Job Purpose
This role is within our crisis alternative service Safe Space - an out of hours service providing a safe and welcoming space for people who are feeling distressed and experiencing crisis. The service operates 365 days a year from 12:00pm (midday) - 11:30pm across all of our boroughs. The role will consist of two shift patterns to ensure the service is covered: 11:30am-6:30pm and 4:30-11:30pm. This will be on a rota basis across support workers.
Staff within the service will provide person-centred, practical and emotional support, face to face or via telephone on a one-to-one or group basis to individuals experiencing mental health crisis. This will consist of 1:1 support; de-escalating and providing interventions, signposting for specialised or longer-term support, and delivering collaborative self-management plans with clients to improve mental wellbeing. The aim of this service is to de-escalate and prevent further crises.
The objectives of the service include:
- To improve the mental wellbeing of people experiencing mental health crisis in HFEH.
- To provide support to clients accessing the service- for instance: signposting, de-escalation, planned wellbeing activities etc.
- To contribute to an improvement in individual mental wellbeing.
- To remain a source of independent support for all clients.
- To treat service users with respect, dignity and personalised support
- To raise awareness of mental health services available with the goal to improve long term mental health and reduce social isolation
- To increase self-management skills of those accessing the service
- To reduce the use of police, ambulance and statutory mental health services whilst experiencing crisis.
- To reduce the use of statutory crisis services by people experiencing mental ill health without positive outcomes for the individual.
The Role
The role of the support workers will be to support the rest of the team in delivering interventions on a one-to-one and group basis. Team managers are responsible for management of their site; including report writing, audits and being the named manager for the site. About the Service
This is a pilot working as a Multi-Disciplinary Team (MDT) with housing teams and Drug and Alcohol Services across District and Borough councils. These teams will be working with individuals who are presenting as homeless and have a number of Complex Needs.
Purpose of Post
Providing advice, information, onward referral and holistic support to clients who are experiencing mental ill health or need support with their mental wellbeing. These will be clients who are presenting to District and Borough councils as homeless and need intervention and support with their mental health and remain a source of independent support for all clients. Ensure that the safety and wellbeing of the client is monitored and reviewed regularly and that a person centred recovery orientated and trauma informed approach is embedded approach in all aspects of the roles.
Service Objectives
- To improve the mental wellbeing of people experiencing mental ill health.
- To increase early access to help for people experiencing mental ill health in, by providing a clear and effective pathway to services provided by Herts Mind Network and other third sector and statutory providers.
- To contribute to an improvement in individual mental wellbeing.
- To remain a source of independent support for all clients.
- To reduce the use of police, ASB, tenancy enforcement, housing needs and environmental health services by clients whose primary need is related to mental ill health.
- To reduce the use of crisis services by people experiencing mental ill health without positive outcomes for the individual.
- To provide an assertive outreach approach for clients who are in crisis and/ or those that may have complex or multiple needs.
- To work collaboratively within an MDT consisting of CGL, District and Borough councils, housing teams and other professionals.

Women and Girls Network (WGN) was established in 1987 and has spent over 30 years supporting women and girls across London affected by gendered violence. This includes childhood sexual abuse, domestic violence, rape, prostitution (including trafficking and sexual exploitation), female genital mutilation (FGM), and so-called 'honour' based crimes such as forced marriage.

What you'll do:
Work proactively with the team to handle the service caseload and support an effective team approach to meeting each customer's identified needs in a recovery-focussed way
Assessing referrals received by the service, within the 4-hour target, and providing a rapid response to these - supporting the team to identify and promote positive risk management.
Contribute to a positive service environment, ensuring the service is supportive and a place of safety and stability for customers. This includes working flexibly and being proactively in delivering, reviewing and arranging effective support for emerging needs and risks in order to prevent escalating concerns
Inducting new customers, and helping orient them to the community and local area
Completing/supporting housing referrals as required, supporting customers with budgeting tools and providing support to customers in accessing any benefits or addressing any issues such as benefits.
Building a relationship with the local borough and any key agencies. For example: no recourse to public funds teams, homelessness teams, community mental health teams etc.
Support planning, including ward management liaison, medication profiles and medication reviews as required, identifying the goals and steps to support customers to manage their own support and risk and ensuring these are correctly documented within Look Ahead's paperwork.

We have a vacancy for a Hospital Mental Health Outreach Worker to join our team. This is a full-time position.
About the Project
Herts Mind Network (HMN) has formed a partnership with Hertfordshire Partnership Foundation Trust (HPFT) to deliver a one-to-one outreach service for people who are in hospitals across Hertfordshire.
We have two existing arms of the project – one based at Kingfisher Court and Albany Lodge facilitating discharge to the community from inpatient settings, and the other based at Watford General Hospital & Lister Hospital supporting those who have attended A&E in mental health crisis and have a dual diagnosis.
This is a new arm of the project focused on serious violence, with one worker based at Watford General Hospital and one worker based at Lister Hospital working in A&E departments and wards facilitating discharge for people aged 16 - 25 who have been admitted to hospital due to being a victim of serious violence.
About the Role:
The aims of the Bounce Back service are: to provide up to 16 sessions of emotional and practical support, advice and information to patients. We will visit patients on the ward and continue the sessions in the local community and in patients homes once discharged. We will facilitate a smooth and timely hospital discharge and focus on re-integration back into the local community.
The Bounce Back Worker will work with the HMN Community Outreach Teams and the hospital staff to ensure patients are offered advice, information, and holistic support. The Bounce Back worker will use a recovery orientated approach working with patients on the ward, and in the local community, including in people’s homes.
The recovery approach includes empathy, warmth, acceptance, authenticity, compassion and humanity.
Key Responsibilities
- Provide practical and emotional support to encourage patients to develop their independence within their local community.
- Facilitate a smooth discharge from hospital back to the community.
- Have a solid understanding on the impact of serious violence on an individual’s mental health.
- Have a working knowledge on the areas of serious violence including knife crime, cuckooing and county lines.
- Monitor outcomes and evaluate the service.
- Ensure that the safety and wellbeing of patients using the service is monitored and reviewed regularly.
- Embed a person centred and recovery orientated approach in all aspects of the roles and responsibilities.
