Age UK Kensington & Chelsea offers dementia services which make a real difference to residents affected by the condition living in the Borough. Those services include advice, practical help, befriending, social activities and emotional support. The Dementia Support Worker is responsible for managing a caseload of individual clients, providing practical and emotional support to the most isolated and vulnerable in our community. The support takes the form of weekly or bi-weekly visits to the client’s home, as well as contact by phone and email and liaising with social care and health professionals as directed by the client’s wishes or needs.
This role requires the worker to work independently and manage their own caseload. With other members of the Dementia Services team, the Dementia Support Worker will also help with transport, group activities, trips, events, training and local forums.

Dementia Wellbeing Worker
Age UK Camden Dementia Wellbeing Service is looking to recruit a Dementia Wellbeing Worker to join their team working 35 hours per week on a permanent basis.
The Dementia Wellbeing Service is part of our specialist dementia services and works closely with the Dementia Befriending Service.
The service supports Camden residents over the age of 55 in the early or moderate stages of dementia, to live independently and safely at home for as long as possible, stay socially engaged and carry on doing what they love.

Carolyn House Service provides housing and support to individuals experiencing mental health issues for up to two years. We promote social inclusion and develop life skills for independent living. Livingstone House is a 24 hour staffed 92 bed mix gender hostel, which provides short term supported housing for single homeless vulnerable people with Low to Medium support needs. The Service provides on average 5 hours of weekly support to customers who reside in the hostel with the aim to provide them with the tools to move on within the community and live independently.

The St Mungo’s Pan London Rough Sleeping Teams (No Second Night Out and Somewhere Safe to Stay) provide a rapid response service, intervening to ensure no one needs to sleep rough. We are currently looking for people to join our teams as Night Assessment Workers in our No Second Night Out Assessment Hubs. In these roles you will:
