Check NowThe post-holder will work with patients to plan the support they need to be discharged home; to support voluntary, community and social enterprise organisations to respond to the needs of people returning home from hospital; and to raise awareness of the wide range of assets available in the VCSE sector that enable people to self–care or provide short-term support.
The Home First Support Worker will use a person-centred approach to support individuals preparing for discharge or recently discharged from Salford Royal Hospital who have been identified as needing additional support within their home and community. The post-holder will work with individuals and families as part of a Home First Community Hub multi-disciplinary team, as well as within the Salford CVS team.
The Home First Support Worker will build strong working relationships across the Home First multi-disciplinary team (MDT) and raise awareness of the wide range of activities and services available in the voluntary, community and social enterprise sector (VCSE) within the Home First Hub as well as the wider Salford Royal Hospital staff team. The post-holder will work pro-actively in partnership with a range of statutory and VCSE colleagues in the Home First MDT to ensure that the needs of individual patients are addressed wherever possible.
The Home First Support Worker will also work with the wider CVS team to map current community provision of activities or services (Age UK Salford, Helping Hands, and the Carers Service, for example) offering support for people to return home and become re-engaged within their community. They will also support a range of VCSE groups / organisations to develop services and activities that meet the needs of individuals.
The Home First Support Worker will be aligned to Salford’s flagship Wellbeing Matters programme, a partnership between Salford CVS and Salford Third Sector Consortium, and draw on existing knowledge from within the Community Connector and Development staff teams from the programme, working closely with that wider team at all times.

About the role:
This role is home based, but will include some travel nationally, and some overnight stays.
As a Peer Support Coordinator you will: -
Manage a number of Peer Support volunteers who have lived experience of bereavement through homicide. You will support them to use their experience to help others who are also bereaved. This will involve monthly supervisions and regular communication to ensure they are up to date with service procedures and developments.
You will have experience of working in a trauma-informed way, with empathy and compassion. You will have good personal resilience, and be able to make effective use of supervisions to manage your wellbeing.
You will have a passion for using digital technology to make support accessible for people. You will take the lead on maintaining our secure online chat room by working with a group of volunteer supporters. You will also be key in the planning and organisation of our online support groups. Strong digital skills and enthusiasm for digital working is essential.
You will need: -
Knowledge of the impact of bereavement through homicide or the psychological impact of significant trauma resulting from a distressing event
Proven ability to work digitally in creative and appropriate ways to engage with service users and/or peer supporters
Confidence in using Microsoft Word, basic Excel and PowerPoint, and the ability to use client databases
Ability to work on own initiative with good time management skills, prioritise work load, and make use of clinical supervision

We are now seeking an Information and Support Worker (ISW) to join our growing team. This is a home-based job so you will require lots of initiative to work on your own, be highly organised, committed and have good time management skills. You can be based anywhere in the UK
You are the first point of contact (along with the existing ISW’s) for women/families contacting us for support. You will need to demonstrate empathy, sensitivity, discretion and an understanding of how we can provide support. You will respond to enquiries via various contact channels; assessing the nature of each enquiry quickly and skilfully
You will be responsible for adding new persons to our private forum for those who have been affected by cancer during or shortly after pregnancy. The topics discussed on here are highly sensitive and you will help ensure that the forum is a safe place for them to share their innermost feelings, reporting any concerns to the team.
You will have good IT skills, including Word, Excel and be confident and competent with the internet, social networking sites and email. Occasional travel will be required, therefore driving/having your own car may be helpful but is by no means essential. All travel is remimbursed and overnight accomodation paid for where neccesary. We’re searching for a part-time nurse with haematology experience to bring their expertise and passion to our small but ambitious blood cancer charity. The role will be focussed on setting up and managing our brand new Support Line service.
You’ll have experience of managing and caring for people living with blood cancer, and it would be beneficial if you have specific experience and knowledge of caring for people with Waldenstrom’s macroglobulinaemia (WM).
Reporting to our CEO, you’ll be responsible for setting up and managing the UK’s first dedicated support line for people living with WM and their friends and families. You’ll be supporting the 4,000 people living with the disease, by helping them to understand their diagnosis, provide emotional support and help them to prepare for appointments.
We’re looking for a nurse who can commit to 15 hours every week, ideally split over 3-5 days, to run the WMUK Support Line. However, there is flexibility for the right candidate.

We’re the only charity in the UK focussed on supporting people living with WM. We provide expert information, a listening ear and support services for patients, their friends and families.
Since the charity re-registered in 2020, it has grown into a vibrant hub of information and support for anyone affected by the disease. Our Community Advice and Support Team (CAST) operates an All-Age Community Social Work and Support team.
The team can offer intensive support from a team of Registered Social Workers and Community Support Workers.
We are a first point of contact for people seeking help and advice via our helpline which is available 5 days a week. Last year we answered over 3000 calls for help.
CAST also signpost to other statutory and voluntary sector organisations for appropriate support. Our work has a broad reach across all parts of the Manchester Jewish Community.
The Mental Health Navigator will be co -located with Community Advice and Support Team with the successful candidate will benefitting from the expertise and guidance of both Jami & The Fed.
Job purpose
This developing and innovative post seeks to add value and resource to the existing mental health support in Manchester through Jami’s partnership with the Fed. The Fed is the leading social care charity in the Greater Manchester Jewish community.
Through this partnership Jami and The Fed are striving to provide high quality mental health support and create an environment of continuous learning, improvement, and innovation. This role is to support the mental health needs of the Jewish community in Manchester through working with communal organisations and closely with statutory services, ensuring that people have access to recovery focussed mental health support. The post holder will play a key role in assessment of need, providing interventions, building relationships with other organisations, identifying gaps in service provision, and evaluating the impact of this new and innovative role.
Responsibilities
- To work collaboratively across voluntary and statutory sector health and social care organisations to meet the needs of people aged 16+ experiencing complex mental health needs in the Greater Manchester Jewish community.
- To work as part of an MDT team, creating appropriate networks to ensure effective and timely referrals and case management.
- To develop an awareness and effective relationships with voluntary and statutory organisations in the Greater Manchester area.
- To carry out holistic assessments of need, identify person centred goals, develop support plans and measure impact.
- To signpost / refer to other organisations and specialist services and identify gaps in service provision.
- To enable effective navigation to services and coordinate support collaboratively with other services.
- To identify risk factors, assess harmful risk and develop safety plans, making referrals to statutory services as appropriate.
