Main Responsibilities of the Job

• To provide support and assistance during journeyspick up and drop of duties at the commencement and end of sessions at our day centres.
• To demonstrate Balance values of partnership, independence, professionalism, empowerment, staff recognition and sustainability in your conduct and approach to your work
• Through partnership, support and develop peoples skills, confidence and interests as part of a multi-disciplinary day service offer.
• To support your colleagues, service managers and other key professionals to put those using our service at the centre of your daily work. .
• To maintain and update client and related operational records with an accurate and professional commitment.
• To ensure compliance with and the delivery of the charity policies and procedures particularly those related to safeguarding, inclusion and disability discrimination.
• To support clients with there daily activities and to promote their independence 
• To be able to communicate effectively with families , colleauges and  clients.

Working Expectations

• The working day is  8.00am - 5.30pm Monday to Friday -  The current rate reflects the additional hours 
• Drive a passenger service vehicle.
• Attendance at training. 
• Use of information technology to support your work.

Social Prescribing Link Worker

Position: Social Prescribing Link Worker

Location: Birmingham & North Solihull based in GP Surgeries

Hours: Full time or part-time hours

Contract: Variety of contracts available

Salary: £27,619 per annum pro rata for part-time

The Role

Social prescribing link workers will work as a key part of the primary care network (PCN) multi-disciplinary team. Social prescribing can help PCNs to strengthen community and personal resilience and reduces health and wellbeing inequalities by addressing the wider determinants of health, such as debt, poor housing and physical inactivity, by increasing people’s active involvement with their local communities. It particularly works for people with long term conditions (including support for mental health), for people who are lonely or isolated, or have complex social needs which affect their wellbeing.

Responsibilities include:

• Working with direct supervision from the Programme Lead/Senior Link Worker and input from the PCN’s Clinical Lead GP, take referrals from staff in GP practices within the local PCN.
• Provide personalised support to individuals, their families and carers to take control of their health and wellbeing, live independently and improve their health outcomes, as a key member of the PCN multi-disciplinary team.
• Developing trusting relationships and taking a holistic approach, based on the person’s priorities and co-produce a simple personalised care and support plan to improve health and wellbeing, introducing or reconnecting people to community groups and statutory services.
• Work collaboratively with all local partners to contribute towards supporting local community groups and organisations, making themselves aware of local resources and “assets” through mapping and then building a relationship with these organisations.  Also sharing intelligence regarding any gaps or problems identified in local provision with commissioners and local authorities. 
• Social prescribing link workers will work collaboratively with GP’s and have a role in promoting social prescribing and educating non-clinical and clinical staff within their PCN multi-disciplinary teams on the benefits of social prescribing
• Raise awareness of other services available within the community and how and when patients can access them. Linking into the work of Neighbourhood Network Schemes, locality Adult Social Care & Community Mental Health Teams.

About You

As a Social Prescribing Link Worker, you will have a Level 3 qualification in a relevant field (Health & Social Care, Health Trainer, Health Improvement, Community Family Worker, Psychology) or be working towards/willing to work towards.

You will have:

• Demonstrable commitment to professional and personal development
• Experience of working directly in a community based context (including unpaid work)
• Experience of supporting people with a mental health need and or complexities/vulnerabilities
• Experience of delivering one to one support
• Experience of partnership/collaborative working and of building relationships across a variety of organisations
• Knowledge of VCSE and community services in the locality
• Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities, individuals, their families and carers
• Understanding of the barriers faced by those living in isolation
• Knowledge of IT systems including ability to use word processing skills, emails, the internet and able to create simple plans and reports
• Ability to complete paperwork, keep records and carry out own administration

The service is designed to respond to the individual needs of clients, supporting people to improve their wellbeing, build confidence, reduce feelings of isolation, and access community services and resources.

About the Role

The role involves managing a one-to-one client caseload while contributing to the delivery of Social Support Groups. Through personalised support, the post holder will offer emotional support and empower individuals to focus on what matters most to them, working towards meaningful and achievable changes in in their lives.

The Social Support service provides tailored, personalised support based on each individual needs.  This includes using a range of tools and techniques to help people improve areas of their life they may be struggling with.  Grounded in the values of Hope, Courage, Togetherness and Responsiveness, the service aims to empower individuals on their journey towards improved wellbeing.

As a Peer Support Worker you will:

• Offer 1-1 support in St Albans, Hatfield and Stevenage, and within other HMN Wellbeing Centres across the County depending on client need and waiting lists
• Deliver Social Support Groups within our Wellbeing Centres
• Work with clients to identify their individual needs and support them through a personalised, person-centred approach
• Support clients in a variety of settings, including within the community, their home, and our Wellbeing Centres
• Play an integral role in embedding HMN’s values through our 1-1 and group support
• Play a key part in the development and delivery of our groups
• Ensure the consistent delivery of a high quality, responsive service that promotes wellbeing and positive outcomes for clients

The successful candidate will have significant proven knowledge of models and services to meet the needs of adults with mental health needs and of delivering mental health services within a health and social care setting as well as an understanding of the relationship between mental health and social issues and how these issues may impact on physical, mental and emotional wellbeing.

• Annual leave entitlement of 25 days per year pro rata, rising in increments to a max. of 28 days after 5 years employment (plus 8 days Bank Holidays)
