About the Role
You will play a key role in delivering our therapeutic programme. Reporting to the Therapeutic Programme Manager and supporting the Family Caseworker, you will help plan, prepare and facilitate meaningful therapeutic sessions for children and their families. This may include leading group sessions supported by Teaching Assistants or collaborating with teaching staff involved in each child’s care.

A Support Worker will help to enable vulnerable people with complex support needs to manage their licences and move towards greater independence within the community by providing a flexible and individually tailored support package.
This role will be based at for Pound Lane, which is a high complex needs 24-hour service with same shift pattern.
The working pattern for this role is: - 5 days a week, working over a seven day pattern, weekends included. Shift pattern 8am-4am and late shift 1.30pm-9.30pm
What you'll do:
* Undertake key-working responsibilities for a caseload assigned by the Team Leader / Manager
* Oversee the daily running of the service ensuring all health and safety responsibilities are met
* Raise safeguarding alerts and complete incident reports within specified timeframe
* Undertake initial and continuous assessment of needs and potential risks and agree levels of support and actions
* Create support/action plans for providing appropriate support based on the assessment and reflecting the services and resources available
* Support customers to undertake domestic tasks wherever possible, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of living standards
* Support customers to move on from the service through their identified move on pathway liaising with our Resettlement Officer and Brent Housing team.

PURPOSE OF POSITION
This role is responsible for the service delivery of an effective, high quality, person centred support service to residents with a background of rough sleeping and range of medium to high needs. These includes entrenched drug and/or alcohol issues, mental health diagnosis and a range of offending backgrounds.
They will be the principal providers of support to residents in the Harbour Project team and will also work alongside Assistant Support Workers, to promote resident engagement through the development of positive relationships with our residents and contributing towards the promotion of a fulfilling, safe and inclusive environment.
RESPONSIBILITIES & ACCOUNTABILITIES
To be responsible to the Team Manager for the day-to-day delivery of the Harbour Project Team.
Use motivational and asset based interviewing techniques to interview, assess and create support plans and risk assessments that address the needs of residents.
Take a positive, trauma-informed approach to working with residents with complex needs and challenging behaviour.
Conduct regular reviews of support plans and risk assessments as required.
Take a pro-active approach in multi-disciplinary support required for residents.
Work within a Psychologically Informed Environment approach and strive to meet targets and deliver results.
Ensure a high standard of customer service is upheld
To attend team meetings and take part in service policy and planning.
To participate in team meetings, team review days and other meetings as agreed with the Team Manager.
To keep abreast of current housing legislation, welfare benefit legislations and other matters relevant to successful support and move on of residents.
To respond and process all referrals made to the Complex Needs service and ensure those accessing the service meet the service eligibility criteria.
To ensure all case work is properly recorded and all service offers and outcomes are entered clearly onto the case management system.
To actively collect follow up information with regards to the outcome of reconnections and report
To deal with the immediate support needs of the residents as appropriate.
To assess resident safety and develop risk management strategies with the resident and other involved services.
To ensure all service delivery policies and decisions made by the Team Manager or team are observed and followed through.
To keep accurate records and statistics on referrals, service outputs and outcomes etc.
To maintain a high standard of record keeping in the office and keep all financial and administrative systems including resident files in the office in an accurate and up-to-date manner in line with policy.
To ensure that all written work both for internal and external use is of a high standard.
To ensure the results of all assessments and all service offers are entered clearly and in a timely fashion onto the case management system.
To liaise and negotiate effectively on a day to day basis with outside agencies including the police, social services and other statutory and voluntary agencies.

Experience
A demonstrable level of experience and understanding of the range of approaches appropriate to working with challenging behaviour including current drug /or alcohol use, antisocial behaviour, offending, substance misuse and complex needs

Demonstrate an understanding of statutory compliances and standards such health and safety, equal opportunities, data protection, particularly within a residential setting.
Demonstrate excellent customer service skills. Ability to develop good working relationships and rapport with residents and stakeholders.
Using motivational interview techniques to produce and approach referrals, needs and risk assessments and support planning in a SMART manner
Knowledge of Psychologically Informed Environments (PIE).

What you'll do:
1. Keyworking, Support Planning & Recovery
? Hold a keyworking responsibility for at least one customer, meeting them at least weekly for
structured keywork sessions focused on support needs, wellbeing, mental health, daily living
skills, risk and recovery goals.
? Record keywork sessions fully on our systems and share relevant updates with the team so
that concerns, progress and agreed actions are clearly understood.
? Take the lead on your assigned customer's day-to-day support needs, including paperwork,
applications, appointments, meetings and practical support, while ensuring the customer has
an active role in how they are supported.
? Contribute to support plans, risk management plans and crisis plans for your assigned
customer, ensuring these are meaningful, up to date, co-produced where possible and linked
to clear achievable outcomes.
? Ensure your assigned customer's paperwork is completed accurately and within required
timescales, recorded on relevant systems and signed by the customer and management
where required.
? Where you will be away from work when customer paperwork or key tasks are due, provide
a clear handover and agree who will take responsibility in your absence.

About you:
Experience & Knowledge
? NVQ Level 2/3 or equivalent, or relevant experience in mental health, social care, housing,

? Understanding of recovery-oriented practice and trauma-informed approaches, or a willingness

A Support Worker will help to enable vulnerable people with complex support needs to manage their licences and move towards greater independence within the community by providing a flexible and individually tailored support package.
This role will be based at for Pound Lane, which is a high complex needs 24-hour service with same shift pattern.
The working pattern for this role is: - 5 days a week, working over a seven day pattern, weekends included. Shift pattern 8am-4am and late shift 1.30pm-9.30pm
What you'll do:
* Undertake key-working responsibilities for a caseload assigned by the Team Leader / Manager
* Oversee the daily running of the service ensuring all health and safety responsibilities are met
* Raise safeguarding alerts and complete incident reports within specified timeframe
* Undertake initial and continuous assessment of needs and potential risks and agree levels of support and actions
* Create support/action plans for providing appropriate support based on the assessment and reflecting the services and resources available
* Support customers to undertake domestic tasks wherever possible, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of living standards
* Support customers to move on from the service through their identified move on pathway liaising with our Resettlement Officer and Brent Housing team.

Our client looking for an Assistant Support Worker (known internally as Duty Worker) to join their team in Hounslow. The Hounslow LIFE service is a large multi-site service that currently has a staff team of 31 supporting 86 clients; covering specialism's such as Homelessness, Substance Use and Mental Health. The service focuses on supporting residents around their strengths and aspiration's for the future.
This is an exciting opportunity to join their new Assessment Hub team at Apple Tree Cottage, providing rapid assessment and support into appropriate accommodation to clients over a 28-day period, ensuring those at risk of rough sleeping and homelessness can be directed to services suitable to their immediate, medium and longer term needs.
The role of a Duty Worker is to jointly manage the safe day to day running of the project with other members of the team and to provide support and advice to clients.
Please note, due to the needs of their services, there is a requirement to work a shift rota, so flexibility is required. This will include some weekends and bank holidays.
For this role staff can claim an additional 20% pay premium for hours worked between 8pm and 8am Monday to Friday, and all hours worked on Saturday and Sunday.
