This exciting pilot is a new approach to preventing rough sleeping, through the provision of accommodation with onsite specialist move-on support to those who are at immediate risk of sleeping rough. The service will provide a route and support for single homeless clients who are presenting to the Local Authority Housing Options or day centres and at imminent risk of rough sleeping, but not yet rough sleeping.

Where their homelessness cannot be prevented by the council and they are at imminent risk of sleeping rough, a referral can be made into the Rough Sleeping Prevention service.

The service will be based across three London locations in Islington, Lambeth and Kensington and Chelsea and staff will be based expected to work across all three sites

• Work on 7 day rolling rota inclusing early, mid and late shifts 
•  (K&C): medium – high support accommodation.
•  (Lambeth): medium support accommodation.
•  (Islington): Assessment hub space for up to 15 clients. Site will operate 24/7.

Each site will have staff delivering casework and support, providing each person with a comprehensive assessment of their needs and circumstances, and intensive specialist support to secure move on.

In this role you will jointly manage the safe day to day running of the project with other members of the team and to provide basic support and advice to clients.

The service forms part of a pair of services supporting up to 12 individuals at any one time across the sites, providing a welcoming and supportive environment for young people across london on a spot purchase basis.

Specialist Support workers will manage a caseload of customers to help them to identify, plan and achieve their goals, develop relationships meaningful to them and achieve greater independence by providing a flexible and individually tailored support package.

Consists of working 5 days a week across all days, 8 hours shifts starting at either 8am or 2pm.

As a Support Worker you will be expected to enable and support vulnerable individuals with complex needs to address this while maintaining their tenancy with the long term goal of living in an independent setting by providing flexible and individually tailored support package. Working with excluded and hard to reach customers can be challenging, however, with intensive support this challenge can be broken. You will need to be able to engage our customers in a non-judgmental way. You will also be responsible for dealing with emergencies that may arise during your shift whilst remaining calm and using initiative then writing up incident reports amongst other administrative tasks.

As a support worker, you will be working as part of a team as well as with external agencies such as London Probation Service, local substance misuse services, Mental health teams, and police amongst others. Sometimes customers may not be linked in with agencies when they entre the service, as a support worker it is part of your role to ensure they are receiving the correct support from agencies required. Ideally, you will have experience of working with vulnerable people or an understanding of the needs of homelessness. You will be required to support customers with managing their arrears; ensuring housing benefit is applied for as well as monitoring payments and acting appropriately if payments are suspended in order to ensure customer does not fall into any arrears.

You will be responsible for supporting the working shift pattern of the service which is a five week rolling rota which consists of working some weekends as well as long days.

There will be a rota for shifts, with working patterns of 7:30-15:30 and 12:30-20:30. There will be some weekend work.

Who are we looking for?

Firstly, you will need either a degree-level education or relevant work experience, such as in the health and social care or employment sectors. Alongside having strong and persuasive communication skills, excellent time management and exceptional organisational skills. Also, the ability to initiate and develop relationships with a variety of individuals and be able to engage employers using a variety of methods such as via the telephone or face-to-face in the community.

About the role:

As a Resettlement Worker and Domestic Abuse Specialist, you will work in person and remotely with clients who have support needs, in particular, those who are survivors of domestic abuse. In conjunction with other members of the Partnership, you will support our clients to engage with services and find accommodation solutions. You will work in a person-centered way to get the best prevention, relief and support outcomes for the client.

Main duties:

• To work with clients with support needs, to engage with their prevention, relief or advice goals by; Agreeing with them a support plan to deliver the actions identified in the Personalised Housing Plan. 
• Undertaking specific short term interventions to progress a client’s prevention or relief goals, including providing advice and support to enable individuals to improve their health and wellbeing. 
• Providing advice and signposting to support clients to access relevant housing and wider support services. 
• Working in partnership with RMG Caseworkers to deliver holistic housing and support service. 
• Providing support to clients to access private rented and supported accommodation options. 
• Referring to specialist internal and external support services as appropriate, including health, addiction and employment services.

General responsibilities:

• With your line manager, to continuously develop the role to ensure that all tasks are being undertaken in an effective and appropriate manner which meets the strategic aims and objectives of The Passage and the HSS Partnership. 
• To participate in internal/external meetings as required, and attend training events, conferences and other functions as necessary. 
• To participate in regular supervision, reflective practice and annual appraisal, and help in identifying your own job-related development and training needs. 
• To ensure that all The Passage and Partnership policies and procedures are being adhered to, particularly those relating to Health and Safety, Code of Practice Data Protection and Confidentiality.

Desired experience:

Desired knowledge:

• An awareness of the issues that face street homeless and vulnerable people. 
• A basic knowledge of the main housing legislation and housing options, relating to homeless and vulnerably housed people. 
• Knowledge of the skills and attitudes needed to support clients within a psychologically informed framework. 
• A working knowledge of the main welfare benefits in the UK. (Desirable)

Are you interested in supporting people at immediate risk of rough sleeping and making a real difference to the lives of people who are homeless?

This exciting pilot is a new approach to preventing rough sleeping, through the provision of accommodation with onsite specialist move-on support to those who are at immediate risk of sleeping rough. The service will provide a route and support for single homeless clients who are presenting to the Local Authority Housing Options or day centres and at imminent risk of rough sleeping, but not yet rough sleeping.

Where their homelessness cannot be prevented by the council and they are at imminent risk of sleeping rough, a referral can be made into the Rough Sleeping Prevention service.

The service will be based across three London locations in Islington, Lambeth and Kensington and Chelsea and staff will be expected to work across all 3 services . 

• 7 day rolling rota including early, mid and late shifts

•  (K&C): medium – high support accommodation.
•  (Lambeth): medium support accommodation.
•  (Islington): Assessment hub space for up to 15 clients. Site will operate 24/7.

Each site will have staff delivering casework and support, providing each person with a comprehensive assessment of their needs and circumstances, and intensive specialist support to secure move on.

The service will work in partnership with the local authority to secure onward move-on options e.g. access the council PRS scheme, advocating for access to supported housing.

As an Assessment and Reconnection Worker, you will: 

• Be part of a busy team environment working closely with those new to, or at risk of rough sleeping. 

• Provide a personalised case management service to clients, turning assessment recommendations into practical action plans which fully involves the client.

• Build and maintain effective relationships to support clients throughout their recovery. 

About you

These roles will suit people with experience of working at pace in a public facing environment with the ability to adapt to new situations; as well as:

• An understanding of the issues faced by homeless or vulnerable people and the difficulties they experience in accessing services.

• Good communication skills and the ability to work with others whilst managing your own time.

• The ability to manage complex behaviours and to deal with people who may be in a difficult personal or emotional situation. 

• Above all we are looking for inspirational, committed individuals who have a genuine desire to support people to rebuild their lives in these key roles; many of our managers began their careers as an Assessment and Reconnection Worker.
