About The Role  

As a Community Builder, you will be at the heart of our charity’s ambition to reach seldom-heard members of the Armed-forces Community (AFC) and engage them in creating positive action that strengthens wider connections. 

You will identify the strengths, passions and interests within the AFC, foster meaningful connections with the wider community, and utilise these to create meaningful and sustainable outcomes.  

About You  

You will be a highly motivated and dynamic individual who is passionate about collaboration and community-led change.  You will have experience of engaging and supporting people in a community, charity, education, social care, housing, youth work, or other people-focused role using a range of facilitation tools, techniques and Community Development skills. You will need experience of working in a community setting with a diverse range of people and interests. You will have good organisational skills, energy and the ability to listen, build relationships, and inspire collaboration, which will be essential in creating positive, lasting change.

We are looking for someone with:  

• A Community Development qualification and/or transferable Community Development skills and experience.

• A basic understanding of the Asset-Based Community Development approach (ABCD) is required however full support and development of this practice and our internal ways of working will be provided.    

• Strong communication and interpersonal skills, with the ability to build relationships that drive action.  

• A track record of successful collaboration with internal and external partners and stakeholders.  

• Effective and efficient organisational and IT skills. 

PLEASE NOTE:  

• The successful candidate will need to be based within a 30-minute commute of their allocated locality and there is a requirement to travel regularly across the wider area (travel expenses covered).  The successful candidate must possess a valid full UK driver’s license and have access to a reliable vehicle with business insurance for work purposes.  

• There will also be a requirement for occasional travel to other UK locations for meetings and events depending on the charity’s needs such as our annual all colleagues in person event.   

• The successful candidate will work 35 hours per week over 5 days, Monday - Sunday. Core working days will be Monday to Friday; however, flexibility is essential, as regular evening and weekend work will also be required to meet the needs of the role. Working hours will vary to ensure a total of 70 hours is covered over a two-week period, so please take this into consideration before applying.

Richmond Carers Centre has the mission to help unpaid and family adult carers to live a full, active and meaningful life by providing them with advice, information, activities, breaks and emotional support either face to face, by phone or by email to enable them to live well.

As the Adult Carers Activities Coordinator, you will directly deliver our leisure break activity and training programmes. You will plan, deliver, promote and review a programme of activities for adult carers, including outreach activities and leisure events, as well as group work to help enhance carers’ health and wellbeing. The core aim of the activity programme is to provide carers with a much needed break from their caring responsibilities, in a safe and social environment where they can meet other carers and focus on their wellbeing.

You will be someone:

• Has experience of leading/planning activities across a range of subjects/activities

• To work with relevant professionals to deliver a programme of wellbeing activities, including yoga, Pilates and complementary therapies.
• To recruit, train, support and manage volunteers and paid sessional staff to contribute to the delivery of a selection of the activities and workshops.
• To research and continually develop the activities and training programmes to ensure it is responsive to carers needs.
• To create promotional material to promote activities and training opportunities to carers registered with Richmond Carers Centre.
• To work with Adult Carers Support Team Leader to ensure activities are within budget, achieving targets and meeting the requirements of funders.

The Independent Monitoring Service is seeking to recruit professionals with experience of working with children and vulnerable adults to the role of Independent Visitor.

We are looking for individuals all across Greater Manchester areas, including but not limitted to: Oldham, Rochdale, Tameside, Central Manchester, Bolton.

The role of Independent Visitors is to provide visiting services on behalf of NYAS which are consistent with the National Quality Standards for Children’s Homes, Residential Special School Standards, Welsh National Minimum Standards, Health and Social Care Standards Scotland and CQC regulations. The main duties are:

• To undertake monthly visits to identified homes in accordance with the requirements of the relevant NYAS manager.

These visits will be:

Regulation 44 visits to residential children’s homes, short breaks and secure units.

Regulation 25 visits to residential family centres.

Care Quality Commission visits to residential adult homes.

Care Inspectorate Wales regulation 8 visits to residential children’s homes.

RSS20 visits to residential special schools.

Health and social care standards Scotland residential children’s homes.

Monitoring visits to unregulated provision.

• To undertake all tasks as identified by the relevant NYAS manager, during the visits and thereafter, in order to fulfil all aspects of the role.
• To provide factual reports within set timescales as required by the relevant NYAS manager.

Candidates should have demonstrable knowledge and understanding of regulated services and their inspection frameworks and also knowledge of current legislation and statutory guidance relating to children, young people and adults at risk particularly those in residential care.

The Role

Social prescribing link workers will work as a key part of the primary care network (PCN) multi-disciplinary team. Social prescribing can help PCNs to strengthen community and personal resilience and reduces health and wellbeing inequalities by addressing the wider determinants of health, such as debt, poor housing and physical inactivity, by increasing people’s active involvement with their local communities. It particularly works for people with long term conditions (including support for mental health), for people who are lonely or isolated, or have complex social needs which affect their wellbeing.

Responsibilities include:

• Working with direct supervision from the Programme Lead/Senior Link Worker and input from the PCN’s Clinical Lead GP, take referrals from staff in GP practices within the local PCN.
• Provide personalised support to individuals, their families and carers to take control of their health and wellbeing, live independently and improve their health outcomes, as a key member of the PCN multi-disciplinary team.
• Developing trusting relationships and taking a holistic approach, based on the person’s priorities and co-produce a simple personalised care and support plan to improve health and wellbeing, introducing or reconnecting people to community groups and statutory services.
• Work collaboratively with all local partners to contribute towards supporting local community groups and organisations, making themselves aware of local resources and “assets” through mapping and then building a relationship with these organisations.  Also sharing intelligence regarding any gaps or problems identified in local provision with commissioners and local authorities. 
• Social prescribing link workers will work collaboratively with GP’s and have a role in promoting social prescribing and educating non-clinical and clinical staff within their PCN multi-disciplinary teams on the benefits of social prescribing
• Raise awareness of other services available within the community and how and when patients can access them. Linking into the work of Neighbourhood Network Schemes, locality Adult Social Care & Community Mental Health Teams.

About You

As a Social Prescribing Link Worker, you will have a Level 3 qualification in a relevant field (Health & Social Care, Health Trainer, Health Improvement, Community Family Worker, Psychology) or be working towards/willing to work towards.

You will have:

• Demonstrable commitment to professional and personal development
• Experience of working directly in a community based context (including unpaid work)
• Experience of supporting people with a mental health need and or complexities/vulnerabilities
• Experience of delivering one to one support
• Experience of partnership/collaborative working and of building relationships across a variety of organisations
• Knowledge of VCSE and community services in the locality
• Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities, individuals, their families and carers
• Understanding of the barriers faced by those living in isolation
• Knowledge of IT systems including ability to use word processing skills, emails, the internet and able to create simple plans and reports
• Ability to complete paperwork, keep records and carry out own administration

The Independent Monitoring Service is seeking to recruit professionals with experience of working with children and vulnerable adults to the role of Independent Visitor.

We are looking for individuals all across Lancahsire and Merseyside areas, including but not limitted to: Liverpool, St Helen's, Warrington, Preston, Lytham, Blackpool.

The role of Independent Visitors is to provide visiting services on behalf of NYAS which are consistent with the National Quality Standards for Children’s Homes, Residential Special School Standards, Welsh National Minimum Standards, Health and Social Care Standards Scotland and CQC regulations. The main duties are:

• To undertake monthly visits to identified homes in accordance with the requirements of the relevant NYAS manager.

These visits will be:

Regulation 44 visits to residential children’s homes, short breaks and secure units.

Regulation 25 visits to residential family centres.

Care Quality Commission visits to residential adult homes.

Care Inspectorate Wales regulation 8 visits to residential children’s homes.

RSS20 visits to residential special schools.

Health and social care standards Scotland residential children’s homes.

Monitoring visits to unregulated provision.

• To undertake all tasks as identified by the relevant NYAS manager, during the visits and thereafter, in order to fulfil all aspects of the role.
• To provide factual reports within set timescales as required by the relevant NYAS manager.
