Sounds great, what will I be doing?
The main purpose of this role is to provide high-quality support and interventions to clients with mental health needs who require early intervention and prevention. The postholder will support clients throughout their time in the service, empowering them with the skills to cope independently, while working in line with organisational values, the ethos of recovery, and principles of co-production. Responsibilities include managing a caseload of clients, delivering group activities and programmes, and clearly communicating the aims, objectives, pathways, and service model to enable clients to fully participate in and co-produce their support. The role also involves helping service users engage effectively with community professionals, developing peer support networks and community-based approaches, monitoring the health, wellbeing, and safety of service users, and contributing to the core work of the service. Accurate and timely administration, referrals, assessments, and service reviews are required, alongside participation in supervision, appraisals, training, and team development activities. Flexibility, initiative, and a collaborative approach are essential to ensure effective service delivery and support for both clients and colleagues.

The ideal candidate will have lived experience of mental health care pathways, including emergency department attendance, acute admissions, and community mental health services, alongside a solid understanding of the issues affecting individuals with a range of mental health conditions. They will be able to communicate effectively and provide support with dignity and respect, forming and sustaining trusting relationships with service users, carers, peer support workers, and professionals. Strong listening and verbal communication skills are essential to engage appropriately with service users in crisis, colleagues, and external agencies, both in person and over the phone. Knowledge of recovery tools, local mental health and wellbeing support, and the ability to assess needs, risks, and aspirations to provide appropriate signposting are required. The candidate will demonstrate strong prioritisation, time management, and the ability to work dynamically under pressure. Proficiency in IT, including MS Word, Outlook, and internet functions, as well as good literacy and numeracy skills to produce clear written correspondence, is expected. They will have a sound understanding of safeguarding issues and the ability to address them appropriately, work effectively both independently and as part of a team, and support the induction of new peer staff and volunteers. Strong communication, partnership building, and interpersonal skills are essential.

About the role:
No one should leave hospital with nowhere safe to go.
As Homeless Pathway Coordinator, you’ll be a consistent and trusted presence for patients experiencing homelessness, helping them navigate what can feel like an overwhelming mix of health, housing and support systems. You’ll spend time with patients on wards and in A&E, listening to what matters to them, understanding the barriers they face and making sure their voice remains at the heart of every decision.
Day to day, you’ll manage a caseload, complete assessments, develop clear discharge and support plans and make referrals to housing, healthcare, social care, mental health and substance use services. You’ll follow up actions, keep plans moving and advocate firmly when someone is at risk of being discharged without the right accommodation or support in place.
You’ll work closely with the Homeless Pathway Lead, hospital teams, Newham Council and SHP’s wider Health Team, bringing professionals together and helping everyone stay focused on the person behind the case. You’ll also build links with community services, identify gaps in existing pathways and share learning that helps the hospital respond more effectively to people experiencing homelessness.
This is a role where strong relationships can change the course of someone’s recovery. You’ll join an experienced and supportive team, with opportunities to deepen your knowledge of inclusion health, homelessness and psychologically informed practice through training, reflective practice and shared learning.

PURPOSE OF POSITION
The Quality Assurance Manager is a blended leadership role combining strategic quality assurance, performance oversight, service management and practice development.
The role is split broadly between focused on quality assurance, data-led improvement and performance oversight, and two days focused on service management, practice leadership and frontline improvement activity.

Work with managers to embed learning, monitor progress and evidence improvement against internal and external standards.
Performance oversight and reporting: Monitor performance against contractual, regulatory and organisational standards, including service outcomes, compliance requirements, safeguarding expectations and commissioner reporting. Identify risks, gaps, trends and good practice, escalating concerns and supporting corrective action where needed. Produce clear, evidence-based reports and briefings for senior leaders, governance meetings, commissioners and external stakeholders.
Data, insight and systems: Use performance, safeguarding, complaints, incidents, resident feedback, outcomes and quality data to identify service strengths, pressures and improvement priorities. Maintain and improve reporting systems, including InForm, so information is accurate, consistent and useful. Analyse patterns, support decision-making and make practical recommendations that improve quality, accountability and resident outcomes.
Information retrieval and STAIRs requests: Act as QA Manager lead for information retrieval relating to STAIRs requests, including triaging scope, gathering internal and contractor records, coordinating redaction and drafting responses within regulatory timeframes.
Service and support management: Provide hands-on support management across frontline services, including Housing First improvement activity, Hope Street and St Mungo’s contracts, team rota management and the Support Manager on-call rota. Support managers and teams to manage risk, strengthen case accountability and recording, and deliver consistent, high-quality support to residents with complex needs.
Practice development: Embed person-centred, trauma-informed and psychologically informed practice across support services. Develop practical tools, guidance, quality forums, workshops and reflective learning that help staff apply policy, improve support planning, strengthen safeguarding and promote consistent resident-centred practice.
Resident voice and accountability: Strengthen resident engagement, feedback and involvement so resident experience informs service design, quality assurance, learning and improvement.
People management and collaboration: Line manage agreed roles, including the Impact Specialist and Resident Engagement Specialist, supporting clear objectives, accountability, development and collaborative working across services.
General duties: Attend key directorate, management, planning, quality and performance meetings; work flexibly across organisational priorities; and undertake other reasonable duties within the scope of the role.

Safeguarding and risk: Sound knowledge of adult safeguarding, risk assessment, MARAC processes and safe information sharing with vulnerable adults.

The role will ensure a joined-up support pathway for residents with low, medium, high and complex support needs, promoting safety, stabilisation, independence, move-on and long-term tenancy sustainment.
The post-holder will lead staff teams to deliver high-quality, trauma-informed, strengths-based and person-centred support for vulnerable single homeless adults, including residents with multiple disadvantage, high levels of risk and complex support needs.
The role will set, monitor and embed clear resettlement and support standards, ensuring residents receive consistent assessment, support planning, risk management, move-on preparation and tenancy sustainment support.
The role will be accountable for safeguarding, housing management, performance reporting, quality assurance, partnership working, commissioner confidence and continuous service improvement.

RESPONSIBILITIES & ACCOUNTABILITIES
Key Responsibilities
• Lead the delivery supported accommodation contracts in line with commissioner expectations, contract requirements, organisational policies and agreed performance standards.
• Provide clear management oversight of service quality, safety, safeguarding, risk, incidents, complex cases and escalation.
• Ensure residents receive effective assessments, risk management, support planning, move-on planning and outcome-focused reviews.
• Set and maintain clear resettlement standards across the service, ensuring residents are supported to develop independent living skills, secure appropriate move-on options and sustain accommodation after leaving the service.
• Ensure support standards are consistently applied, including timely assessments, person-centred support plans, regular key work, outcome-focused reviews, accurate case recording and clear escalation of risk or non-engagement.
• Promote trauma-informed, strengths-based and psychologically informed practice across the service.
• Work closely with the Intensive Housing Management team to oversee housing-related support, including referrals, placements, rent accounts, Housing Benefit claims, arrears prevention, voids, licence or tenancy obligations, move-on and tenancy sustainment.
• Ensure the intensive supported accommodation service provides safe, responsive and proportionate support for residents with complex needs and high levels of risk.
• Monitor performance against contract indicators, including outcomes, safeguarding, engagement, rent accounts, move-on, tenancy sustainment, voids and resident feedback.
• Prepare accurate reports, evidence and performance commentary for commissioner monitoring, audits, inspections and service reviews.
• Lead, develop and support staff to work as one team while maintaining service-specific boundaries, contractual standards, professional accountability and Your Place values.
• Maintain effective staffing arrangements, including cover for key working, weekend, out-of-hours and intensive provision requirements.
• Build strong partnerships with commissioners, Adult Social Care, health services, substance misuse services, criminal justice partners, housing providers and community agencies.

PERSON SPECIFICATION
The successful candidate will bring the experience, knowledge, leadership skills and values needed to manage high-quality supported accommodation services and contribute effectively to the wider Your Place team.
Essential Criteria
• Experience managing or supervising staff in supported housing, homelessness, social care, community support or a similar setting.
• Ability to lead change, improve practice and support staff through service development.
• Experience supporting vulnerable homeless adults, rough sleepers or people facing multiple disadvantage.
• Strong understanding of risk, safeguarding, trauma, mental health, substance misuse, tenancy sustainment and move-on barriers.
• Experience leading services that manage challenging behaviour, anti-social behaviour, safeguarding concerns, tenancy breaches, arrears or complex risk.
• Experience of contract management, performance monitoring, quality assurance, reporting and service improvement.
• Good understanding of housing management, including arrears, voids, tenancy or licence breaches, maintenance, health and safety, and property-related risk.
• Ability to analyse performance information, identify risks and trends, produce clear reports and use data to drive improvement.

PURPOSE OF POSITION
This role is responsible for delivering a high-quality, trauma-informed and gender-informed support service for women living in the Complex Needs service. The post-holder will hold a caseload of women residents who may be experiencing multiple disadvantage, including homelessness, rough sleeping, domestic abuse, mental ill health, substance use, exploitation, offending histories, social isolation and complex trauma.
The Women’s Support Worker will provide persistent, compassionate and practical support to women who may find it difficult to trust services, sustain engagement or accept support consistently. The role requires someone who can build safe, respectful relationships at the woman’s pace, while maintaining clear professional boundaries and supporting women to stabilise, feel safer, reduce risk and work towards their own goals.
The post-holder will work as part of the wider frontline support team, leading on support planning, advocacy and multi-disciplinary working for women residents. They will also support colleagues’ day to day to apply gender-informed practice, strengthen engagement with women and ensure support is coordinated, responsive and centred on women’s safety, choice and wellbeing.
This role forms part of a seven-day rota and will include some evening and weekend working.
