Job description

About the role:

We are seeking additional staff to join our experienced and committed teams within our Complex Needs Service in Westminster.  You will be in a specialist role designed to work with people with complex support needs, relating to mental health, multiple physical health needs, poly-substance use and offending.

As a Project / Support Worker Complex Needs, your role will be to provide support to clients to enable them to understand their support needs and to manage these effectively.  You will hold a caseload of clients and be responsible for helping them to identify short term objectives, meet their goals and complete risk assessments. You will also support clients to develop their living skills, develop and manage relationships, build social networks and to access various services.

About you:

  • Previous experience of working in the health and social care, supporting vulnerable people, rough sleepers or people with complex mental health, physical health or substance use support needs. 
  • An understanding of working with people using person-centred approaches. 
  • The ability to manage a caseload and deliver positive outcomes utilising your knowledge of the health and social care sector. 
  • A flexible and creative approach to working with a sometimes hard to engage client group.

Flexibility is required, as you will be working a rota inclusive of early shifts, late shifts and bank holidays.
