Young Carers Support Coordinator (Families Lead)

Winchester, Hampshire (Hybrid)

£26,390 per annum (FTE / Actual Salary: £15,780)

Part-time (22.5 hours per week to include a Wednesday and Thursday)

Our client is a small independent charity who supports young people who take on caring responsibilities for an unwell or disabled family member between the ages of 8 - 25. They provide programmes of support to empower Young Carers to look after their physical and emotional wellbeing and have experiences they would otherwise miss out on.

They believe that Young Carers are children first, and should be free to develop emotionally, and take full advantages of opportunities for educational achievement and life success.

As a Young Carers Support Coordinator (Families Lead), you will work as part of a small dynamic team to deliver their support programmes for Young Carers. This will include supporting their exploration programme and activities programme and young adult carers programme.

You will also coordinate the organisations referral and assessment process form start to finish. This will include database entry, contacting referrers, allocating cases and booking and carrying out assessments. You will also provide on-going support to families via phone, referrals, signposting and attending relevant multi-agency meetings.

They run an Exploration Programme which focuses on wellbeing – this currently runs on Wednesday afternoon/evenings during term time.

They run an Activities Programme which focuses on new experiences and respite – these are daytimes during school holidays. Usually, two or three per school holiday week.

We are seeking a flexible, reliable, and enthusiastic Support Worker to deliver and develop services for Young Carers aged 5–16. The role involves three key parts. The first is delivering engaging, age-appropriate activities that promote wellbeing, reduce isolation, and give Young Carers a break from their responsibilities. The second is providing one-to-one support to help Young Carers manage challenges, access services, and prioritise their mental and emotional wellbeing. The third is leading outreach to identify and engage hidden Young Carers. 

About The Role: 

You will work across two key age groups, 5–11 and 12–16, delivering and supervising activities, supporting events and trips, and providing behavioural management where needed. Alongside group activities, you will offer one-to-one support to individual Young Carers, ensuring they feel heard, included, and supported as they navigate the challenges of their caring roles. You will also deliver awareness training sessions, run stalls, and build partnerships in the community to identify and engage Young Carers. 

This is a part-time post (3 days per week). The regular days will be Wednesday - Friday, However flexibility is required, with regular evening and occasional weekend work.

Carers Manchester is a partnership of Statutory and Voluntary Sector organisations who work together the plan, enhance and deliver advice and support services for unwaged carers in the city. • Support the delivery of a number of projects and workstreams co-ordinated by Gaddum.
• Take responsibility for receiving and inputting data into databases and spreadsheets.
• Provide an initial point of contact for carers and professionals contacting our services.

• Ability to collate and input data onto data management systems.

• To research and continually develop the activities and training programmes to ensure it is responsive to carers needs.
• To create promotional material to promote activities and training opportunities to carers registered with Richmond Carers Centre.
• To work with Adult Carers Support Team Leader to ensure activities are within budget, achieving targets and meeting the requirements of funders.

About the Role and the Service

The Carer’s Wellbeing Navigator will play a key role in a new service to ensure unpaid carers in Milton Keynes are supported through a “no wrong door” approach.

Working as part of a partnership delivery hub with other local service provides, the postholder will provide person centred 1-2-1 support to carers of individuals living with mental health challenges and/or addictions. The Navigator will work flexibly offering face-to-face and telephone support to co-create wellbeing plans, strengthen resilience, and promote access to appropriate information, resources and services.

The Carer’s Wellbeing Navigator will also identify opportunities for peer-led support by developing and facilitating peer group(s) empowering carers to share experiences and reduce social isolation.

The role will also contribute to wider awareness-raising of unpaid carers through initiatives such as National Carers Week and the celebration of carers in the community.

Highly dynamic and self-led, the Navigator will actively help to shape the service, building strong links with local partners and ensuring unpaid carers of those with mental health difficulties receive inclusive, and accessible support.

Key Responsibilities

• Deliver a service and ensure outcomes which supports unpaid carers who are caring for someone experiencing difficulties with their mental health and/or additions, in line with the service delivery model, contractual requirements and Key Performance Indicators (KPI’s).
• Promote and raise awareness of this service across Milton Keynes including building relationships with referral organisations, VSCE and statutory services and the wider community.
• Build effective collaborative relationships with other hub partners and attend partner meetings when required.
• Support with effective referral routes in to and out of the service which break down any access barriers.
• Contribute to the development and review of service delivery.
• Effectively manage and process referrals into the service and prioritise a caseload of unpaid carers that ensures timely and effective support in line with the service delivery model.
• Carry out an initial assessment of individual support needs, priorities and risk, in line with the Access to Service procedure and processes.
• Enable individuals to explore the situation that is affecting their mental health and wellbeing and work with them to co-produce a personalised support plan to help address their immediate needs.
• Offer focused support with the aim of building knowledge, skills, confidence, independence and resilience.
• Signpost, and support to access, other relevant organisations and the wider community offer to address wider issues affecting their mental health.
• Share personalised information and advice about each unique circumstance and provide resources to support the carers mental health and wellbeing.
• Develop and facilitate peer support groups for unpaid carers who are caring for someone experiencing difficulties with their mental health and/or additions
• Carry out health and safety responsibilities as directed by Managers in line with Mind BLMK’s H&S policies, procedures, and guidance (risk assessments, reviews, reporting, safeguarding, incidents/accidents, premises).

You will have the ability to work with staff across the organisation in supporting good financial governance and oversight of financial operational matters. You will have the ability to analyse financial information and present this in a suitable format for the audience. You will have good ability to meet deadlines, be IT literate with an emphasis on financial systems, spreadsheets, Quickbooks and Excel. You will be highly organised with excellent attention to detail and be able to work with minimum supervision.

About the role

The role will involve the management of the day-to-day financial activities, VAT, gift aid, payroll and the monthly, quarterly and annual reconciliations and statutory accounts preparation. The job holder will work closely with the Chief Operating Officer, Finance Officer and volunteers within the team. There will be a requirement to work closely with the income generating staff, budget holders, auditors, banks and credit card providers on a regular basis. The Finance Manager will be responsible for managing the Finance Officer and Finance volunteer.

This is an exciting opportunity to join our team as a Home from Hospital Co-ordinator. We are looking for 2 Home from Hospital Co-ordinators, working in the Selby area. The posts will be a hybrid model of working.

Salary Range: £24,437 - £26,802 (Pro Rata for full time equivalent)

Key Responsibilities:

· Support individuals discharged from hospital, aiding in their transition to home life.

· Assess client needs and develop tailored support plans.

· Collaborate with hospital staff and community health & social care teams.

· Establish referral pathways to increase community awareness.

Requirements:

· Previous experience in assessing client needs and developing support plans.

· Ability to provide support in home settings.

· Strong interpersonal skills and ability to work independently.

· Previous experience of imputing information to a database
