Job description

Are you passionate about making a positive impact in the mental health sector? Do you thrive on promoting health equity and ensuring everyone receives the care they deserve? We are looking for a dedicated individual to join us as an SMI Health Checks Promotion Worker in Harrow.

Role Purpose:

  • Increase the number of people with severe mental illness (SMI) receiving annual physical health checks
  • Raise awareness about the importance of these checks among patients, carers, GP's, and third sector providers
  • Reduce barriers to accessing annual physical health checks for those with SMI
  • Coordinate personalised care planning, engagement, and psycho-social support for NICE-recommended interventions

Key Responsibilities:

  • Promote awareness of SMI annual physical health checks
  • Coordinate marketing & engagement activities
  • Receive referrals and assist patients in arranging appointments
  • Identify and address barriers to access
  • Co-produce personalised access support plans
  • Collaborate with stakeholders to improve systems and protocols
  • Monitor, evaluate, and provide insights for commissioners
