Hours of Work: Part time, 22.5hrs per week

The Role:
This is an exciting time to join the organisation, as we are planning the refurbishment of our flagship 10-bedroom supported accommodation service alongside ongoing development of our wider accommodation portfolio.
The Facilities Manager is responsible for ensuring the safe, compliant and effective operation of all facilities at York Road Project, which includes a 10-bedroom main site shelter and three additional properties comprising a further 14 beds. The role ensures all buildings are maintained to a high standard, remain fit for purpose, and meet statutory and regulatory requirements.
Key Responsibilities:
Facilities Management
- The Facilities Manager monitors the delivery of maintenance activity across all sites, ensuring issues are escalated where standards, timescales or compliance requirements are not met.
- The role ensures all accommodation and premises are maintained to a high standard, remain fit for purpose, and operate safely and efficiently.
- The Facilities Manager reviews and improves facilities processes to enhance operational efficiency and ensure consistent delivery across all sites.
Health and Safety & Compliance
- Oversee Health and Safety compliance across all properties and service.
- This includes the development, review and maintenance of risk assessments for all properties and relevant work activities, ensuring they are suitable and sufficient, regularly reviewed, updated following changes or incidents, and effectively implemented.
- The role also ensures statutory inspections, fire risk assessments, fire safety compliance, emergency preparedness, HSE requirements, and DSE compliance for home working staff are in place and maintained.
- Responsible for ensuring compliance systems and documentation are maintained to an audit-ready standard.
- The role supports internal and external audits, including annual health and safety and fire risk assessment audits, ensuring evidence and corrective actions are completed.
Governance & Reporting
- As part of the role you will attend the charity’s Health and Safety Sub-Committee, providing updates on H&S, risks, compliance, audits, and improvement actions.
- The role contributes to audit processes, monitoring compliance status and ensuring follow-up actions are completed and embedded across the services.
Budgets & Operational Reporting
- Manage facilities-related budgets, reporting on spend, risks, compliance status and planned maintenance priorities.
- Develop a maintenance schedule for all properties.
Professional Standards & Ways of Working
- Maintain accurate and timely records of inspections, compliance checks, maintenance activity and statutory requirements
- Ensure all property and compliance documentation is audit-ready and consistently maintained
- Work collaboratively with the administrative team to ensure effective coordination of maintenance, suppliers and compliance systems
Knowledge and Skills:

- Experience in facilities, maintenance, property management and compliance roles.

Job Purpose
The hospital to home service works collaboratively with hospitals to support people when they are discharged from hospital to return home. As a Hospital to Home Outreach Worker, you will provide temporary, practical and emotional support to service users within their home and make sure they understand the care and local community services available to them.
The service is time limited, usually up to 4 or 6 weeks, to adjust and settle back in their homes.
Key Tasks

Assess the needs of service user’s either in hospital before discharge, or in their homes, and understand what barriers and enablers there might be to moving home.

Aid the discharge process and improve patient experience through tasks such as:

Key cutting
-
Co-ordinating with colleagues for the setup of the home environment such as checking fridge for in-date food
-
Checking heating, electric and gas is functional prior to discharge
-
Allowing access to the property for cleaning/repairs and receiving equipment
-
Provide and/or arrange the practical and emotional support needed to enable service users to return home and/or remain safely at home.

Be vigilant and make referrals/signpost for additional needs that our wider team, or partners can support. E.g. for benefit checks, energy advice, carers support, befriending.

Help maximise service user independence by adopting an enabling approach.

Develop a listening, caring and professional relationship with the service user and their families/carers.

Provide domestic practical support within the home such as:

Light cleaning,

Washing (e.g. bedding),

Support Coordinator
We are looking for an innovative, passionate and professional individual with excellent communication and organisational skills to join our Stroke Recovery Service in Sheffield.
This is an exciting opportunity to work with stroke survivors and their families to provide support following a stroke.
Position: 000016 Stroke Association Support Coordinator
Location: Home-based Sheffield. However, extensive travel across the region will be required as part of this role (including team meetings and other work related meetings)
Hours: Full-time, 35 hours per week
Salary: Circa £28,300 per annum
Contract: Fixed-term. Our services are contracted, we currently have funding until 31 March 2027

The Role
Reporting to the Service Delivery Coach, the Stroke Support Coordinator will deliver high-quality, person-centred support to stroke survivors and their carers/family members across Sheffield.
You will have the ability to support stroke survivors to identify their goals. Using strong assessment skills and a person-centred approach enabling them to maximise their recovery and improve communication.
Key responsibilities will include:
· Supporting newly diagnosed stroke survivors and their carers from hospital discharge into the community.
· Supporting a diverse caseload including anyone experiencing communication or cognitive difficulties
· Providing support via a combination of face-to-face visits, telephone calls, emails or letters and digital methods (such as video calls).
· Using a person-centred and person first approach to provide specific, tailored information, advice and support to stroke survivors and their carers.
· Empowering stroke survivors to make informed lifestyle changes which will help them to live life well after stroke.
· Working with other health and social care professionals across the stroke pathway to ensure high quality support.
· Working from home but as part of a team of coordinators.
· Delivering stroke support groups in the area.
· Visiting stroke survivors in hospital to introduce the Stroke Recovery Service and provide information and support.
About You
You will have:
· Experience working with people within a health and/or social care profession or other caring capacity.
· Experience of maintaining accurate records using IT skills and database systems.
· Effective caseload management and organisational skills.
· Excellent interpersonal skills to work with a diverse range of people.
· Experience designing and facilitating groups.
· A flexible working approach, being able to work independently and use initiative.
This role requires extensive travel across the service area to visit people at home, in hospital and community settings as well as running groups across Sheffield and also travel further afield for team and Locality meetings. This is an exciting opportunity to work with stroke survivors and their families to provide stroke recovery support following a stroke.
Position: 000012 Stroke Association Support Coordinator
Location: Home-based Herefordshire and Worcestershire. However, extensive travel will be required as part of this role (may include team meetings or other work-related meetings)
Hours: Part-time, 28 hours per week
Salary: Circa £22,600 per annum (FTE circa £28,300 per annum)
Contract: Fixed term. Our services are contracted, we currently have funding for this contract until 31 March 2027

The Role
The service aims to identify and address the needs of stroke survivors and carers across the stroke pathway, by providing a range of innovative support solutions, supporting them to meet their desired outcomes. A key part of the role will be to support service users and the delivery of the service.
Reporting to the Stroke Association Service Delivery Coach, the Stroke Association Support Coordinator will:
· Support new stroke survivors and their carers from hospital discharge into the community.
· Take a person centred approach to goal setting and support to enable stroke survivors and their carers to improve communication
· Provide personalised information, advice and support.
· Support stroke survivors to make informed lifestyle changes which will help them to prevent further strokes.
· Work collaboratively with NHS colleagues and other areas of the community to make a difference in the lives of people affected by stroke.
About You
You will have:
· Experience/background in a caring profession, ideally supporting people with disabilities
· Excellent IT skills and an ability to maintain accurate records.
· An affinity with the values of the Stroke Association.
· A flexible approach and an ability to effectively manage a caseload.
This role requires extensive travel across a large geographical locality; Herefordshire and Worcestershire, to visit people at home and in community settings. Candidates must be able to demonstrate how they can meet this requirement of the role
