About this role…

Assessment Centres are fast paced services where people are referred in, usually from the street, and rapidly referred on to appropriate longer term accommodation. The services are short stay, and staff provide wrap around support and liaison with several external partners, including physical health, mental health, substance use and housing. Staffed 24 hours to ensure this quick turnaround, they are a key service to ensure people are off the streets and in stable accommodation.

We are looking for a Night Support Worker to join both the brand new Snow Hill Court team and the City Inn Assessment Service team.

• Conduct rapid risk and accommodation assessments with clients moving in to the service

Thames Reach’s Outreach teams operate across London and helps those who are rough sleeping to find routes away from the street. The teams respond rapidly and work flexibly to reach people. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those offering immigration support.

You will:

• Complete outreach shifts, providing a rapid response to referrals for people who are rough sleeping in the borough 
• Take a lead role within the team, working closely with support workers and assistant support workers to identify the quickest and most appropriate route off the street for people found rough sleeping, and devise plans to achieve this 
• Provide direct support to people who are found rough sleeping to enable them to move away from the streets and into accommodation 
• Work alongside partner agencies to ensure that clients are accessing appropriate support to access and sustain accommodation
• Accurately record all work completed within given timeframes 

To succeed as the Lead Worker, you will have:

• A strong commitment to providing a high standard of service 
• Experience working with people who have rough slept or vulnerable adults and supporting them to access services 
• Excellent organisation and time management skills in order to remain strongly outcome focused, with the ability to coordinate a high number of cases and complete tasks promptly and efficiently

Age UK Kensington & Chelsea offers dementia services which make a real difference to residents affected by the condition living in the Borough. Those services include advice, practical help, befriending, social activities and emotional support. The Dementia Support Worker is responsible for managing a caseload of individual clients, providing practical and emotional support to the most isolated and vulnerable in our community. We are seeking a Part Time Dementia Support Worker (up to 21 hours per week)

This is an exciting opportunity to be part of and help shape a new DHSC funded project - Early Support Hubs - providing innovative ways to support Young People’s (11 – 25 years) Mental Health and Wellbeing across the borough of Hackney.

This Early Intervention approach will enable young people to access support in a timely way within their own community and we will work collaboratively with the Hackney Super Youth Hub enabling a one stop shop experience for local young people.

We are looking for passionate and motivated Therapeutic Youth Workers who are committed to supporting positive change for local young people to join our team.

Main Responsibilities:

· Undertake 1:1, group, detached and outreach work with young people

· Provide one-to-one key-working up to 12 sessions, combining youth work values and principles with a therapeutic, trauma-informed approach to help address a wide range of issues that may impact on young people’s mental health and wellbeing e.g. housing, employment, physical health, education, finances and relationships.

· Use a supportive, young person-centred approach. e.g. explaining what services are and what they do, ensuring information is accurate and up-to-date, filling out forms together, making personal introductions, accompanying young people to initial meetings/sessions, being with them during phone calls etc.  

· To work with the Hackney Super Youth Hub, to support young people and liaise with appropriate professionals in the network.

· To maintain confidential, up to date and accurate records of all sessions and plans.

· Be an advocate for young people, supporting colleagues to understand adolescence, and how to engage with young people, particularly with concerns around:

o mental health and wellbeing of children and young people.

o Supporting vulnerable groups who would not otherwise receive support (levelling-up).

o Improving children and young people’s engagement with EET.

· QCF Level 3 Diploma in Youth work or equivalent and have experience of youth work in a youth work setting

· Experience of working with young people, delivering therapeutic groups and/or 1 to 1 interventions in a youth work or similar setting.

· To have an active interest in working with those affected by mental health issues and recognise the impact this has on their life.

· Experience of building positive, impactful relationships with young people responding to their needs

· Excellent, up to date knowledge and understanding of safeguarding needs and risks young people may face.

Campbell Rd Young Peoples service provides support and housing for 55 young people in Tower Hamlets that are either leaving care of affected by youth homelessness. The Support Worker role will be to manage a caseload of approx 7-10 young people .You will be expected to deliver positive outcomes for all our young people and support them to reach independence, keep safe and maintain their tenancy's. Aside from support workers delivering support there is a strong element of housing management associated wit this role, for example managing rents, raising/monitoring repairs and health and safety.

Carolyn House Service provides housing and support to individuals experiencing mental health issues for up to two years. We promote social inclusion and develop life skills for independent living. We also work with several agencies and organisations in the local area to meet individual customers' needs. We support customers to build confidence in order to empower them to use community resources; we promote healthy lifestyles and wellbeing and offer customers choice and control over the activities they would like to participate in.
The service is staffed 24 hours 7 days a week. All customers referred to the Service are allocated a support worker who will provide guidance and support on a range of issues and help signpost customers to relevant services located in the Redbridge Borough.
The Post holder will be expected to work on a rota basis (Monday - Sunday) and two sleep-ins per week which attracts £30.00 extra pay.The shift patterns are 8.00 am - 4.00pm, 2.00 pm - 10.00 pm and sleep ins.

Hounslow Young People is Supported Accommodation Service for young people and young parents aged 16-25 in Hounslow. The Service encompasses a mix of units across six sites supporting 57 vulnerable young people and 19 teen parents. Waking Night Support Worker will work in line with person centred risk assessments; support plans and liaise with Support Workers to provide comprehensive handover and partnership working. Waking Night Support Worker will be able to carry out all necessary health and safety actions including light cleaning tasks and environment management. Dementia Wellbeing Worker 

Age UK Camden Dementia Wellbeing Service is looking to recruit a Dementia Wellbeing Worker to join their team working 35 hours per week on a permanent basis. 

The Dementia Wellbeing Service is part of our specialist dementia services and works closely with the Dementia Befriending Service.

The service supports Camden residents over the age of 55 in the early or moderate stages of dementia, to live independently and safely at home for as long as possible, stay socially engaged and carry on doing what they love.  

The successful candidate will: 

• Offer a holistic and person-centered assessment for people living with mild cognitive impairment or Dementia to ensure meaningful interventions.

• Manage a case load of clients, providing practical and emotional support to the most isolated and vulnerable in our community.

• Provide information, advice and support to carers (family/friends), health/social care professionals and other voluntary organisations and to disseminate knowledge and best practice so that individuals living with cognitive impairment in relation to dementia or undiagnosed dementia have the best care possible.  
   

• Maintain accurate and appropriate records via Charity Log database and in accordance with UKGDPR.
