solutions that work for everyone, which are as simple as possible to implement, and which are immediately understandable by a user who’s never seen them before.

Independence and initiative
 

You will work closely with Unifrog’s two co-founders. While colleagues from the wider team will be involved in coming up with ideas and telling you what they need, you will be the only team member, apart from the co-founders, in the product development team.

Because Unifrog’s co-founders work remotely and are also involved in other areas of the business, this will require you to be a good independent worker:
 

• Able to plan a project and stick to timelines, even when you need to get input from other team members

• Confident to come up with your own ideas, sort out good ideas from less useful ones, and make decisions based on information you’ve researched or been given by other team members

You will need to communicate what development work is happening to the rest of the Unifrog team, and you’ll need to help your colleagues find the best way to tell users about it in turn.

Finally, you will need to be able to adapt your communication style depending on the person you are communicating with, including our lead programmer, other Unifrog team members, or partners and users.

The role will deliver the Walking Together Project across communities in north London, Slough and Reading. By providing the proper support, patients can be discharged sooner, and the risk of readmission is significantly reduced.
