Specialist Support Workers within Look Ahead are required to have either an extra dimension of experience or a higher qualification level as determined relevant for the particular post. Specialist Support Workers provide information, social inclusion and vocational opportunities to support people to recover and stay well; exercise choice and control in their care and lives; and participate on an equal footing in the community. Support is also provided to develop a model of re-enablement towards self-advocacy, self-management and empowerment, incorporating the principles and practice of recovery, co-production and peer support.

Oaklands & St Kilda is a pair of 6 unit supported accommodation mental health rehab services working with young people between the ages of 18-25 who may have experienced a mental health related hospital admission to gain and develop skills for future independent living. The service will support individuals transitioning from Child and Adolescent Mental Health Services to Adult services.

The service forms part of a pair of services supporting up to 12 individuals at any one time across the sites, providing a welcoming and supportive environment for young people across london on a spot purchase basis.

Specialist Support workers will manage a caseload of customers to help them to identify, plan and achieve their goals, develop relationships meaningful to them and achieve greater independence by providing a flexible and individually tailored support package.

Consists of working 5 days a week across all days, 8 hours shifts starting at either 8am or 2pm.

This role requires a valid Right to Work for the UK as Look Ahead are not a sponsoring organisation.

What you'll do:

Building supportive, trusting relationships with customers and creating a positive atmosphere
Carrying out holistic assessments of new customers which incorporate relevant statutory referral information
Conducting regular key work sessions that are innovative and engaging in order to achieve Support Plan goals
Create support plans for providing appropriate services based on on-going assessment and reflecting the services and resources available to enable customer to progress in their recovery
Ensuring ongoing assessment and management of risks associated with customers within an attitude of 'positive risk taking
Provide support and access to relevant services to enable customers to continue their recovery and empower customers to self-manage their medication regimes through individual re-enablement programmes and informed decision making
Undertake support work in partnership with external stakeholders to compliment their interventions

Fareham and Gosport Wellbeing service is looking to recruit a PCN Wellbeing Advisor, to be based in PCN surgeries in the Fareham and Gosport Localities.

In this role you will hold a caseload of patients offering assessments & 1-1 sessions to help manage mental health Wellbeing. You will attend meetings to enable patients to get the best service for them and will support people to access groups and activities within our wellbeing centre and the community. As the post-holder, you will be working in our Camden Women's Recovery Service hostel providing specialist interventions for a designated caseload and to the wider client group, via our in-house recovery programme. 

Your role will be working intensively with the residents on their supports needs. The role of Complex Needs Project Worker is a specialist role, designed to work with people who may be more difficult to reach and establish working relationships with.  As such, you will role model good practice for working with those who have experienced multiple disadvantage by adopting a trauma informed approach.

Within this setting, you will also work to establish a culture of co-production where residents are at the forefront of the work that is carried out, helping to promote autonomy and build individual confidence and interpersonal skills.

The rota for the post is earlies (8 am - 4 pm) and lates (2.30pm - 10.30pm).

About you:

• Previous experience of working with female rough sleepers or people with complex mental health, physical health or substance use support needs. 
• An understanding of working with people using person-centred approaches and working collaboratively within a harm minimisation and recovery framework.

ABOUT THE ROLE

We're looking for a Children and Family Worker to join the team in our West London Women's refuge. The service is a refuge accommodation for women and children surviving domestic abuse many of whom are living with trauma. we are looking for someone who really cares about the rights of women and their children.

As a Children and Family worker, you will support in providing a much needed safe and nurturing environment for children and their mother's, providing pathways, support and appropriate interventions to enable resettlement. You will support various teams to co-produce a tailored support plan which is altered to the individual's needs through a recovery model providing trauma informed and enabling environments to achieve this. 

Shift Pattern: 37.5 hours per week, working on a rota which is over 7 days including evenings. You will be based on site within the service.

OVERVIEW OF KEY RESPONSIBILITIES

Children and Family Relationships

• Provide parenting advice to support mothers and their families.
• Provide stimulating play opportunities to encourage children from all backgrounds to develop and express themselves, as well as support the children with their general learning, homework, and literacy.
• Support with early learning development, supporting mothers through advice and activity packs which aid their children’s learning and development.
• Deliver family sessions to support contract orders by social services and other organisations, as well as to help rebuild relationships, life skills, confidence, reliance, self-esteem and manage trauma.
• Offer age-appropriate sessions to children to provide a safe, encouraging, and development led environment which provides meaningful and memorable experiences.
• Work with residents and their keyworkers to understand complex needs to help build children’s and their families lives.

Support Working

• Build and sustain trusting and respectful relationships with our residents, including children.
• Create a safe, stable, psychologically informed environment which is nurturing to children and families.
• Provide support for residents, contributing to the development of support plans, risk assessments, reviews, regular support sessions and other personal development activities.
• Support with reducing barriers to reintegrating into communities, and re-developing confidence, skills, and coping mechanisms to support those with previous trauma.
• Develop and sustain therapeutic relationships with our residents, providing practical and emotional support with a trauma informed approach to ensure they are always treated with respect and dignity. Deal with sensitive information in a professional manner.

Additional Responsibilities

• Attend appointments as and when required in support of residents.
• Undertake shift lead responsibilities on rotation, taking responsibility for ensuring day-to-day support delivery is effective, with staff carrying out their designated responsibilities, alongside ensuring resident and service-specific follow up actions are completed.
• Housing Management responsibilities, ensure residents and staff are safe, and the surroundings are kept clean and well maintained.

Other responsibilities than those described above may be required to be undertaken from time to time and will be expected to be performed to as long as it is within the capability and level of the position. 

KEY CRITERIA

What we are looking for:

• Understanding and/or experience of working with people of complex backgrounds
• Previous experience and/or in-depth knowledge about working with children who have experienced trauma
• IT Proficiency, ability to learn new software programs, basic Microsoft experience
• Understanding of the trauma children and families may go through due to domestic violence and other previous experiences
• Understanding of the social needs and support required for people with multiple and complex needs
• Understanding and/or practical knowledge of the social and societal marginalisation that can be attached to people with various challenging backgrounds, mental health, addiction, exploitation, homelessness, and previous convictions

What we would like, but not essential:

• Previous experience creating co-produced support plans, key working, and care planning, providing appropriate interventions for service users
• Previous experience providing housing support and practical assistance within a residential support role
• Appropriate Professional Qualification in: Social Care, Early years Education, Certificate in Childcare or other relevant
• Understanding and/or practical application of key legislation regarding social care, housing, criminal justice, and mental health
• Ability to Recognise signs of deteriorating mental health and initiate appropriate interventions to prevent crisis

About the role:

The post holder will work within The Passage’s Community Services as a Complex Case Outreach Worker and will lead on case management for people experiencing homelessness who are presenting in the footprints in which the team operate and/or who are accessing the Resource Centre. The post-holder will support individuals with substance use support needs and/or mental health issues and will lead on work with clients who are experiencing alcohol and/or drug dependencies and who may also be experiencing mental health problems or other health or social care needs.

Main duties:

• To manage a caseload of clients presenting within the footprints in which the Engagement & Outreach service operate and individuals who are accessing The Passage Resource Centre. To support clients with complex needs, supporting them to identify and work towards recovery and away from homelessness.
• Carry out assessments, case management, support planning and the delivery of interventions for clients including those experiencing mental health and/or substance misuse issues who are multiply-disadvantaged in their homelessness.
• Provide assertive outreach and engagement with street-based service users presenting with complex needs and barriers to engagement. Carry out monthly street-based audits to inform reporting and analysis of service outcomes.
• Support clients who are rough sleeping or involved in day time activity in the footprints in which the team operate to end their homelessness by linking them in with appropriate internal and external support services relevant to their needs. To provide support to clients who are accommodated, but may require ongoing support in order to maintain this accommodation, linking them in to appropriate local services.

General responsibilities:

• In conjunction with your line manager to continuously develop the role to ensure that all tasks are being undertaken in an effective and appropriate manner which meets the strategic aims and objectives of The Passage.
• To participate in internal/external meetings as required, attend training events, conferences and other functions as necessary.
• To participate in regular supervision and annual appraisal, and help in identifying your own job-related development and training needs.

Hillside Clubhouse is recruiting for a mental health employment specialist to join our dynamic Camden team supporting people with mental health conditions to gain and retain employment.  The role is working within the nationally recognised IPS (Individual Placement and Support) model within an NHS secondary care mental health team based in the heart of Camden.

Who we are: 

Hillside Clubhouse is a co-produced mental health employment support charity working across Camden and Islington. We support people with mental health conditions into training, volunteering, and employment opportunities. People can access our services via our Clubhouse, where they can share their skills and expertise with others and support the running of the charity.  Alternatively, they can receive support out in the community from one of our employment specialists based within primary and secondary care.

The Role: 

The role is within our IPS Service and will be based within a Camden NHS secondary care team. As an IPS Employment Specialist, you’ll play an important role in supporting people with lived mental health experience on their recovery journey by supporting them back into paid employment. This is an opportunity to offer ongoing support, to motivate and provide people with hope, while developing the tools and skills required to recover, ultimately transforming their lives.

A variety of skills are needed in this post including but not limited to -  a strong ability to develop empathy and rapport with clients; a determined approach to seeking out job opportunities; a person centric-approach to working with people; strong communications skills to motivate clients; strong administrative capabilities and an attention to detail engage with prospective employers via business development and face-to-face meetings, and to be able to challenge some perceptions around mental health. 

The postholder will be directly employed by Hillside Clubhouse but work under an Honorary Contract from Camden and Islington Foundation Trust.  They will be managed directly by the IPS Operations Manager but work as team member within the clinical team. The role is dynamic and involves being integrated in a clinical team, working within the community, and we operate a flexible working policy where working from home is also permitted up to two days per week.

Overall aim: 

To enable people with mental health issues to gain and sustain competitive employment by working within the evidence based Individual Placement and Support (IPS) model.

Primary Duties: 

• Manage a caseload of up to twenty clients and work within and apply the 8 principles of IPS in all aspects of role. 
• To work with and enable clients to find work using a mix of conventional approaches and employer engagement. 
• To generate ongoing referrals from the clinical team and to manage those effectively. 
• To engage directly and collaboratively with the clinical team while maintaining the existing multi-disciplinary approach to assisting clients seeking work. 
• To report and meet regularly with clinical team service managers and the IPS Operations manager, and to participate in both casework and clinical supervision from IPS Operations and clinical team services managers. 
• To be an active member of both the clinical and Hillside teams 
• To be responsible for own management of administrative duties tied to both Hillside Clubhouse and clinical team and adhering to existing Hillside and NHS protocols. 
• Source job opportunities specific to client needs from a range of sources including those advertised via current and conventional means, as well as through contact with local and Pan London employers. 
• To build up a network of employers to secure available and future vacancies. 
• To maintain existing and seek out new relationships with a range of external agencies of use to the post. 
• To provide ongoing support to both client and employers (where appropriate) when starting work and until such time as the client makes the decision to end the support provided. 
• To work with clients to collaboratively manage job searching activities and engagement with the service. 
• To manage and meet all KPIs, statistics and documentation as required, including but not limited to: referrals, first meetings, job outcomes, job sustainments of 6, 13 and 26 weeks, and work retention. 
• To provide guidance on the impact and/or the potential change to benefits when clients start work. 
• To work and manage time effectively in both community/external settings, and at clinical team.

• Secondary Duties: 
• To work out of hours as and when required, working in line with Hillside Clubhouse risk management procedures. 
• Undertake training when required and participate in ongoing learning and development. 
• To maintain professional boundaries with all internal and external stake holders including clients, and staff members of both Hillside and the clinical team. 
• To engage and participate in external IPS support services, training, and events. 
• To undertake additional tasks and duties as and when required, and to work outside of normal office hours when necessary. 

An exciting and rewarding opportunity has arisen to join our Aquarius service in Birmingham. The Youth Participation Worker will facilitate and develop a Youth steering group, support aftercare for young people leaving treatment, coordinate and facilitate diversionary activities to support young people and their families.

The post holder will develop and support delivery of all volunteer programmes inclusive of Peer Support and Peer Mentoring Programmes, to include the recruitment, placement and retention of volunteers/peer mentors.  They will ensure that general service User participation and empowerment is implemented across the Service.

In addition, the post holder will:

• Actively recruit volunteers, peer mentors and facilitate Youth Steering group meetings.
• Work with the Senior practitioner & Team leader, to develop and implement a service user involvement strategy, including establishing and supporting a service user board.
• Develop and deliver volunteer, peer mentoring and service user involvement programmes in accordance with the Aquarius organisational framework.
• Support the delivery of aftercare provision.
• Assist colleagues with the coordination and delivery of diversionary activities.
• Keep accurate records and the necessary data for reporting and evaluation purposes.
• To provide written records and reports as required by the organisation.
• Be available to work days, evenings, and weekends when required, to meet the demands of the service and be willing to travel to various locations, as necessary.

An energetic and confident self-starter, you have a certificate in youth and community work e.g. NVQ Level 3 or above, DipSW, Mental Health Nursing, Counselling, Addiction Studies) or significant experience of working in a similar role with a commitment to complete NVQ Level 3 Health and Social Care. As well as significant knowledge and experience of working with young people, you have knowledge of project planning and experience of working in the substance misuse sector. What’s more, you have a flexible approach, a commitment to the principles of confidentiality and excellent record keeping and Microsoft Office.

This is a permanent full-time role requiring the post holder to work 37 hours per week.

ABOUT THE ROLE

A new opportunity has arisen to join a new contract in Lewisham. You will support over 2 properties which supports residents and participants with acute and/or complex mental health needs who are supported/treated in the community in their recovery as appose to hospital inpatient care. You will focus on rehabilitating and reintegrating our residents and participants and help our residents and participants achieve their personal goals and gain a level of recovery that enables them to return home or move into supported housing.

Shift Pattern: Rota based Monday to Sunday between 08:00 and 19:30 

ABOUT THE SERVICE

The Integrated Community Rehabilitation Services will enable people to receive a community-based offer and avoid admission into a rehabilitation unit. 

The service is across 2 properties in Lewisham (one 8 self contained rooms, and another with 6 self contained rooms) located in close proximity to each another and both will be staffed 24/7 by a minimum of 2 Mental Health Recovery Workers (MHRW) at all times, at each site. This will include Shift Leads to support the Management Team. 

These addresses are a short walk from each other and are served by good public transport links; close to Bellingham, Catford and Catford Bridge train stations, and close to bus stops.

KEY CRITERIA

What we are looking for:

• Previous experience working with people with multiple complex backgrounds/needs, mental health, substance misuse, alcohol, and behaviours 
• Previous experience creating co - produced support plans, key working, and care planning, providing appropriate interventions for service users
• IT Proficiency; ability to learn new software and basic Microsoft
• Understanding of the housing and social needs of people with multiple and complex needs
• Understanding and/or practical knowledge of the social and societal marginalisation that can be attached to people with various challenging backgrounds, mental health, addiction, exploitation, homelessness, and previous convictions

What we would like, but not essential:

• Understanding and/or practical application of key legislation regarding social care, housing, criminal justice, and mental health
• Ability to Recognise signs of deteriorating mental health and initiate appropriate interventions to prevent crisis
