We are looking for a dedicated and passionate Young Persons Housing Advisors to join our team based in Hatfield! You will be responsible for providing advice, guidance and support to vulnerable people aged 18-24 years.

You will support vulnerable young people to find or maintain suitable accommodation. This involves setting up and or / sustaining their tenancies thus helping to prevent repeat homelessness. What will you be doing?
- Manage a caseload of young people aged 18-24yrs.
- Ensure that all young people work towards an agreed support plan which is regularly reviewed and adapted to ensure that they meet their individual needs and goals.
- Build a professional and trusting relationship with the young people to enable to them to engage with the support and improve their situation and prospects.
- Engage effectively in multi-agency working, developing collaborative relationships and partnerships with relevant staff in a range of agencies to broker and co-ordinate support for young people.
- Maintain an up-to-date knowledge of income related subjects such as benefits, money advice & grants.
- Be hyh’s benefits & money advice specialist, sharing knowledge and providing advice to the teams.
- Take the lead in developing and delivering benefits and/or money advice training.
- Work in partnership with the Catalyst Housing Floating Support staff to provide a consistent service countywide.
- Be ambassador for the charity both internally & externally to further hyh aims whilst protecting and enhancing its reputation.

Fantastic opportunity to work in a small friendly team. As Project Lead for APAP's Night Shelter provision you will be responsible for a caseload of clients and the wider management of the Night Shelter project. You will work with clients from assessment, through to placement in our provision and then support them to move beyond homelessness.
We work in partnership to provide single room, 24 hour staffed accommodation to clients in our provision. You will take a lead on maintaining a relationship with the accommodation provider through regular liaison and communication. Working predominantly with clients that have No Recourse to Public Funds, our Night Shelter Provision provides respite from the streets and an opportunity to work across agencies to address issues including immigration, mental and physical health and substance misuse.

Our Welfare Officers provide direct and impactful welfare support to RAF personnel and their immediate family. As a trusted and confidential source of support, you will assist our service users with challenges they are facing, from mental ill-health, difficult relationships, domestic abuse, financial and cost-of-living concerns, resettlement, housing, childcare, equality issues, bereavement, military discharge or deployment and many more - no day will be the same, as such this is a great opportunity to develop your knowledge and skills within the Welfare sector.

Job Title: Northern Ireland Support and Development Worker
Salary: £27,776 pro rata for part time (FTE 35 hours)

Core role

To provide information to individuals, families, carers affected by Spina Bifida and/or Hydrocephalus.

To provide advice, support and guidance to enable people living with Spina Bifida and/or Hydrocephalus to access the correct benefits to become financially stable, the right services to improve their health and well-being, and to better co-ordinate their support and independence through a variety of methods.

To refer individual members and/or their families/carers to the most appropriate source of expertise and support in Shine’s Health team and other designated leads within the organisation, or signpost to external agencies as appropriate.

About the role
We are looking to recruit a permanent multi-skilled advocate to join our busy Berks multi- disciplinary team, working up to 37 hours per week, minimum of 28 hours, starting salary £23,611 per annum, (pro rata).
In this exciting role you will be acting as an independent advocate supporting clients to ensure they get their voices heard. This may include providing advocacy across a number of different strands, such as the Mental Capacity Act, Care Act and Mental Health Act.
What we need from you
We are looking for someone who has the creativity and enthusiasm to support people in difficult circumstances, ideally has experience in a health or social care and may have their own life experiences relevant to the role. If you haven’t got advocacy qualifications it’s not a problem, we will provide the required training.
As this role is home based with regular travel, mainly but not exclusively in the Berkshire area, you must have access to both a stable home broadband service and easy access to transport as well as flexibility with working hours.
There may be times when work is required outside normal working hours so flexibility is required.
We also need you to live within Berkshire or surrounding counties.

We are looking for an Independent Advocate to join our team working on our Advocacy contract on a part time basis in Broadmoor. Our ideal candidate will have worked in advocacy, information and advice or similar role before, ideally in a health, social care or voluntary setting and will have experience of working with vulnerable adults.
The candidate will have strong experience working with adults at risk who are affected by significant health and social care issues who find it difficult to speak up for themselves; and experience providing persuasive verbal and written representations.
In this role the ideal candidate will be supporting a wide range of adults with profound and multiple needs. The successful candidate will need a good working knowledge of public and voluntary organisations to enable them to work in partnership with clients and professionals, gaining an understanding of their issues by listening, exploring options and taking action as directed by them.
You will need to have a flexible attitude and must be comfortable handling highly sensitive information whilst able to work well independently and confidently on behalf of others. Strong communication skills and time management skills will be required and you will need to be computer literate. Dementia Direct is an advisory service for people living with Dementia and their Carers.
You will provide practical support and advice pre and post diagnosis. You will connect those you support with resources, activities, groups and their community. The support you provide will sustain, as far as is possible, peoples’ independence, lifestyle and relationships.
We are looking for a friendly, caring and compassionate team player with a practical and solution focused approach to supporting people. Experience of working with people living with Dementia, their families and Carers will give you a distinct advantage.
The support you provide could be face to face in the individual’s home, over the telephone, virtually or in writing.
You will work with partners from other services, such as the Memory Service, GPs, health and social care, to build good working relationships and strong referral pathways.
You will have the opportunity to raise the profile of people living with Dementia their family and Carers by conducting outreach in the community and working with local organisations and community groups.
Main Duties
You will be a key source of support, ensuring the needs of people living with Dementia, their family and Carers are recognised and a plan put in place to meet them.
You will provide support that empowers Carers and people living with Dementia to access appropriate services including support in their local and wider community. Those you support will be enabled to overcome barriers to accessing support and will feel valued and included. Carers and people living with Dementia will be involved in the development and delivery of plans relating to their support.
You will take referrals from the wider community and colleagues from health and social care. You will work with Carers and people living with Dementia to enable them to maintain their independence, improve their wellbeing and help them to feel more in control of their lives. The support you provide will assist those living with Dementia to adjust positively to their condition and enable them to live well.
You will be part of a supportive and inclusive team that work together to ensure people living with Dementia and their Carers are valued, included, confident, listened to, independent and supported.
Key roles and responsibilities:

Maintain and effectively manage a caseload providing structured, practical and solution focussed support with agreed outcomes, and signpost appropriately based on level of need.

Host Memory Cafés, support groups and other activities throughout North Lincolnshire working with volunteer resource, and ensure venues are accessible.

The Role
As our Stop Smoking Specialist you will help deliver our new Stop Smoking Service and underpin its commitment to support people to stop smoking and reduce the risk of cancer. You will provide specialist advice and support to our service users around tobacco addiction and provide evidence-based approaches in line with national guidance and good practice to help them to quit smoking.
You will be part of a growing team at the charity and will be vital in the implementation of our strategy to ensure people in Yorkshire receive the best prevention, diagnosis and treatment.The role will deliver evidence based stop smoking interventions to patients referred into the service, providing support, advice and guidance either face to face, virtually or by telephone to maximise quitting outcomes. You will work as part of a team to respond to referrals and work closely with our Active Together Services, that are being rolled out across Yorkshire, and NHS Trusts where we fund in patient smoking cessation programmes.
Specifically, you will:

Autonomously deliver smoking cessation assessments, advice and support, minimizing harm from tobacco and health-related issues in line with best guidance and protocols.
-
Undertake client assessments, making appropriate pharmacotherapy decisions, provide ongoing client support and undertaking 4- and 12-week follow-ups.
-
Advise on stop smoking medications to patients including all forms of NRT, vapes (e-cigarettes) as well as non-nicotine treatments.
-
Help patients who smoke to quit, communicating in a sensitive, client centred manner.
-
Contribute to the monitoring and evaluation of the service, including the completion of the minimum data set aligned to NHS England mandatory requirements. This will include following up with patients following discharge to monitor stop smoking outcomes.

Manage caseload of patients and be responsible for all associated tasks, including follow-up appointments, and managing diary commitments.
-
Communicate effectively with colleagues, providing clear verbal, digital or written information and instructions when sharing information, delegating, or handing over responsibility for care to ensure that smoking cessation records are kept up to date and accurate.

Work with the Services Manager and Head of Services to ensure that public and patient insight and involvement is captured, which will shape the development, delivery and evaluation of our service to make sure they are patient centred.
About You
To be considered for this role, you will need:

To be educated to GCSE (or equivalent) grade C or above in English and Maths.

To have NVQ level 3 in Health care (or equivalent) is desirable but not essential.
-
To be National Centre for Smoking Cessation Training (NCSCT) Level 2 trained (essential).
-
Intermediate theoretical knowledge of stop smoking support, gained through Level 2 NCSCT.
-
To have substantial experience of providing smoking cessation support in a stop smoking service with good levels of quit rates.

To have experience of working in Health and Social Care setting or community.
-
To have experience of using Microsoft Office software and SystmOne.
-
To have proven IT skills, including data entry, accurate reporting of agreed care plans, using IT infrastructure to support effective handover of care.

To have good knowledge and understanding of evidence-based practice in stopping smoking, the consequences of smoking and the benefits of quitting.

To have excellent communication (written and verbal) and listening skills.

To have a good understanding of GDPR and confidentiality issues.
