The postholder will be responsible for all aspects of the project, including design, delivery, reporting, and will have the scope to further enhance the programme to continue to deliver great outcomes, seek further partners and also seek further funding to ensure sustainability.
The postholder will become a key part of the Operational Team within Red Rose and work alongside Team Leaders and Engagement staff to ensure the Leadership Programme is suitable to the needs of our members. They will also work alongside other training staff to further develop our internal and external training offers.
In addition, the Project Lead will also be expected to deliver further training sessions to our members in areas such as Safeguarding, Boundaries, ICT, etc
They will also attend Lancashire User Forum meetings to inform the community of our Leadership Programme and the wider training package we offer. They will consult with the LUF to ensure our training offer meets the needs of this community.
We would also like the postholder to seek to develop partnerships with external partners from the public and private sector. These partnerships would be utilised for:
- Potential work experience placements for Leadership participants.
- Mentoring opportunities for external staff to mentor Leadership participants.
- External projects for Leadership participants to engage in.
- Utilising partners expertise to deliver and add value to the Leadership training sessions.

- Breaking down stigma.
- Offering addiction and recovery support to external organisations.

The Liaison and Diversion (L&D) Service works with people of all ages (including children and young people) with a wide range of vulnerabilities including poor mental health and learning disabilities, who have offended or are at risk of offending. We support service users who have contact with police stations, the courts, prisons and probation. The Liaison and Diversion service is delivered by Wakefield Council across the West Yorkshire Police force area. The team is a multi-agency, multi skilled team. Touchstone is one of the partners in the service.
The aim is to work together to reduce the risk factors for reoffending by providing short-term support and signposting service users into longer-term support services.
L&D service across West Yorkshire will operate 7 days per week from 08.00 to 18.00. Project Workers will be required to work shifts to ensure appropriate cover, this will include regular weekend and evening work.

Background to the role
Safe Haven is an exciting new development for Leeds Survivor Led Crisis Service and Touchstone as the first of our crisis services to be open 24 / 7. Unlike LSLCS’ other services (which are all self-referral), Safe Haven will be accessed via Leeds NHS Crisis Resolution and Intensive Support Service, and the Older People’s Support and Intensive Home Treatment Team (for those aged over 65).
Leeds Safe Haven will be open 24/7 to provide an alternative to hospital admission for people in acute mental health crisis. The service will have space for 6 guests (who may stay for up to one week) and 4 day visitors (who will come only in the day time, again for up to 7 days). The service will be staffed by a manager and deputy manager, a team of approximately 18 crisis support workers, and an inclusion worker (with a focus on outreach to disadvantaged groups).
About the role
From a service perspective, the post-holder will promote engagement with local communities and organisations to ensure that all groups are aware of the service and to provide outreach to engage those people who are under-represented. They will be a resource for the team to identify gaps and needs and consider the wider mental health system and access to relevant advice and training.
The post-holder will be on the rota to deliver support at the Safe Haven and be required to offer person-centred support to people in crisis. The Crisis Support Worker element of the post provides a range of practical and emotional support to people in acute mental health crisis, face-to-face. The team will usually work independently, although the Manager and Deputy Manager will be available during office hours to provide, supervision, support and guidance.
- To deliver support to guests/visitors in the house, provide a resource in the team on diversity issues and to work with external stakeholders.
- Hours: 30 hours per week:2 x 8 hours support work shifts per week, the rest are outreach hours. 1 x 3 hour monthly reflective practice and team meeting, which occurs on the second Wednesday or third Monday 6:30pm-9:30pm.

We are recruiting a Lead Worker to join our City Outreach Team. The team forms a significant part of the borough’s complement of services working with rough sleepers. The City Outreach team works to rapidly reduce the number of people rough sleeping in the borough, by providing short sharp interventions to support people to access accommodation, and ensure that the appropriate support is in place to sustain this.
This role will enable you to use your existing knowledge and experience to move into a role where you will have more influence and responsibility within a team, to work towards quick outcomes, to support rough sleepers off the streets. Working in this team will give you an opportunity to make a real difference to rough sleeping.
You will be responsible for:
- Managing a caseload of rough sleepers, identifying the quickest and most appropriate route off the street, and devising a plan to achieve this, while holding a caseload of your own.
- Conducting regular outreach shifts, with volunteers and partner agencies.
- Representing the team at multi-agency meetings such as Task & Targeting meetings.
- Identifying gaps in provision and researching new pathways off of the streets for a variety of rough sleepers, including those with no recourse to public funds.
- Accurately recording all work done.

Hopkinson House provides 24 hour accommodation based support for up to 63 individuals with complex histories, multiple traumas, complex personality disorder(s) and a varied, disrupted housing history with experience of homelessness as well as individuals who have been diagnosed with severe and enduring mental health conditions who are under the Care Programme Approach (CPA).
Hopkinson House implements and promotes the Psychologically Informed Environment (PIE) model. This is an approach which was developed to support people out of homelessness, in particular those who have experienced complex trauma or are diagnosed with a personality disorder. However, its application is far broader than only homelessness. It considers the psychological needs of staff: developing skills and knowledge, increasing motivation, job satisfaction and resilience.
When working with individuals with complex needs, who may display challenging, chaotic behaviours, regularly testing boundaries and have experienced multiple traumas as a result of abuse, neglect, years of street homelessness, or long stays in mental health hospitals, it is integral that the postholder is motivated to support individuals to progress. They should continually hold aspirations for individuals despite multiple setbacks, and display emotional resilience, able to process challenging incidents and remain motivated to deliver excellent support.
Specialist Support Workers within Look Ahead are required to have either an extra dimension of experience or a higher qualification level as determined relevant for the particular post. You will provide information, social inclusion and vocational opportunities to support people to recover and stay well; exercise choice and control in their care and lives; and participate on an equal footing in the community. The Role
In-Reach is a pilot project delivered in collaboration with Surrey and Borders Partnership NHS Foundation Trust. The In-Reach Worker will work intensively with a caseload of individuals at the point of discharge from mental health wards and continue to support them beyond discharge for up to 3 weeks. The In-Reach Worker will work with individuals, families, carers and health professionals to develop wrap-around support plans to enable better integration into community services and reduce re-admission rates. The role will involve working as part of a small team and linking in with ward staff, home treatment teams, community mental health recovery teams and care coordinators.
The In-Reach Worker will:
- Haveappropriate previous community experience of mental health, wellbeing and behaviour change work with adults
- Be highly independent
- Have a high degree of initiative
- Be a self-starter
- Be confident working in a developing and fast-paced service

Carers Federation Ltd is a national charity and Company Limited by Guarantee established in 1992 to empower Carers, the people for whom they care and the wider community. The charity is managed by a Board of Trustees elected by our membership.
Our Action for Young Carers service helps young people aged 5-18. As part of a small team of Support Workers you will work closely with Young Carers and their families in Nottingham City. The attributes of AYC Support Worker would be.
- Deal with emotional, practical, and physical pressures
- Working with families providing assistance and one to one support
- Offer appropriate interventions and identify needs of individual and or family with internal and external support organisation statutory.
- Confident, conscientious, enthusiastic
- Previous experience in working with children
- You will be expected to deliver group activities during school holidays and at least one evening a week during term time.
You will work within Nottingham City Safeguarding Board policies and procedures alongside those of Carers Federation.
It is essential that you are a driver and car user as the role entails travel throughout Nottingham City and you will need to transport Young Carers to and from various activities.

To support the development of the Thames Ward resident-led health outreach project. The purpose of the project is to connect both local people and the local groups who are interested in improving the health of their community by working through the Thames Ward Locality Board to jointly create health programmes with LBBD Public Health, social care and lifestyle teams; NHS clinical commissioning groups (CCGs), primary care networks (PCNs) and local provider trusts (including BHRUT and NELFT); and the community and voluntary sector (including local groups and regional / national organisations).
The postholder will support and enable the mobilisation, development and implementation of the project. They will play a catalysing role, helping to deepen and embed the emerging model of care, working alongside residents, community groups, commissioners and health professionals.
