A Support Worker will help to enable vulnerable people with complex support needs to manage their licences and move towards greater independence within the community by providing a flexible and individually tailored support package.

The shift pattern of the role consist of: 40 hours 1 x scattered site 2 x Pound Lane Shift pro-rota 7.30-3.30 and 1.30 -9.30 shift pattern with weekends included.

What you'll do:
* Undertake key-working responsibilities for a caseload assigned by the Team Leader / Manager
* Oversee the daily running of the service ensuring all health and safety responsibilities are met
* Raise safeguarding alerts and complete incident reports within specified timeframe
* Undertake initial and continuous assessment of needs and potential risks and agree levels of support and actions
* Create support/action plans for providing appropriate support based on the assessment and reflecting the services and resources available
* Support customers to undertake domestic tasks wherever possible, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of living standards
* Support customers to move on from the service through their identified move on pathway liaising with our Resettlement Officer and Brent Housing team.
* Develop and maintain links with all key agencies and service providers in the local community
* Empower customers to ensure they receive the service and benefits they are entitled to

Look Ahead's Hammersmith & Fulham Mental Health Service is a Mental Health supported accommodation which is a pathway service for people to work towards their independence. There are 3 buildings in the cluster, all of them are lone-working.

As a Support Worker, you will play a key role in supporting our residents to help them to enhance life skills and increase independence and manage their health. This also includes tenancy management and other life skills.

Support Workers in the service work across all 3 projects in Hammersmith and Fulham; working with customers with varying levels of support needs. Many of the customers have complex needs including dual diagnosis and Support Workers are expected to liaise with other agencies within the borough to best support customers. Support Workers are expected to support customers on their journey to independence in a person-centred, non-judgemental way, whilst adhering to the Look Ahead core values.

The shift schedule covers Monday to Sunday, working five days each week on a rolling rota that combines weekdays and weekends. Over a five-week period, there are two complete weekends off.

NVQ Level 2/3 or transferable skills in customer centred roles

Campbell Road Young Peoples service provides support and housing for 37 young people in Tower Hamlets that are either leaving care of affected by youth homelessness.

What you'll do:
- Building supportive, trusting relationships with customers
- Working proactively with other members of the team to handle the service caseload and administrative responsibilities
- Supporting key customers to set personalised goals in the form of a Support Plan
- Conducting regular key work sessions that are innovative and engaging in order to achieve Support Plan goals
- Ensuring ongoing assessment and management of risks associated with customers within an attitude of 'positive risk taking'
- Proactively manage risk and safety both in and outside of their physical living environment
- Providing support with daily living activities, including practical assistance where skills are not yet developed, to ensure that customers enjoy a high quality of accommodation

We are seeking a flexible, reliable, and enthusiastic Support Worker to deliver services for Young Carers aged 5–16. The role involves three key parts. The first is delivering engaging, age-appropriate activities that promote wellbeing, reduce isolation, and give Young Carers a break from their responsibilities. The second is providing one-to-one support to help Young Carers manage challenges, access services, and prioritise their mental and emotional wellbeing. The third is leading outreach with schools, colleges, community settings and professionals to identify and engage hidden Young Carers. 

About The Role: 

You will work across two key age groups, 5–11 and 12–16, delivering and supervising activities, supporting events and trips, and providing behavioural management where needed. Alongside group activities, you will offer one-to-one support to individual Young Carers, ensuring they feel heard, included, and supported as they navigate the challenges of their caring roles. You will also deliver awareness training sessions, run stalls, and build partnerships in the community to identify and engage Young Carers. 

This is a part-time post (3 days per week). Flexibility is required, with regular evening and occasional weekend work.

Key Requirements Include: 

• Experience working with children, young people, or vulnerable groups 

• Experience facilitating groups or activties 

• Ability to provide one-to-one support in a sensitive and empowering way 

• Strong communication skills with young people, families, and professionals

Well House is a crisis house supported accommodation service that supports individuals for between 14 and 28 days as an alternative to requiring hospital admission for their mental health needs. The service works closely in partnership with a multi-disciplinary team in order to meet individual needs, manage risk and support individuals to achieve positive outcomes.

The service operates 24 hours a day, 7 days a week to ensure members of the local community are able to access critical mental health support at all hours. The service receives referrals primarily from a local hospital emergency department and crisis lines, and may receive these referrals at anytime of day or night.

Bank Support Workers may be able to work shifts from all of Look Ahead's specialisms including Mental Health, Learning Disabilities, Homelessness and Young People. Bank Support Workers will provide support to customers to help them develop the life skills they require to meet the assessed needs of the customers of the service. They will work with customers to promote social inclusion and alongside other members of the team to promote independent living, encouraging customers to maximise their skills and choices. The Pelham Road Service is for people with learning disabilities, Autism and Complex needs, and we are looking for someone to support an existing staff team with an excellent value base to support the customer to build their life and develop their skills as an independent person.

Support Workers will provide support to customers to help them develop the life skills they require to meet the assessed needs of the customers of the service. At Pelham Road this includes providing physical, domestic, emotional and social care. We adopt a trauma informed and psychologically informed approach to supporting our residents to help them build essential life skills, identify their goals and support residents into independent accommodation. We have a dedicated team of Support Workers, Night Workers and additional Bank Workers who provide support, guidance, and signposting around areas such as housing, budgeting, living skills, jobs and relationship building.

We are looking for a Supported Housing Support Worker to join our Cook Road, Horsham team, who will hold a caseload of residents and meet with them weekly to build a support plan. Main areas of responsibilities are:

Housing

• Coach young people to manage their occupancy agreement and adhere to house rules, in preparation for independent living. 
• Promote a credit culture, encouraging young people to keep up to date with all payments for rent.
• Maintain up-to-date knowledge of housing and welfare benefits for young people and be well-informed on significant changes to housing law.
• Deal effectively with non-compliance issues, such as non-payment of rent or damage to room, using restorative practices and working collaboratively with the rest of the team.

Coaching and Engagement

• Coach young people so they can articulate their aspirations and ambitions and take the lead in acquiring the skills they need to live independent and fulfilling lives.
• Ensure young people are encouraged to take responsibility for their own personal development, to engage with the services on offer and build strong networks and connections within the local community. 
• Ensure consistent standards of safeguarding and Trauma Informed Practice when supporting young people, observing our safeguarding procedures, and keeping yourself and residents safe by respecting professional boundaries.
• Maintain client records on In-Form (client database) detailing the young person’s journey in relation to their strengths and needs, any risks, and any outcomes (to monitor service performance). 

General 

• Work as part of a team, on a rota shift pattern, ensuring young people at the service have non-judgemental, objective, and supportive staff during the day/evening, along with taking responsibility for personal safety during periods of lone working
• Contribute to a great working environment, with a calm, yet assertive manner, being able to handle potentially difficult situations.
• Participate in relevant continuing professional development and utilise Reflective Practice Supervision as part of leading psychologically informed practice.

• Knowledge of good safeguarding procedures in relation to young people and the ability to maintain effective professional boundaries.
• Demonstrated confidence and competence in recording notes/actions in service log, incident forms and health and safety check lists.

Skills and Abilities 

• Ability to communicate clearly both verbally and in writing for appropriate recording of a resident’s progression, and to evidence outcomes achieved.
• Ability to build and maintain strong relationships with all stakeholders, including signposting and advocating for clients as necessary. 
• Ability to work autonomously, and use own initiative, as well as being part of a team.
• Clear verbal and written communication skills, good IT, and keyboard skills.
• Ability to de-escalate volatile situations and manage challenging behaviour appropriately.

Are you an experienced manager with a passion for delivering high quality services to people with learning disabilities? This varied and dynamic role will lead on the set up, coordination and provision of excellent person-centred care and support to service users, enabling them to live independently in their home and be supported in all aspects of day-to-day living. As Project Manager, you will oversee the development and implementation of care plans designed to provide meaningful choice and a range of activities and support to meet individual needs. You will also contribute to the development of the team and be responsible for encouraging a culture based on respect and dignity for those in need of care and support, providing supervision and clear management to a team of support workers.

The ideal candidate will have experience of leading and coaching a team supporting people with learning disabilities or another vulnerable client group. Experience of supporting people with high and complex support needs would be an advantage.
