Dementia UK is looking for a Stewardship Executive to join our Philanthropy and Partnerships team. This is an exciting new role to the charity as a result of our five-year strategy and ambitious plans to grow and diversify our income. This role will help to deliver a consistent and meaningful experience for supporters across Philanthropy and Partnerships.

This is a varied coordination and supporter stewardship role at the centre of the team. You will play a key role in coordinating stewardship activities such as thanking, supporter communications and cultivation and stewardship events, including our annual carol concert. You will support internal processes, ensure fundraisers are well-supported to build strong relationships and hold some of your own, and support the Prospect Research Manager with prospect tracking and pipeline processes.

We are looking for you to have experience of administrative or coordination work ideally in a fundraising, charity or customer-focused setting or have an interest in this setting. You will be highly organised, proactive and confident managing multiple priorities. You will have strong communication and coordination skills, with the ability to build relationships with colleagues and supporters.

Age UK Wandsworth is a local, independent charity that works to promote the wellbeing of all older people in the London Borough of Wandsworth. The Doncaster service is a partnership with Rotherham, Doncaster and South Humber NHS Foundation Trust to provide joined-up support for people affected by memory concerns and dementia. This includes both practical advice and access to clinical expertise through a shared pathway.

As part of a supportive multi-disciplinary team, you’ll play an important role in helping people with dementia and their carers achieve better outcomes and feel more supported in their day-to-day lives.

You’ll also get out into the community to raise awareness of the service, helping people understand what support is available and how to access it. Working alongside clinical colleagues, you’ll build strong relationships with primary care teams so we can reach those who may not yet be receiving the help they need.

This is a busy and rewarding service, so being flexible and able to manage different priorities is important. Above all, we’re looking for someone who is committed to making a difference, continuously improving the service, and providing high-quality, person-centred support that adapts to the changing needs of the people we support.

You will work closely with people living with dementia and those who care about them, providing ongoing support. You will listen carefully, understand what matters most to each person and offer guidance that helps them feel steadier and more confident as their situation changes.

Your days will be varied and meaningful. You might be visiting someone at home, offering calm reassurance over the phone, or working closely with other professionals to make sure the right support is in place. We operate a triage system for new referrals and you will play your part in making sure people are responded to quickly and thoughtfully. You will manage your own caseload, keeping clear and accurate records, while always seeing the person behind the paperwork.

You will be part of a caring and dynamic local team who share ideas, reflect together and support one another. There will be time to meet as a team and opportunities to keep growing your knowledge of dementia, with your manager and colleagues alongside you.

About you
You will have:

- An understanding of dementia, or a willingness to learn about the experiences of people living with dementia and those who care for them.
- Experience providing information, guidance, advice or emotional support - this could be from health, social care, community, voluntary, customer service or other people focused roles.
- You will be able to listen and communicate, with the ability to build trust and rapport with people from a wide range of backgrounds.
- The ability to manage your workload effectively, with appropriate support and tools, and to balance competing priorities.
- Confidence using IT systems to maintain accurate and confidential records.
- A compassionate, non-judgemental and person-centred approach, with respect for diversity and individual lived experiences.
- The ability to travel independently.
- The confidence and ability to communicate with a wide range of people, adapting your approach to ensure understanding. You will be required to speak publicly at events about dementia, delivering information and signposting people to further resources and networks.

We have an exciting opportunity to join our team as a Dementia Adviser, working within the Dementia Wellbeing Service in Bristol.

A partnership organisation bringing together the NHS and the Alzheimer’s Society, as a Dementia Adviser within the service you will work as part of a multi-disciplinary team to deliver dementia support, signposting and advice across the city.

This is a role that allows you to make a real difference in the lives of those affected by dementia.

About you

We are looking for someone who:

- is passionate about supporting people to maintain their independence, improve their sense of well-being, and help them to live well with dementia.
- has professional or personal experience of dementia and the challenges people affected by it may face.
- is comfortable working in the community – meeting people in their homes to deliver dementia support.
- is able to manage difficult conversations and adopt a non-judgemental approach.
- can network effectively and collaborate with other professionals to achieve positive outcomes for people.
- is capable of managing a caseload of service users effectively and independently, ensuring face to face reviews and telephone contacts happen within expected timescales.
- has strong written communication skills and can write case notes with clarity, accuracy and timeliness. Previous experience of writing case notes is not required.
- has the ability to travel across the local area independently as often as required

What you’ll focus on:

-      Managing an existing caseload of service users as well as new referrals to deliver ongoing post-diagnostic support.
-      Working collaboratively with NHS clinicians to ensure the right support is offered to people at the right time.
-      Writing and updating of ‘Wellbeing Plans’ that capture a person’s history, current support needs, and future wishes. 
-      Supporting our peer-support groups, delivering carers education courses and engaging in awareness raising events in the community.
-      Identifying service user support needs and referring onwards to appropriate organisations (e.g. social services, other charity organisations).

Role Overview
This is a unique opportunity to help shape and develop our dementia support service, working with an experienced and committed team. We are looking for someone who brings good experience of supporting people living with dementia and their carers, good service coordination, line management, service development and partnership working experience, alongside the warmth and confidence to build supportive relationships with people living with dementia, their carers and partners across the community.

The Dementia Service Co-ordinator is a newly created role within our Healthier Lifestyles Team responsible for coordinating and developing Age UK Croydon’s Dementia Service. A varied role with development, line management, and group facilitation/delivery responsibilities, it offers an exciting opportunity to join the team and help shape the service to support people living with dementia, their carers and the wider community. The Dementia Service sits within the Healthier Lifestyles Team and currently offers six monthly Memory Tree Cafés for people living with mild to moderate dementia and their carers, alongside co-located support groups for carers. Cafes are delivered at our Brigstock Road Community Hall and across the borough in local community venues.

The main areas of responsibility for this role are: coordinating, facilitating and expanding the service with lead responsibilities for our newest café and our carers groups. existing café and carers’ group; including prioritising café expansion supported by current funding.

The post holder will take lead responsibility for the delivery, monitoring and evaluation of our dementia-related services across the borough. You will line manage and support the dementia service staff and volunteer team to provide a welcoming, inclusive environment within our Memory Tree Cafes and carers’ support groups which promotes interaction, stimulation and wellbeing. You will also build and maintain positive working relationships across Age UK Croydon and with external partners, referrers and stakeholders, including health professionals and voluntary organisations, to strengthen, develop and promote the service.

Age UK Croydon is currently reviewing its Dementia Service, and in this role the post holder will work closely with the Healthier Lifestyles Service Manager to put the review’s recommendations into practice, ensuring quality assured delivery, data collection and monitoring and exploring opportunities for service development and partnership working.

- The ability to travel across the County of Hertfordshire independently when required. This post will mainly cover South West Hertfordshire including Watford, Potters Barr, Borehamwood and Rickmansworth and therefore you would need to live in an easy commutable distance from these areas.

About the opportunity

We have an exciting opportunity to join our team as a Dementia Adviser (12 months maternity cover), working within the Dementia Wellbeing Service in Bristol.

A partnership organisation bringing together the NHS and the Alzheimer’s Society, as a Dementia Adviser within the service you will work as part of a multi-disciplinary team to deliver dementia support, signposting and advice across the city.

This is a role that allows you to make a real difference in the lives of those affected by dementia.

About you

We are looking for someone who:

- is passionate about supporting people to maintain their independence, improve their sense of well-being, and help them to live well with dementia.
- has professional or personal experience of dementia and the challenges people affected by it may face.
- is comfortable working in the community – meeting people in their homes to deliver dementia support.
- is able to manage difficult conversations and adopt a non-judgemental approach.
- can network effectively and collaborate with other professionals to achieve positive outcomes for people.
- is capable of managing a caseload of service users effectively and independently, ensuring face to face reviews and telephone contacts happen within expected timescales.
- has strong written communication skills and can write case notes with clarity, accuracy and timeliness. Previous experience of writing case notes is not required.
- has the ability to travel across the local area independently as often as required

What you’ll focus on:

- Managing an existing caseload of service users as well as new referrals to deliver ongoing post-diagnostic support.
- Working collaboratively with NHS clinicians to ensure the right support is offered to people at the right time.
- Writing and updating of ‘Wellbeing Plans’ that capture a person’s history, current support needs, and future wishes.
- Supporting our peer-support groups, delivering carers education courses and engaging in awareness raising events in the community.
- Identifying service user support needs and referring onwards to appropriate organisations (e.g. social services, other charity organisations).

About the opportunity

At Alzheimer’s Society, we believe no one should face dementia alone. Every conversation, every visit and every small moment of reassurance can make a real difference. We are looking for a warm, thoughtful and organised Dementia Adviser to join our supportive team in The South Eastern Health & Social Care Trust working 35 hours per week.

You will work closely with people living with dementia and those who care about them, providing ongoing support. You will listen carefully, understand what matters most to each person and offer guidance that helps them feel steadier and more confident as their situation changes.

Your days will be varied and meaningful. You might be visiting someone at home, offering calm reassurance over the phone, or working closely with other professionals to make sure the right support is in place. We operate a triage system for new referrals and you will play your part in making sure people are responded to quickly and thoughtfully. You will manage your own caseload, keeping clear and accurate records, while always seeing the person behind the paperwork. You will also facilitate the care information support programme for carers.
You will be part of a caring and dynamic local team who share ideas, reflect together and support one another. There will be time to meet as a team and opportunities to keep growing your knowledge of dementia, with your manager and colleagues alongside you. 

About you

You will have:

- An understanding of dementia, or a willingness to learn about the experiences of people living with dementia and those who care for them.
- Experience providing information, guidance, advice or emotional support - this could be from health, social care, community, voluntary, customer service or other people focused roles.
- You will be able to listen and communicate, with the ability to build trust and rapport with people from a wide range of backgrounds.
- The ability to manage your workload effectively, with appropriate support and tools, and to balance competing priorities.
- Confidence using IT systems to maintain accurate and confidential records.
- A compassionate, non-judgemental and person-centred approach, with respect for diversity and individual lived experiences.
- The ability to travel independently.
- The confidence and ability to communicate with a wide range of people, adapting your approach to ensure understanding. You will be required to speak publicly at events about dementia, delivering information and signposting people to further resources and networks.
