This role offers the opportunity to support and develop social action and social justice work across the Diocese of London. Working as part of the Compassionate Communities team, the postholder will help churches and communities respond to local needs by connecting, equipping and resourcing them to grow their impact.

The role involves working across a wide range of social action themes, supporting partnerships, training, events and engagement activities. It also includes helping to communicate and share the work churches are doing across local communities and contributing to wider diocesan priorities.

Job Summary

The Social Action Lead supports the development of social action and social justice work across the Diocese of London. The role works closely with churches, diocesan teams and external partners to support community engagement, strengthen partnerships and help deliver initiatives responding to local needs and challenges.

Job responsibilities

· Support churches and communities with advice, resources and connections relating to social action and social justice

· Develop and support diocesan training and engagement activities across key social action themes

· Organise and contribute to events, meetings and partnership activities

· Build and maintain relationships with churches, charities and external organisations

We are looking for new team members to join our service supporting Carers across Havering to improve their health and wellbeing, and access wider support that meets their needs.

This is a great opportunity to support local communities, working with Adult Carers and Young Adult Carers and helping them to make informed choices, build confidence, sustain their independence, and access support that meets their individual needs. You will carry out assessments to understand people’s individual circumstances and level of need, working together to co-produce support plans. This will involve meeting Carers in community settings and conducting home visits. The team also facilities coffee mornings and activities offering Carers opportunities for peer support and to take a break from caring.

You’ll build on your local knowledge to signpost and facilitate access for Carers to support services, including financial and benefit advice, housing, health and safety home systems, and leisure and social activities. You will support the development of escalation plans so that Carers are prepared for unforeseen circumstances that would impact negatively on ability to care.

We have a part-time roles available (22.5 hours), and applicants should have relevant personal or professional experience of working with Carers and/or vulnerable adults in a relevant sector. It’s essential that you are confident using IT and maintaining excellent records. You’ll need a practical understanding of barriers faced by Carers, and an awareness of cultural differences and access to services.

We are looking to recruit a Senior Welfare Rights and Housing Advisor to join Camden Disability Action’s Advice Service.

You will have at least two years’ experience providing specialist-level welfare rights advice and experience of providing housing casework. The role involves identifying opportunities for income maximisation, supporting clients to ensure benefits are correctly assessed, and assisting with mandatory reconsiderations and appeals. You will also support Disabled people with housing-related issues, including homelessness prevention and access to suitable accommodation.

You will work holistically, managing complex cases and advocating for clients across statutory services, specialist advice agencies, and community organisations. The role provides social welfare rights advice, advocacy, and casework support within CDA’s remit and does not involve the provision of regulated legal services.

You will be committed to developing your understanding of the Social Model of Disability and share our vision for a radically inclusive world where people are not disabled by the society in which they live.

About the Role

A stimulating and rewarding opportunity has arisen for a Practitioner Psychologist with a Forensic background. You will work alongside the Group Psychological Lead and other members of the team within our Independent Approved Premises (IAP) to deliver psychological services to residents and staff. In this role, you will apply your skills and knowledge in building new teams and expand our operational delivery within our IAP. You will play a strategic role in our organisational growth with new services, with a focus on shaping service delivery; offering specialist advice, supervising assistant psychologists and postgraduate research.

Our IAP's support residents who have left prison and are based within a residential setting to reintegrate into the community. They are given support and guidance to aid their reablement, resettlement and rehabilitation post prison. You will contribute towards them achieving goals necessary for their personal development and move on within the community. 

Key Responsibilities include:

• Providing high quality psychological services, contributing to the delivery of Approved Premises aims as specified by the Ministry of Justice.
• Ensuring all services are delivered in accordance with HCPC standards of conduct, performance and ethics.
• Demonstrating clinical leadership by setting direction for the psychological service and driving high standards for quality, safety and ongoing improvement.
• Exercising clinical responsibility for residents’ psychological care, showing skill and judgement in selecting appropriate assessments and interventions.
• Consulting with other parties contributing to a resident’s diagnosis, formulation, treatment and risk management in the community.
• Create opportunities to collaborate with peers including AP-based psychologists nationally and OPD pathway psychologists delivering services to APs.
• Model and facilitate a welcoming and inclusive environment where everyone feels valued and respected.

This team offers support to 52 residents across services in Gillingham, Chatham and Rochester for single homeless people andthose who may have a history of offending, these projects are commissioned by the Medway Council Housing Related Support team.  

As a Support Coach, you will identify individual resident needs and provide person centred, tailored support to residents to help them overcome their personal challenges and achieve their goals, during which we support them to improve their quality of life, access treatment programs, and secure stable housing. Our Medway homeless project focuses on people with a housing related support need we encourage service residents to develop coping strategies and supportive relationships to empower them to maintain their mental health, be a part of the community, and achieve their personal goals. Support enables service residents to significantly improve their opportunities and develop fulfilling and settled lives within the community. Key Responsibilities Include:

• Keep support plans updated on our online portals as appropriate

• Holding a caseload, conducting key work sessions which are personable to their needs

The Role

Social prescribing link workers will work as a key part of the primary care network (PCN) multi-disciplinary team. Social prescribing can help PCNs to strengthen community and personal resilience and reduces health and wellbeing inequalities by addressing the wider determinants of health, such as debt, poor housing and physical inactivity, by increasing people’s active involvement with their local communities. It particularly works for people with long term conditions (including support for mental health), for people who are lonely or isolated, or have complex social needs which affect their wellbeing.

Responsibilities include:

• Working with direct supervision from the Programme Lead/Senior Link Worker and input from the PCN’s Clinical Lead GP, take referrals from staff in GP practices within the local PCN.
• Provide personalised support to individuals, their families and carers to take control of their health and wellbeing, live independently and improve their health outcomes, as a key member of the PCN multi-disciplinary team.
• Developing trusting relationships and taking a holistic approach, based on the person’s priorities and co-produce a simple personalised care and support plan to improve health and wellbeing, introducing or reconnecting people to community groups and statutory services.
• Work collaboratively with all local partners to contribute towards supporting local community groups and organisations, making themselves aware of local resources and “assets” through mapping and then building a relationship with these organisations.  Also sharing intelligence regarding any gaps or problems identified in local provision with commissioners and local authorities. 
• Social prescribing link workers will work collaboratively with GP’s and have a role in promoting social prescribing and educating non-clinical and clinical staff within their PCN multi-disciplinary teams on the benefits of social prescribing
• Raise awareness of other services available within the community and how and when patients can access them. Linking into the work of Neighbourhood Network Schemes, locality Adult Social Care & Community Mental Health Teams.

About You

As a Social Prescribing Link Worker, you will have a Level 3 qualification in a relevant field (Health & Social Care, Health Trainer, Health Improvement, Community Family Worker, Psychology) or be working towards/willing to work towards.

You will have:

• Demonstrable commitment to professional and personal development
• Experience of working directly in a community based context (including unpaid work)
• Experience of supporting people with a mental health need and or complexities/vulnerabilities
• Experience of delivering one to one support
• Experience of partnership/collaborative working and of building relationships across a variety of organisations
• Knowledge of VCSE and community services in the locality
