What you’ll be doing? 

You will be part of the MYcommunity Linkworkers team. Your main role will be to:

• Provide personalised brief intervention and social prescribing support to individuals, their families and carers.

• Meet clients on a one-to-one basis, making home visits where appropriate and allowing clients the time and space to discuss ‘what matters to me.

• Provide person-centred support to enable clients to identify their health and wellbeing needs and goals. Co-produce support plans with clients to discuss what’s important, where they would like to be in the future and how we can support them to achieve this. 

• Manage and prioritise a caseload to ensure clients' needs, priorities and urgent support are met.

• MYcommunity Lambeth works in partnership with a large number of statutory and community services and organisations across the borough. You will link clients with appropriate services to meet their needs. It is vital to have a strong understanding of when it is necessary to refer someone back to health care professionals/ agencies when the client’s needs are beyond the scope of the MYcommunity team.

We have an exciting new opportunity for a Social Prescribing Link Worker to join our team in Merton.

The Merton Social Prescribing service is one of the most established and mature services in the UK and is celebrating its 7th anniversary this year. We are seeking an individual to join our successful service in Southwest London. This role will help to strengthen the links between Primary Care, voluntary and public services to improve patients’ health and wellbeing. 

You will support patients within Primary Care in Merton, and you will support each patient to access appropriate services provided by voluntary and community organisations and other agencies. We expand the options available in Primary Care by creating a ‘managed’ way for referrals of patients with social, emotional and/or practical needs to a variety of local non-clinical services in the community. These include leisure, social activities, education, welfare, housing and employment advice and support.

About the Role: This is an interesting and varied role. As the post holder you'll be not only be setting up, administering and delivering schemes and activities to benefit Barnet’s looked after young people, you'll also be creating marketing materials and supporting fundraising events.  Promoting the work of our charity is key too through updating our website, writing our quarterly newsletter and social media channels, as well as supporting the running and administration of the charity.

Rose Voucher Project Officer, Southwark Rose Vouchers for Fruit and Veg Project

The Rose Vouchers for Fruit & Veg Project in Southwark has been successfully supporting families with young children to ensure fruit and vegetables are a part of their everyday diet since 2018. As we enter the seventh year of the project, we are now looking for a new member of staff to join the 1st Place Rose Voucher Project team and support our work in delivering the project, while measuring its wider impact on families. This is a great opportunity for someone with experience of working in a community setting and an understanding of impact and evaluation to take on a key role in the project, involving ongoing interaction with families and professionals.

Job Summary

As Rose Voucher Project Officer, you will be working alongside the Southwark Rose Voucher Project Lead to forward the development of the Rose Voucher Project in Southwark. Your key areas of responsibility will be registering new families onto the Rose Voucher Project, overseeing the two Rose Voucher distribution sessions which take place at both our centres, supported by our dedicated Rose Voucher volunteers, and coordinating and supporting the ongoing monitoring and evaluation of the Project. You will also provide support to our network of other Rose Voucher distribution centres in Southwark to ensure they are working in a coordinated way and according to agreed processes.

You will be a motivated individual, with excellent written and communication skills, and proven experience of working in a community setting where service users are valued, listened to and respected. Overall Purpose

Voids - Each year, Look Ahead collects c£21m of rent and service charge from the c3,000 units of property that we own and manage. This amount represents roughly a third of Look Ahead's annual income and therefore it is essential for us to maximise occupancy rates within our properties.

This is a varied opportunity that will see you managing the completion of all void works and ensuring that all properties are let within our Corporate deadlines. Look Ahead manages approx. 1500 units on other landlord's behalf, of which there are approx. 35. You will therefore have strong organisational skills and have the ability to build rapport with our landlords. We also operate across London and the Home counties, and the successful candidate will need to familiarise themselves with our stock quickly and establish effective working relationships with the teams on the ground.

I am working with a regulator, and we are looking for a Project Coordinator to assist the wider team with their strategic relationships across the public sector (Gov, NHS, Trade Bodies and Charities) to improve their relationships overtime. The colleagues you will be working with vary from operations, comms, policy and external affairs.

Your new role
The successful candidate will:

• Coordinate and provide proactive support for team meetings, in-person events and quarterly reviews.
• Drafting and distributing agendas, papers, other relevant documents as well as maintaining an action log.
• Updating project documents, which include risk logs, monthly reports, a team calendar, and intranet pages.
• Provide cross team coordination, which includes travel options and making group bookings.
• Coordinating the team's business plans, ensuring they are on time and regularly updated.

What you'll need to succeed
This position is ideal for someone who:

• Experience in the public sector / charity body as a Project/Programme Coordinator/Officer
• Proactive, well-organised with good time management skills
• Able to work within a team and demonstrate excellent customer service skills
• Good communication skills

Salary: £32,076 starting salary (salary range will increase to a max £34,834 via the length of service) Plus £4,087.65 Inner London Weighting if based in London per annum pro rata.
