This is an exciting opportunity to join our new service supporting Unpaid Carers across Lewisham to improve their health and wellbeing, and access support that meets their needs.

The team works with unpaid Adult Carers, Young Adult Carers and Young Carers, completing statutory and holistic assessments to understand people’s individual circumstances and level of need. You will carry out home visits and community meetings, co-produce support plans, and develop effective relationships with Unpaid Carers, families, and professionals.  

You’ll build on your local knowledge to signpost and facilitate access for Unpaid Carers to support services, including financial and benefit advice, housing, health and safety home systems, and leisure and social activities. You will also support the development of escalation plans so that Unpaid Carers are prepared for unforeseen circumstances that would impact negatively on ability to care.

Applicants should have relevant personal or professional experience of working with Unpaid Carers and/or vulnerable adults or children in a social care, health, education or voluntary and community sector. You’ll need a practical understanding of barriers faced by Unpaid Carers, and an awareness of cultural differences and access to services. Experience of working in mental health or dementia services would be an advantage.

You should be an excellent communicator and able to motivate and empower others. This is a full-time role.

You will be part of the MYcommunity Linkworkers team. Your main role will be to:

• Provide personalised brief intervention and social prescribing support to individuals, their families and carers.

• Meet clients on a one-to-one basis, making home visits where appropriate and allowing clients the time and space to discuss ‘what matters to me.

• Provide person-centred support to enable clients to identify their health and wellbeing needs and goals. Co-produce support plans with clients to discuss what’s important, where they would like to be in the future and how we can support them to achieve this. 

• Manage and prioritise a caseload to ensure clients' needs, priorities and urgent support are met.

• MYcommunity Lambeth works in partnership with a large number of statutory and community services and organisations across the borough. You will link clients with appropriate services to meet their needs. It is vital to have a strong understanding of when it is necessary to refer someone back to health care professionals/ agencies when the client’s needs are beyond the scope of the MYcommunity team.

At our independent charity based in London Borough of Sutton, the Head of Health Services will lead on and develop Age UK Sutton's community health commissions including; our Hospital Discharge service which provides practical support to older people in the first days and weeks after discharge from a hospital admittance; Dementia Peer and Practical support service, delivering information, advice, peer support and some practical support to those living with or affected by Dementia; and the innovative Older Person's Mental Health Peer Support service operating in partnership with Sutton's Community Mental Health services and South West London Mental Health Services.

The Head of Health Services will have delivery objectives to achieve with their operational team, with current direct line management of 9 staff over 3 services and have strategic development opportunities to deliver excellent services to the older population in London Borough of Sutton.

• You will undertake casework, assisting clients to access their entitlement to welfare benefits and other sources of financial help, up to and including tribunal representation.
• Advice will be delivered virtually according to the service user’s needs and preferences. This will include phone, secure email or secure video calls.
• Help to design welfare benefit information leaflets and upskill referrers, preparing and providing training on our referral processes.

We are looking for an experienced fundraiser, to lead on Neuroscience related fundraising for the Departments of Psychiatry and Clinical Neurosciences as part of the friendly team working across the Medical Sciences Division at the University of Oxford.

The Medical Sciences Division at Oxford has been ranked number one in the world for the last twelve years in the Times Higher Education World University Rankings for clinical, pre-clinical and health sciences. Recent accomplishments of the Medical Sciences Division include its research into the COVID-19 pandemic, most notably through the delivery of the Oxford–AstraZeneca vaccine, which is estimated to have saved over 6 million lives.

About the role:

This fundraiser will work closely with the Heads of Departments and academics in the Departments of Psychiatry and Clinical Neurosciences. The post will focus primarily on funding for research, academic posts, scholarships and infrastructure related to Neuroscience which includes Mental Health and neurological disease (e.g. Motor Neurone Disease, Dementia, Parkinson’s, Stroke and Epilepsy). The fundraiser will work on special projects including the possible major capital redevelopment of the Warneford site. While an interest in medical research and neuroscience is essential, you do not need to be a sciences graduate, just an enthusiast for knowledge.

South East London Mind and Bridge Support are working in partnership with Oxleas NHS Foundation Trust to deliver the Greenwich Mental Health Hub.  This will form part of the new transformation of mental health services in the Royal Borough of Greenwich under the NHS Long Term Plan. The service will bring together the expertise of local primary, secondary and voluntary sector mental health care providers with the aim of providing flexible, holistic and integrated services for adults with mental health problems.

The integrated team will provide support to people with long-term mental health problems within a primary care and community mental health setting, helping people develop their independence, self-management skills and achieve their recovery goals.

We are looking for a Senior Mental Health Advisor with experience of working within mental health services. Applicants should have strong line management skills alongside experience of completing assessments, managing a diverse caseload, working in a busy work environment, and possess effective skills in communication. An ability to work independently but collaboratively, to be proactive and show initiative is essential for this role.

Job Description

Job Title: Project Coordinator

Salary:£26,000 + 5% pension contribution (above Living Wage)

Hours of work:Part-time, 21 hrs a week. Work flexibly over 5 days – negotiable.

Contract length:     One year (subject to extension)

Holidays:Starts at 28 days + bank holidays +3 days for Christmas (pro-rata)

The role

The post of Project Coordinator is critical to the work we do and this is a great opportunity to make a real difference to improve health and care services and make them work better for the patients and local people of our city.

You will be responsible for ensuring the delivery of a variety of patient-focused projects on different topics, against quality, time and cost constraints. You will manage projects from inception to delivery including publicising, following up on any recommendations, and promoting the impact that we make. As we are a small team you will also be expected to contribute collectively to a shared workload to keep the team working effectively.

Some of the projects which Healthwatch Brighton and Hove projects have delivered in the last year include:

• Improving home care services
• Improving dementia services
• Improving maternity services
• Tackling health inequalities

We have a strong team of employees and volunteers and you will be supported by our Head of Research and CEO.

Work Location

Our office is at Community Base, 113 Queens Road, Brighton. We’re a home-based team, but hold team meetings at the office every Monday and you will be expected to work in the office on this day. There is also an option of office working on Thursday and hot-desking in either the office or Community Works’ office space on other days.

We will provide line management, technology and equipment support to enable home working. Applicants must be prepared to undertake this role in either form of work setting.

Main responsibilities and duties:

1. To be responsible for all aspects of planning and delivering a portfolio of time-limited projects. Typically, this will include:

• scoping evidence-backed projects which support our wider strategic aims
• contributing to written project plans
• Using Excel or SPSS to analyse data
• leading on undertaking service review fieldwork including interviews, ‘Enter and View’ visits (where we visit services in person) and Mystery Shops of our local hospitals
• supporting others e.g. volunteers and other members of the team to undertake fieldwork
• drafting clear, well-written and evidence-backed reports
• developing workable recommendations
• presenting findings at internal and external meetings
• delivering promotional and publicity materials to support projects
• working in partnership with other groups to deliver projects.
•  All of the above will be carried out with the support of the Head of Research.

2. Proactively undertake any duties that assist with the smooth running of Healthwatch Brighton and Hove e.g. representing the organisation at meetings with a wide variety of stakeholders including the NHS, City Council, voluntary and community sector partners, taking part in promotional events, leading and supporting volunteers in project and representative work. This post will shadow other Project Coordinator’s before attending meetings alone.

3. To take shared responsibility with the rest of the team for developing and delivering the Healthwatch Brighton and Hove workplan.

4. Managing specific pieces of routine work that support the work of Healthwatch Brighton and Hove. Examples may include, supporting the production of an annual report, contributing to newsletters, helping to maintain our social media presence and updating our web site, etc.

 5. Undertake other duties as may be appropriate to the role, in accordance with the values, policies and procedures of Healthwatch Brighton and Hove as well as the ‘Ways of Working’ statement attached to this Job Description.

• Experience gained from work or personal experience of delivering and planning small-scale time-limited projects to deadline and/or budget
• Experience of reviewing services and/or conducting research fieldwork (planning and delivering research)
• Experience of project management and project planning
• Experience of preparing and presenting written reports that include detailed information to a diverse range of audiences.

Knowledge

Job Purpose: To develop and manage, to a high standard, Age UK Bromley & Greenwich’s Information and Advice Service. To be responsible for the effective delivery of the I&A Service which will be delivered through a range of channels, including face to face, telephone, email, digital, and other means, across the Bromley and Greenwich boroughs.

Key Responsibilities:

Line management and supervision

• Provide line management and supervision to advisers and support staff.
• Supervising team advice sessions and casework
• Manage the practicalities of the advice session and ensure adequate staffing and resources.
• Undertake monthly supervision with individual team members and annual appraisal.
• Ensure referrals are actioned and clients are supported in a timely manner.
• Develop team competence in key advice areas. (Benefits, Housing,  Social care)
• Provide technical support to advisers and identify areas of further development for individuals and the team.

Support learning and development

• Support the team to embrace continuous learning and development and provide timely feedback on cases reviewed.
• Make recommendations for staff training and development areas of need.

Staff recruitment and development

• Create a positive working environment in which equality and diversity are well-managed, dignity at work is upheld and everyone can do their best.
• Participate in recruitment and selection activities as delegated.
• Undertake new  staff induction.
• Ensure the team receive appropriate support, supervision and appraisal and have opportunities for continuous learning and development.

Quality assurance and reporting

• Undertake Independent File Reviews ( IFR) and monitor the quality  to meet quality standards and service level agreements
• Maintain and develop  QAS/ AQS standards incl. renewal process.
• Ensure remedial and developmental issues are identified and acted on to develop individuals, improve the quality of advice, and ensure clients do not suffer detriment due to poor or inadequate advice.
• Keep technical knowledge up to date and provide technical support to advisers.
• Maintain a case load as demonstration of ongoing personal development and learning.
• Undertake advice work as required e.g. during staff absences.
• Provide statistical reports on outputs and outcomes as required by funders and management
• Review and implement outcome measures for the work of the I&A team, including an annual survey of clients.

Partnership working and development

• To manage networking/ liaison with other local key stakeholders and organisations and feed into wider local engagement and community development work.
• Develop effective partner relationships with referral agencies across  both boroughs, and to refer/ signpost cases to external agencies as appropriate.
• Represent the organisation at outreach and community events as required, continuing to raise the organisational profile and develop wider awareness of our services.
