What You'll Do: -

Collaboration and Efficiency: Work hand-in-hand with existing Mental Health Provision for people and their families, as well as other local agencies/organisations to effectively support people transitioning from services.

You will have a strong understanding of the mental health support needs of LGBTQ+ young people and be confident in engaging with young people in a supportive, affirmative, and non-judgemental way.

You will support the service with high quality, consistent delivery to young people with our partners at P3. You will be part of a wider team of counsellors at Spectra who deliver across different client groups, in a supportive environment. 

We are seeking an experienced and enthusiastic support professional to join our unique Support Team.

You should have a genuine interest in helping people to maintain their quality of life and independence, regardless of the potential barriers they face. You must be patient, kind and sympathetic to the difficulties that an individual faces. You will facilitate their independence as much as possible and encourage their wellbeing at the same time.

Purpose of the role:

Croydon Health & Wellbeing Space (CHWS) is an exciting and dynamic service, run in partnership with Mind in Croydon and Croydon BME Forum and commissioned by South London and Maudsley Mental Health Service (SLaM). The CHWS opened in 2022 with the aim of providing advice, information, and specialist support to Croydon residents with mental health concerns, who drop-in, or have been referred to the via the local Integrated Care Network Plus multi-disciplinary clinical and community mental health support teams.

The Advice Worker will be part of a friendly team, including support workers, crisis workers and clinical mental health professionals and function as the first point of access for visitors to the CHWS. Following an initial conversation to ascertain the clients’ needs, the Advice Worker will provide information and advice, as well as signposting and referrals to specialist services, such as the Mental Health Personal Independence Co-ordinators, Social Prescribers and Community Link Workers, in addition to the wider voluntary and statutory services in the borough. The role will be fast paced and vary each day offering a fantastic opportunity for the postholder to develop their knowledge and skills and make a difference to the users of this service.

We are looking for a Complex Caseworker to join our Rough Sleeping Navigator team in Bristol who provide intensive support to male clients with complex needs experiencing homelessness. This passionate team offer person centred support service to help people who require a specialist intervention approach to support them through the transition from rough sleeping into sustainable accommodation.

In this role you will work collaboratively with the Rough Sleeper Navigator Coordinator and Outreach team as well as relevant partner agencies in Bristol to deliver sustainable outcomes to a male cohort of clients.

• You will advocate for clients who have been some of the most marginalised in the local area, to make a real and long lasting difference to their recovery.
• You will be responsible for overseeing a caseload of clients and providing day to day support to clients with their transition from the streets into accommodation and independent living with a strong focus on tenancy sustainment and rescue.
• This includes working with people on health, housing, employment and training support. You will be given support to build strong relationships with a range of other services to find creative and sustainable routes to recovery for clients.

New Horizon provides a range of Youth Work opportunities for young people experiencing homelessness. One stream of work is our Jobs, Education and Training (JET) team. The JET team support with young people to support them into meaningful and appropriate employment, training opportunities, apprenticeships, volunteering, and education.

In this role, you will work with young people 1-1, holding small caseloads, as well as working with young people in groups and delivering employment or education-focussed sessions on a regular basis.

• In the role of Housing First Worker, you will work 9-5 Monday to Friday, working with a person centred approach to support clients in their own homes as well as in the community.
• You will be responsible for managing a small caseload of client’s day to day, delivering ongoing and intensive support to help people set up their home, with the overall goal of supporting them to sustain their tenancy and achieve individual goals and aspirations.

About Us

Centrepoint, the UK’s leading youth homelessness charity is looking for dynamic, passionate and motivated Support and Progression Officers that can provide quality support to our young people to join our Barnet Foyer service in Cricklewood. We help young people by giving them the practical and emotional support they need to find a job and live independently. Together with our partners, we support thousands of homeless young people each year.

The service accommodates 24 single young people, usually, between the ages of 16 and 21. Young people stay at the service for up to 2 years before moving to more permanent accommodation. Barnet Service is quite a large building and offers a number of additional resources including a fully equipped gym. This role involves working 37.5 hours per week on a rota Monday to Friday (not including Bank Holidays). All staff must be available to work the hours of 8:00 am to 4:00 pm and 1:00 pm to 9:00 pm. You will be working with a team of three other Support and Progression Officers and the manager. Two staff members work together for each shift.

In this role you will:

• Work face to face with young people and support them to achieve their goals and independence.
• Work independently and as part of a team to ensure the highest possible standards of accommodation
• Undertake initial and ongoing needs/risk assessments for young people. Be able to identify safeguarding risks and understand how to escalate any concerns
• Manage your own caseload of young people, person centred support in line with their needs and aspirations
• Liaise with multiple agencies to ensure effective communication across the service and externally including social work teams, YOS, health and other statutory services and voluntary agencies
• Make internal and external referrals to a wide range of statutory and voluntary agencies for specialist support and Employment and Training opportunities
• Support young people to manage their rent accounts through liaison with the DWP, Social Services CenterPoint’s rent team and building their own personal budgeting skills
