Hillside is looking for a part time Caseworker to join the Mental Health Recovery Team which supports Islington residents with:

• Person-centred, coproduced and trauma informed recovery-focused interventions.
• Promotes independence and self-management for members
• Delivers a wraparound service in partnership with local authority, statutory agencies and community partners.
• Ensures members ‘involvement in active volunteering and coproduced activities, service planning and reviews

Main responsibilities:

• Motivate, lead and encourage member participation at all levels of responsibility, working alongside members to complete tasks.

• To work as a member of Hillside Team to deliver outcomes for the Recovery Programme
• To keep systems and database up to date with members ‘information
• To provide information and support at a time of crisis to service users.
• To manage a caseload of complex adult client cases, providing client centred, practical and emotional support.
• To conduct risk assessments and deal with safeguarding concerns promptly and in line with Safeguarding protocols and procedures.
• To offer information and support to members regarding their mental health, and signposting to appropriate organisation for issues concerning housing, benefits and any legal matters

Skills, Knowledge and Abilities

• Experience in direct support work with adults with mental health diagnosis or other vulnerable people including providing emotional and practical support (one year minimum)
• Excellent caseworker with good assessment, motivational and empowerment skills
• Experience of working cooperatively and effectively in a team
• PC literate (Word, Excel, PowerPoint, Outlook)
• Sensitivity to cultural differences and the ability to work in diverse settings and with people with diverse needs.
• Knowledge and skills to operate within a trauma-informed approach.
• Ability to provide non-judgemental, non-directive, person-centred and confidential support to members in crisis
• A clear understanding of safeguarding policies and procedures
• Ability to break down objectives/tasks to manageable components and maximise the potential for member participation
• Understanding of and commitment to the principles of social inclusion
• Understand and be able to implement the Equality Act and GDPR
• Deal with difficult and/or challenging situations as they arise to ensure members’ safety.

Other tasks

1. Act as designated key worker with responsibility for goal planning to a group of members

2. Participating with colleagues organise social programme activities for members

5. To work occasional out of hours work, on a rota basis if needed, in accordance with the needs of the programme. 

At the Guide Dogs our Access Assistants help our employees to live the life they choose by working as a driver and support worker to enable the person supported to carry out their duties efficiently and effectively. An Access Assistant does not fulfil the role for the individual but enables the person supported to carry out their role by supporting tasks to enable them to be successful in their role.

This role will include driving daily, working closely with the Habilitation Specialist. You will ideally be located within a 30-minute commute of NG9, use of vehicles will be discussed in more detail at interview. You’ll be working 22 hours per week and the majority of hours are likely to be spread across 3 days between 9am to 5pm. However, there may be some flexibility around how hours can be split, which we are happy to discuss at interview.

The Access Assistant will be confident when driving to new and unfamiliar locations and will possess strong written skills, including the ability to take accurate notes and keep clear records. They should be comfortable using basic IT tools such as email, word processing, and data entry programs. The role requires someone who can follow verbal instructions and work independently once tasks have been explained. Reliability and good time management are important, as is the physical ability to lift and carry items when needed.

Previous experience working with individuals who have physical or sensory processing access needs would be beneficial, as would familiarity with accessibility software or adapting digital and printed content. Experience supporting professional or training events may also be useful.

The successful candidate will approach their work with a calm, friendly, and respectful manner, maintaining a high level of discretion and confidentiality. They should be adaptable, open to varied tasks, and comfortable working one-to-one or in unfamiliar environments. About the role

We’re looking for a Social Worker to support children and young people diagnosed with cancer in our Leeds Team. 

We pride ourselves on delivering the highest quality support tailored to the needs of the children and young people and their families using a needs based assessment framework. The work we do is rewarding but also complex and demanding. You will be part of a close-knit Young Lives vs Cancer social work team, working with an established NHS multi-disciplinary team and services in the community.

This role is part home, part site-based. Your contractual base will be both Home and Leeds General Infirmary and St James's Hospital

What do I need?

You will be a creative self-starter who will support the service and look for innovative solutions to support clients with complex needs. You are confident professional relationship builder and has good awareness of Safeguarding procedures. Strong Administration skills and understanding of compliant behaviours also experience of supporting. With good knowledge of the criminal justice system and awareness of other support provisions.

Bounce Back workers work with the Herts Mind Network Community Outreach Teams and hospital staff to ensure that patients are offered advice, information and holistic support for smooth, timely and supportive discharge from hospital back in to the community.

About the Project

Herts Mind Network (HMN) has formed a partnership with Hertfordshire Partnership Foundation Trust (HPFT) to deliver a one-to-one outreach service for people who are in psychiatric hospitals across Hertfordshire.

About the Role:

The Bounce Back Worker will be based at Kingfisher Court or Albany Lodge and HMN Centres, promoting joint working between Kingfisher Court staff and HMN, creating a seamless pathway into community support for clients recovering from mental ill health.

The aims of the Bounce Back service are; to provide up to 10 sessions of emotional and practical support, advice and information to patients. We will visit patients on the ward and continue the sessions in the local community and in patient’s homes once discharged. We will facilitate a smooth and timely hospital discharge.

The recovery approach includes empathy, warmth, acceptance, authenticity, compassion and humanity.

Key Responsibilities

• Receive referrals from the ward teams at Kingfisher Court, Albany Lodge and other appropriate hospitals across Hertfordshire.
• Provide practical and emotional support to encourage patients to develop their independence within their local community.
• Facilitate a smooth discharge from hospital back to the community.
• Have a solid understanding on the dual effect of mental health and drugs and alcohol.
• Monitor outcomes and evaluate the service.
• Ensure that the safety and wellbeing of patients using the service is monitored and reviewed regularly.
• Embed a person centred and recovery orientated approach in all aspects of the roles and responsibilities.

Our Peer Support Service started 13 years ago, with 2 Wellbeing Centres offering this new service. Since then, we have seen our service grow and we now offer Peer Support across the whole of Hertfordshire

Working in co-production with clients, staff, volunteers and stakeholders, we were able to create our own Peer Support Charter Values (this can be found on our website). These values form the ethos we work towards within our peer support team. These values also complement HMN’s values of Hope, Courage, Togetherness and Responsive, which form the basis of all the work we do as an organisation. 

About the Role

Working with the team to offer 1-1 peer support to anyone accessing HMN. The Peer Support service provide tailored support for individuals to support their needs. This includes using different tools and techniques to help an individual improve areas of their life they may be struggling with. Having a lived experience of mental health issues or supporting someone who has is a key element to what makes this service so unique.

The Peer Support approach includes empathy, warmth, acceptance, authenticity and compassion. Peer Support is based on the recognition that there is no better person to support the path towards recovery, than someone who has walked a similar path as that person.

To meet client needs, and work to this personalised approach, we do support people out in the community, within their home, as well as in one of our Wellbeing Centres. Being able to drive and having access to your own vehicle (or equivalent) is essential for this role.

The successful candidate will need a personal understanding of mental ill health, either from your own lived experience or by caring for someone with a mental health condition and have awareness of issues in mental health service provision.

Peer Support Workers will also provide support within our Peer Support Groups, Peer Support Learning Sets and Peer Support Academy when needed.

We would like someone who is passionate about supporting young people from ages 10 to 18. The person would have experience in leading detached and youth club sessions in the delivery of high-quality and exciting activities which contribute to personal and social development.
