As the post-holder, you will be working to maintain an environment of support for adults with complex needs. The successful candidate does not have to have direct experience in a similar environment. We are seeking a candidate who is willing to learn and who has a strong understanding of the needs of homeless and vulnerable people, especially those with complex needs. Dedication, personal resilience and a relationship based, non-judgemental approach is essential.

In this role you will:

• Support clients to liaise with external services like GP, substance misuse teams, mental health teams, social services.
• Support with all necessary skills to enable an individual to move on to more independent accommodation,
• Have an experienced Team to support you.
• Receive comprehensive training and full induction
• Be working shifts which are normally 8-4 or 2-10pm and you will be expected to work some weekends and Bank Holidays.
• Carry a caseload of clients and be their lead worker.

About you

We are looking to recruit a Project Worker to our diverse team at Sutton Vulnerable Adults.  Reporting to one of the Deputy Manager you will be responsible for providing person-centred support to clients to manage their tenancies and access independence.

You will be able to liaise effectively with external agencies, have clear communication skills, a non-judgemental approach, and a good working knowledge of homeless clients and the challenges they face.

The Barnsley Street Project is a pilot service funded by NHS money following a bid by East London NHS Foundation Trust (ELFT). The service comprises of Look Ahead staff, ELFT staff, and Tower Hamlets Mental Health VCSE Alliance, formed of 17 local VCSE organisations. The Barnsley Street Project operates 24 hours a day, 7 days a week to provide support for people in the catchment area of one primary care network in Bethnal Green, Tower Hamlets. It provides a variety of psychosocial and clinical interventions, including the provision of 6 Hospitality Beds for short term support and intervention.

Support Workers within the Barnsley Street Project will be expected to support individuals requiring support from the local community that walk in to the service and those staying in our hospitality beds. This support may include providing tailored and co-produced practical, emotional, trauma-informed support to individuals with a variety of challenges relating to their mental health. This may include the management of symptoms, social support, navigating benefits and welfare systems, practical skills and signposting to specialist services in the local authority for intensive or specific support as required.

What you'll do:

• Building supportive, trusting relationships with customers and creating a positive atmosphere
• Working proactively with other members of the team to handle the service caseload and administrative responsibilities
• Conducting key work sessions as required, that are innovative and engaging in order to achieve individual goals
• Ensuring ongoing assessment and management of risks associated with customers within an attitude of 'positive risk taking'
• Providing support with daily living activities, including practical assistance to support customers to independent living e.g. budgeting, cooking, establishing a night time routine
• Supporting customers to maintain a healthy environment and where appropriate assisting them to clean their accommodation and maintain good personal hygiene
• Assisting in the recording and reporting of customer incidents
• Managing customer referrals, completing assessments and obtaining and maintaining all relevant paperwork
• Developing productive relationships with partner organisations to improve service outcomes
• Adhering to lone working protocols and maintaining high levels of awareness in relation to the health and safety of yourself and colleagues
• Involving customers in the design, development and delivery of the service

Context

Childhood First enables children and young people to recover from early life abuse and relational trauma, to enjoy life and to achieve their potential. We do this through the provision of specialist therapeutic residential care and treatment, together with family and network support.

Greenfields House is a specialist residential children’s home providing a programme of care, treatment and education for up to ten children aged between 5 and 13 years.

Role Description

The role of the Placement and Family Support Worker is to directly support, maintain and develop the network around the child, liaising, building relationships and communicating between multiple agencies and multi-disciplinary roles, these include, the local authority, social workers, family members, carers and significant others and the child in placement.

Within the community, the role is closely linked with each child in placement and will closely work with the Community Director and the senior management team to ensure that it meets the therapeutic and educational needs and interests of each child. This will necessarily include working closely with professionals and developing their expertise in a psychoanalytic-systemic approach to care, education and treatment.

Responsibilities include:

• Ensure that plans for the process of assessment, co-ordination and delivery of placement and family support and transition plans are developed and managed on a day-to-day basis to agreed standards.
• Co-ordinate and facilitate contact arrangements, linking with networks and families in a manner which is integrated with the community’s existing relationship and contribute to and develop its professional effectiveness.
• Contribute to the holistic assessment of needs of children, families and networks.
• Devise and develop tailored packages of support based on assessment.

Policy and Public Affairs Officer (maternity leave)

CSAN is the official agency of the Catholic Bishops’ Conference tackling the root causes of poverty and injustice affecting people who live in England and Wales. We’re facing a steep rise in poverty and significant pressures on social, economic and church resources. Currently, the Church is called to raise a prophetic voice against poverty and the rhetoric of division and work for the building of a more just society.

Over the last 20 years, CSAN has built up a network of 50 diocesan and direct service charities with a combined annual spend on social mission estimated at £400m, excluding the work of schools and religious congregations. Our members build up local community life in diverse ways, and many of them support individuals in difficulty, including with housing, prison and detention, social isolation, ill-health, violence, disability, employment, care, therapeutic and welfare support.

The key responsibilities of the post are:

1. To scan the social policy environment to capture developments in legislation, consultation papers and Bills relevant to the Caritas network for the purpose of comprehensive tracking and briefing.

2. To deliver an effective programme of Catholic advocacy and political campaigns that contributes to the common good, with particular attention to the priorities of the dignity of workers, child poverty, social care and end of life; supporting and connecting well with the team’s other activities, and where possible with the CSAN membership’s priorities,

3. To draft campaign and advocacy materials for the range of media channels used by CSAN and support the CEO in engaging with the press and approaches from campaigning organisations including contributing to CSAN’s social media networks.

4. To provide admin support and contribute to CSAN’s Alliances as required, especially the Advocacy Alliance and the Criminal Justice Alliance with information and education on policy and legislation.

5. To ensure that our advocacy is consistent with the Bishops’ understanding of the Church’s role in society and supports the priorities of the Bishops’ Conference, especially the Department for Social Justice.

6. To act as a conduit of information and communication between the Bishops’ Conference and the CSAN members, under the guidance of the CEO, assisting CBCEW where possible in gathering information and the lived experience of poverty.

7. To develop and contribute to practice materials for the network, especially in the areas of campaigning, advocacy, social policy and formation for mission.

8. To oversee monitoring of the Catholic press and relevant wider networks for relevant articles, developments and campaigns.

9. To support the general work of CSAN as required by the CEO, including leading in the development and organisation of the charity in specific areas subject to skills and experience.

The work of the small national team requires a high level of integrity and teamwork, respect for and capacity to navigate complex civil and church contexts at pace, and a stable commitment to personal formation and training. Our Policy and Public Affairs Officer (maternity leave) will bring a professional track record of relevant research and analysis, production of compelling communications, and diligent administration. Shine supports a community of over 15,000 members living with spina bifida and/or hydrocephalus, including 5,000 children and young people (0–25).

You will be delivering high-quality support and creating opportunities for children and young people living with spina bifida and/or hydrocephalus and their families/carers. Supporting Shine members to lead healthy, independent, and fulfilling lives by improving condition management and fostering connections within the Shine community. 

This role will focus on children and young people (0–25), you will be primarily working within the Children, Young People and Families team. However, there will be occasions when you will work across age groups to ensure the best outcomes for our members. 

The role is home-based but you will be required to attend regular clinics in London including GOSH (Great Ormond Street Hospital). Other travel across Southern England including Bristol, Devon and Hampshire may be required. There will be occasional travel required across wider areas and nationally including attendance at events, conferences and meetings at our head office in Peterborough.

The Haringey Wellbeing Network shall be working in partnership with the Haringey GP Federation, focusing on improving physical health outcomes for those living with Severe and Enduring Mental Illness (SMI) within the BAME community.

The aim of this work is to create a seamless and integrated physical health check service, which improves outcome targets and builds better rates of engagement within the BAME community.

The purpose of the BAME Community Advocate is to support the work of the Haringey GP Federation, which is commissioned to focused on improving the uptake of Physical Health Checks with people living with a SMI. The Community Advocate will enable a joined-up approach in identifying and coordinating patient care with local community groups and secondary care services. In addition, the community advocate is expected to manage the SMI registers for the Haringey Wellbeing Network and report to the Federation.

Tenancy Support & Financial Wellbeing Project Worker  
Hours of work: Full-Time, 37.5 per week (5 days per week)

Wintercomfort works with people who are homeless, at risk of homelessness or with a history of homelessness. We provide services to aid every stage of recovery -from immediate basic welfare needs to long term help in identifying and dealing with the problems which are undermining their stability, and enabling them to engage with education, employment and specialist health services.  National statistics rank the numbers of rough sleepers in Cambridge within the highest 20 UK local authorities. 

Wintercomfort is the only day-time service in the city, providing year-round advice and support for homeless or vulnerably housed people. Over the past three decades Wintercomfort has continued to grow and adapt to meet the needs of the homeless community. 

Our specialist Housing Team provides expert advice and support to help people successfully navigate the search for housing and crucially, support them to sustain life away from the street. 

About you and the Role 

As a member of Wintercomfort’s Project Worker Team, reporting to the Senior Tenancy Support Officer the Tenancy Support & Financial Wellbeing Project Worker is responsible for delivering Wintercomfort’s specialist range of 1:1 Pre and Post Tenancy Support, implementing proactive, holistic strategies designed to prevent people from losing their homes, whilst simultaneously providing them with the financial tools and capability needed to sustain stability, aimed at empowering Service Users to move out of the spiral of homelessness and back into mainstream society, preventing homelessness and foster financial inclusion.

Key Responsibilities 

• To provide emotional and practical support and assistance to prepare and assist Service Users to move into accommodation once a placement is confirmed, working with internal and external colleagues. To work in a flexible manner within Wintercomfort and the wider community to provide outreach and support services in person, occasionally in exceptional circumstances this could include in their own home when accompanied with external partners also includes via text, email, telephone and video calls.   
• To provide a wide range of resettlement-based support along with the Senior Tenancy Support Officer to ensure Service Users access the most suitable housing pathway for their individual needs. To take the lead on all housing related referrals for Service Users. To ensure all suitable Service Users are registered and supported on Homelink. 
• To provide crucial assistance to tenants, focusing on maintaining tenancies, managing money and debt and improving overall financial stability – including, signposting, budgeting, welfare benefits checks, rent arrears support, setting-up and managing utilities, housing related life skills, offering guidance and support to enable Service Users to maintain their own homes, aiming to prevent eviction and enhance quality of life. 
• To provide on-going long term tenancy support to Service Users moving into independent living, this includes private rental housing or alternative housing pathways without any on site support. To assist with hand over post tenancy support to accommodation providers at a suitable point in the Service User support pathway, with the aim of enabling long-term independence.
• To ensure all Service Users have access to external grants and any available funding to support their individual needs to maintain their accommodation. Signposting and assisting with applications for funding to assist people moving into accommodation are supported with the appropriate items: floor coverings, white goods, basic furniture and home furnishings. 
• To support residents to keep their current homes, including mediation with landlords, accessing legal support on evictions, addressing anti-social behaviour, identifying high risk groups targeting support towards vulnerable groups such as those fleeing difficulties, leaving prison, hospital or care systems. 
• To ensure that accurate records are kept, documenting the support services provided to Wintercomfort’s service users onto the Salesforce/Inform system. Ensuring that record keeping meets the requirements of internal and external monitoring of outcomes, assisting with reports and case studies to the fundraising team.

Horsham Y Centre provides 24-hour supported housing services offering medium levels of housing-related support for young people aged 16-25 with 50 bedspaces and shared communal facilities. We adopt a trauma informed and psychologically informed approach to supporting our residents to help them build essential life skills, identify their goals and work towards aspirations so that they can move on to enjoy fully independent futures. We have a dedicated team of Support Workers, Night Workers and additional Bank Workers who provide support, guidance, and signposting around areas such as housing, budgeting, living skills, jobs, relationship building and wellbeing. Situated in the middle of Horsham, the service has strong links with and contributes to the local community.

We are looking for a Housing Night Worker to join our team at Horsham Y Centre. You will respond promptly to any reported incidents, accidents, or concerns, carrying out thorough checks and taking appropriate action to ensure the safety and wellbeing of everyone on site. You will also take a proactive approach to preventing anti‑social behaviour, working closely with support staff to maintain a positive environment. In addition, you will carry out one‑to‑one interventions with young people who may be experiencing a mental health crisis or who require medical assistance, ensuring they receive timely and appropriate support.

Main areas of responsibilities are:

Safety and security

• Provide a friendly, professional presence and ensure the building remains safe, secure and welcoming at night.
• Support resident wellbeing by upholding health and safety procedures, house rules and tenancy requirements.
• Monitor risks and respond to incidents calmly and appropriately, recording and reporting to the relevant teams. 

Resident engagement

• Welcome residents and offer informal, responsive assistance to help them develop confidence, independence and life skills.
• Encourage residents to share their views and engage with available services and opportunities.
• Assist with evening groupwork sessions that promote learning, wellbeing and positive development.

Administration and housekeeping

• Complete essential admin tasks including data entry, case notes and record management.
• Carry out routine health and safety, compliance and room checks.
• Log maintenance issues to ensure the building remains safe and well maintained.

General

• Work collaboratively on a four‑on, four‑off rota, providing consistent, non‑judgemental support overnight.
• Maintain a calm, professional approach and handle challenging situations effectively.
• Take part in ongoing training and reflective practice to support psychologically informed care.

This role involves lone working, so you will need to be confident in decision‑making and in following procedures. We are wanting to recruit a motivated and experienced support and recovery worker to support homeless and vulnerable guests at our daily drop-in, and in Hope House - our 4 bed supported house 

Responsibilities

Our support workers both work with guests in both Drop-in and in Hope House, but each takes a lead responsibility in one or the other of these projects. This post holder leads on the housing management responsibilities for Hope House.

Drop-in responsibilities :

· Welcome and assess first time guests, identifying priority needs.

· Signpost and support guests to access key statutory and voluntary services, especially housing, benefits, homelessness services etc

· Provide practical support and advice to guests , for example with helping with forms, bill enquiries, budgeting .

Hope House responsibilities

- Vet the suitability of potential residents and support them with their transition in/out of Hope House

· Hold weekly individual support sessions with residents

· Ensure tailored support plans (using Homeless Outcomes Star) are in place for each resident

· Identify “next step” housing options for residents and work with the residents to access these.

Oversee the safe and efficient running of Hope House, taking into account any relevant Supported Housing/ HMO specific requirements.
