Specialist Support Workers within Look Ahead are required to have either an extra dimension of experience or a higher qualification level as determined relevant for the particular post. Specialist Support Workers provide information, social inclusion and vocational opportunities to support people to recover and stay well; exercise choice and control in their care and lives; and participate on an equal footing in the community. Support is also provided to develop a model of re-enablement towards self-advocacy, self-management and empowerment, incorporating the principles and practice of recovery, co-production and peer support.

What you'll do:

* Building supportive, trusting relationships with customers and creating a positive atmosphere
* Carrying out holistic assessments of new customers which incorporate relevant statutory referral information
* Conducting regular key work sessions that are innovative and engaging in order to achieve Support Plan goals
* Create support plans for providing appropriate services based on on-going assessment and reflecting the services and resources available to enable customer to progress in their recovery
* Ensuring ongoing assessment and management of risks associated with customers within an attitude of positive risk taking

* Developing productive relationships with partner organisations to improve service outcomes
* Demonstrate strong commitment to the functioning of the staff team and to support colleagues in providing a high level of service and safety to customers
* Involving customers in the design, development and delivery of the service
* Adhering to lone working protocols and maintaining high levels of awareness in relation to the health and safety of yourself and colleagues

Successful candidates will be given the opportunity to join a busy, dynamic and engaging team working in Newham. Support Workers enable people with particular needs to manage their tenancies and live independent lives of their choosing in the community and move towards greater independence by providing a flexible and individually tailored support package. Support Worker will give practical support to help people maintain their tenancy, stay well, exercise choice and control in their care and lives and participate in the community. The service includes four sites and form a mini pathway of higher, medium and move on support accommodation.

The shift pattern for this role consists of three shift timings at: 9.00 - 17.00, 12.00 - 20.00 and 14.00 - 22.00. This includes weekends and bank holidays.

Dementia Wellbeing Worker 

Age UK Camden Dementia Wellbeing Service is looking to recruit a Dementia Wellbeing Worker to join their team working 35 hours per week on a permanent basis. 

The Dementia Wellbeing Service is part of our specialist dementia services and works closely with the Dementia Befriending Service.

The service supports Camden residents over the age of 55 in the early or moderate stages of dementia, to live independently and safely at home for as long as possible, stay socially engaged and carry on doing what they love.  

The successful candidate will: 

• Offer a holistic and person-centered assessment for people living with mild cognitive impairment or Dementia to ensure meaningful interventions.

• Manage a case load of clients, providing practical and emotional support to the most isolated and vulnerable in our community.

• Provide information, advice and support to carers (family/friends), health/social care professionals and other voluntary organisations and to disseminate knowledge and best practice so that individuals living with cognitive impairment in relation to dementia or undiagnosed dementia have the best care possible.  
   

• Maintain accurate and appropriate records via Charity Log database and in accordance with UKGDPR.

Carolyn House Service provides housing and support to individuals experiencing mental health issues for up to two years. We promote social inclusion and develop life skills for independent living. We also work with several agencies and organisations in the local area to meet individual customers' needs. We support customers to build confidence in order to empower them to use community resources; we promote healthy lifestyles and wellbeing and offer customers choice and control over the activities they would like to participate in.
The service is staffed 24 hours 7 days a week. All customers referred to the Service are allocated a support worker who will provide guidance and support on a range of issues and help signpost customers to relevant services located in the Redbridge Borough.
The Post holder will be expected to work on a rota basis (Monday - Sunday) and two sleep-ins per week which attracts £30.00 extra pay.The shift patterns are 8.00 am - 4.00pm, 2.00 pm - 10.00 pm and sleep ins.

Hounslow Young People is Supported Accommodation Service for young people and young parents aged 16-25 in Hounslow. The Service encompasses a mix of units across six sites supporting 57 vulnerable young people and 19 teen parents. The aim of the service is to empower Young People to move towards independent living, supporting their successful transition from childhood; laying the foundations for healthy, ambitious, and safe adult lives with minimal ongoing support. Support is delivered in partnership with a number of stakeholders and partners in the community such as mental health services; substance misuses agencies, housing teams, police and social services. We are recruiting Waking Night Support Workers to join our dynamic service and team. In the role of Waking Night Support Worker; individuals will ensure continues support is provided to young people at the standards of day staff with particular focus on safety of vulnerable young people.

About the Role:

Waking Night Support Worker will work in line with person centred risk assessments; support plans and liaise with Support Workers to provide comprehensive handover and partnership working. They will be observant of changes in behaviour and presentations ensuring appropriate actions are taken to document and report changes in support needs of young people. Waking Night Support Worker will be able to carry out all necessary health and safety actions including light cleaning tasks and environment management. Furthermore they will actively seek out ways to work with the team and stakeholders to improve the experience of young people in the service. The role requires individuals to work on a rota pattern including weekend and bank holiday working, as the service is open 365 days a year. Hours worked: 21:30 - 07:30.

Hounslow Young People is Supported Accommodation Service for young people and young parents aged 16-25 in Hounslow. The Service encompasses a mix of units across six sites supporting 57 vulnerable young people and 19 teen parents. Waking Night Support Worker will be able to carry out all necessary health and safety actions including light cleaning tasks and environment management. Colindale Services are a mental health, care home and supported housing services, working in a recovery model. • Work on a 7 day weekly night rota pattern with an average of 3 waking night shifts in a row (for example 8pm–8.30am or 9pm–8am)
