Job Purpose 

The weekend Hospital to Home Project Officer is responsible for responding to referrals received over the weekend from both Royal London Hospital & Homerton Healthcare NHS Foundation Trust. You will work with the manager on Friday to deliver services and plan visits on the weekend for you to attend.   
  

The service supports people returning after a stay in hospital, helping them to settle back at home, regain confidence and support with some practical tasks, e.g. shopping/cleaning. You will manage referrals, carry out assessments, and in liaison with the manager, allocate service users to Hospital to Home Outreach Worker caseloads. You will provide direct frontline support and co-ordinate visits to the service users’ home in preparation for their return home. You will work collaboratively to lead and manage the weekend service to support the smooth transition from hospital to home for service users. The service is time limited, up to 4 or 6 weeks, to adjust, build confidence, and settle back in their homes.    

You will also be required to undertake community-based outreach as part of service delivery, including visiting service users in their homes both pre-and post-discharge. This includes conducting access visits, welfare checks, and providing practical support such as shopping assistance to ensure a safe and sustainable transition from hospital to home. Over the weekend you be based in the hospital, with lone working visits to client’s properties for support.  

Key Tasks 

• Act as the first point of contact for all referrals over the weekend across both hospitals.  

• Meet with patients in the community and/or hospital and carrying out assessments and risk assessments.   

• Enter referrals on to AUKEL’s case management system Salesforce in accordance with GDPR guidelines.  

• Allocate referrals to Hospital to Home Outreach Workers using tech available Field Service App, training will be available.  

• Collect feedback from service users and upload to case management system.  

• Deputise for the manager at discharge planning and other meetings as required.  

• Monitor the case loads of each of the Hospital to Home Outreach Worker (HHOW) to ensure they are evenly and fairly balanced.  

• Work collaboratively with other agencies providing support services.  

• Work with people with a range of needs, dealing with issues ranging from social isolation and keeping people engaged in their community, to prevent unnecessary readmission to hospital.  

• Plan and develop person centred interventions to provide short term support for people after hospital discharge.  

• Deliver the front-line support services over the weekend such as:   

• Undertake home visits to service users pre- and post- discharge to support safe and timely hospital discharge.  

• Provide welfare checks to monitor service users’ wellbeing during post-discharge period.  

• Deliver practical, person-centred support during visits, including shopping assistance and other essential tasks to aid independent living.  

• Identify and respond to risks within the home environment, escalating concerns where appropriate  

• Provide occasional cover for the A/L of other Project Officers   

• Work with people with a range of needs, dealing with issues ranging from social isolation and keeping people engaged in their community, to preventing unnecessary admission to hospital.  

• Supervise, support and develop HHOWs and volunteers in your service.  

• Liaise with the handyperson service about referrals and jobs for service users.  

• Ensure HHOW are resourced to sign post service users to other services when the home and settle service intervention ends. 

Administration 

• Keep AUKEL’s case management system up to date, input all data, and ensure all referrals, interventions, time spent with service users are recorded.  

• Allocate referrals to HHOW using tech available Salesforce ‘Field Service’ App.  

• Ensure data collected meets the requirements of the service’s contract monitoring and key performance indicators.  

• Ensure service user feedback questionnaires are sent at the end of each intervention and record returns.  

• Ensure admin tasks relating to service users, HHOW or general admin is carried out in a timely and efficient manner. 

Quality  

• Be familiar with and to implement AUKEL policies and procedures in line with our Values.  

• Ensure the service is delivered in accordance line with Care Quality Commission principals.  

• Ensure integration with other AUKEL projects, and service users are referred and/or sign-posted to appropriate support services available to them.    

• Respond to safeguarding or other concerns and make appropriate reports and management action 

Liaison  

• Represent AUKEL and participate in appropriate external meetings and events to remain aware of local, regional and national issues affecting quality and compliance affecting care and home support services.  

• Work in collaboration with hospital colleagues including discharge teams, A&E, ward staff, community teams to support the successful discharge of people and reduce readmission.  

• Liaise with other AUKEL home and settle services across London and work collaboratively to maintain effective services, including sharing staff where appropriate.  

• Work closely with the AUKEL handyperson service to meet contractual obligations where a handyperson service is provided as part of the service.  

• Liaise with contractors to arrange deliveries and works to service user’s homes.  

 General 

• Meet regularly with your line manager for support, supervision, and appraisal.  

• Attend team and staff meetings, and other meetings as required.  

• Undertake any other duties within the competence of the post holder as may be required from time to time for the continued smooth running of AUKEL.  

• Complete any training which is required to fulfil the role.

Job description 

Job Purpose 

The hospital to home service works collaboratively with hospitals to support people when they are discharged from hospital to return home. As a Hospital to Home Outreach Worker, you will provide temporary, practical and emotional support to service users within their home and make sure they understand the care and local community services available to them.

Key Tasks 

• Assess the needs of service user’s either in hospital before discharge, or in their homes, and understand what barriers and enablers there might be to moving home. 

• Aid the discharge process and improve patient experience through tasks such as:

• Key cutting

• Co-ordinating with colleagues for the setup of the home environment such as checking fridge for in-date food 

• Checking heating, electric and gas is functional prior to discharge 

• Allowing access to the property for cleaning/repairs and receiving equipment

• Provide and/or arrange the practical and emotional support needed to enable service users to return home and/or remain safely at home. 

• Be vigilant and make referrals/signpost for additional needs that our wider team, or partners can support. E.g. for benefit checks, energy advice, carers support, befriending.  

• Help maximise service user independence by adopting an enabling approach. 

• Develop a listening, caring and professional relationship with the service user and their families/carers. 

• Provide domestic practical support within the home such as:

• Light cleaning,

• Washing (e.g. bedding),

• Changing bed sheets etc. 

• Prepare and service drinks and simple meals with or for the service user ensuring nutritional needs are met. 

• Accompany service user outside the home e.g. hospital/GP appointments etc. 

• Support service users with other tasks e.g. making telephone calls on their behalf, reading and responding to correspondence, completing simple forms (training will be provided) as directed by the service user’s etc. 

• Ensure services provided by AUKEL in the community are safe and person centred. 

• Report safeguarding concerns, in accordance with AUKEL safeguarding procedures. 

• Provide cover in the case of sickness and annual leave periods of colleagues. 

• Support service user reviews, ensuring all visiting information is uploaded on to Salesforce. 

• Support volunteers in the team and provide support and guidance to their development. 

• Work with a range of professionals including: 

• Supporting health and social care professionals with patient discharge 

• Liaising with the hospital health and social care professionals (e.g. Social Workers) to facilitate smooth supported discharge. 

• Handypeople & contractors.

Administration  

• Input all records, reviews, and visiting information to service user’s case notes in a timely manner to maintain up-to-date and accurate records on Salesforce.

• Report all “no access” failed visits, or changes in service user’s condition or circumstances in accordance with AUKEL’s policies and procedures. 

• Ensure service user comment sheets are completed.

• Ensure service user’s feedback questionnaires are sent at the end of each intervention and record returns. 

• Submit travel expenses regularly for sign off by manager.

EIA is seeking to recruit an Outreach and Communications Officer to strengthen the visibility, influence and coordination of its climate campaign.

The role sits at the intersection of communications, public affairs and stakeholder engagement. The postholder will focus on translating and amplifying EIA’s policy and advocacy work through media, stakeholder engagement and external positioning. An important aspect of the role will be to support engagement and coordination with EIA partners, to build the movement and amplify their efforts.

The postholder will not be responsible for developing detailed policy positions or leading policy campaigns but will work closely with the campaigns team to ensure EIA’s analysis and priorities are effectively communicated and reach key audiences.

Key areas of responsibility

Media and strategic communications

· Support the development and delivery of communications strategies aligned with campaign priorities

· Build and maintain relationships with journalists, editors and key media outlets

· Identify strategic opportunities for media engagement and external visibility, particularly around key policy moments

· Shape narratives, draft and place op-eds, comment pieces and reactive media lines

· Translate campaign priorities and technical analysis into clear, compelling messaging for media, policymakers and external stakeholders

· Contribute to campaign strategy by advising on outreach, positioning and narrative development.

Stakeholder engagement

· Support the development and maintenance of relationships with policymakers, including Members of the European Parliament (MEPs), Members of the British Parliament, European Union institutions and other relevant stakeholders

· Support campaigners in planning and delivering stakeholder engagement, including preparing messaging and coordinating meetings

· Support the organisation of events, including identifying and securing speakers and managing outreach and invitations

· Attend key international meetings and fora (e.g. COP, Montreal Protocol) to identify strategic developments and key messages, and produce timely communications outputs, including media lines, briefings and stakeholder updates.

Coalition and network coordination

· Support engagement with subgrantees, including onboarding, facilitating their integration into relevant coalitions and ensuring alignment with EIA’s communications and outreach objectives

· Act as a point of contact for subgrantees on coordination and communications

· Support coordination across partners and stakeholders during key moments, ensuring alignment on messaging and effective external engagement.

· Demonstrated experience in communications, public affairs, campaigning or media work within NGOs, civil society or advocacy organisations

· Proven experience engaging with journalists and securing media coverage

· Understanding of global and EU climate change policy and/or environmental governance

About this role

The role

The Armed Forces Project Officer will be responsible for the day-to-day management of the Armed Forces and Veteran Community Projects.

The post holder will have knowledge and experience of outreach/engagement, as well as experience of planning and running events and projects. The role requires flexibility and multi-tasking, as well as confidence to complete varied tasks as delegated. The role requires the coordination and chairing of working group meetings and representing the organisation at armed forces events and steering groups.

Overall, duties will differ according to the requirements of the HWE operational plan. Day-to-day responsibilities of the role will include ensuring that project work is completed to a high standard and all reporting requirements are complied with. The main purpose is to ensure that the influence of HWE is maximised through the delivery of high-quality and well ­organised events and activities and meetings that will capture and articulate the voice and lived experience of the people of Essex.

Key accountabilities

· To work with HWE staff, and especially the Senior Management Team, to support the delivery of high quality and well-organised HWE Armed Forces Projects. This includes:

o Plan and deliver the Armed Forces Working Group online and in person meetings.

o Plan and deliver the Primary Care Network Armed Forces Working Group online and in person meetings.

o Evaluate and report on impact of the working groups.

o Enhance the attendance of the working group meetings.

o Be a point of contact for the armed forces and veteran community within Essex.

The post holder will be highly organised, and an engaging and effective communicator, with good written and verbal skills. You will be capable of building effective relationships with individuals and organisations, including volunteers, and will be comfortable and highly capable in planning, coordinating and delivering different projects and events. We’re looking for an experienced Project Officer to play a key role in delivering and reporting on a major multi-year funding programme supporting Samaritans’ work across our London region.

You will lead the coordination, administration, and reporting of activity funded through a significant seven-year partnership with City Bridge Foundation. About the Project

Communities Prepared works with volunteers and communities across England to help them build the skills, confidence and knowledge needed to prepare for, respond to and recover from emergencies. This includes supporting communities to plan for climate-related risks such as flooding, storms, heatwaves, and severe weather.

Despite the successes we have had through our programme to date, we are not currently reaching diverse enough audiences and too often there are people missing from the resilience sector. This needs to change. To address this we are now embarking on an exciting new UK-wide partnership programme funded through the National Lottery Community Fund’s Climate Action Fund over the next five years.

The programme responds to growing evidence that climate-related emergencies, including extreme heat, flooding, fire, cold and severe weather, disproportionately impact marginalised communities, while those same communities are often excluded from resilience planning and decision-making.

The programme brings together Equally Ours, Communities Prepared (part of Groundwork South), and the VCS Emergencies Partnership (VCSEP, part of the British Red Cross) to strengthen climate resilience by ensuring that communities experiencing discrimination and disadvantage are at the heart of climate preparedness, response, recovery and policy-making.

Through a rights-based and co-produced approach, the programme seeks to shift climate resilience policy and practice away from models that frame communities as “vulnerable”, and towards approaches grounded in agency, participation, equality and shared responsibility.

Key Responsibilities

As Senior Project Officer, you will:

• Work with the project team to map stakeholders and co-develop a structured engagement plan to recruit and work with marginalised communities.
• Build a collaborative and trusting relationship with the partner organisations and other stakeholders for this project, ensuring this work meets their expectations and complements areas of the work programme led by others.
• Work with project participants and partners to co-design Communities Prepared material and delivery methods that are more accessible and relevant to diverse audiences.
• Lead on the delivery of resilience training and workshop sessions with diverse audiences, both in-person and online.
• Work with project participants and partners to co-design a train the trainer model that will enable diverse audiences to deliver resilience training within their own communities, amplifying reach and impact.
• Work with the core Communities Prepared team to adapt our offer with the long-term goal of the Programme reaching more diverse audiences, especially those seldom heard in this sector.

The role involves regular travel across the UK, with a focus on South West England during the project’s pilot phase.

We are looking for someone with:

• Experience in community engagement or community development.
• Experience delivering training or workshops.
• Strong organisational and project management skills.
• Excellent communication and relationship-building skills, confident to engage people from a range of different backgrounds.
• Experience working with multiple stakeholders or partners.
• Confidence working independently in a remote role.
• An interest in climate change, community resilience and wider community development.

About the role

We are seeking an organised, dynamic and people-focused Volunteer Coordinator to lead volunteering across both our programmes. This role will supporting and coordinate volunteers across

•  Touching Safe Ground – supporting women and children seeking sanctuary.
•  Well Old – supporting older people experiencing loneliness and isolation.

You will support our established volunteer team within Touching Safe Ground, and develop volunteering opportunities within Well Old. This role is responsible for recruiting, coordinating, supporting and developing a diverse team of volunteers who make our sessions and activities possible.

You will help ensure that volunteers have a positive, rewarding and meaningful experience, while enabling Globe Community Project to deliver high-quality support to participants. What you’ll do

• Coordinate volunteer availability for both of our projects, including weekly Touching Safe Ground sessions. 
• Recruit new volunteers through social media, volunteering websites, local university liaison and community outreach.
• Act as the first point of contact for volunteering enquiries, manage applications, conduct interviews, and manage references and onboarding requirements
• Build and maintain effective partnerships and referral pathways with organisations, universities and community groups, including including universities, faith organisations and community groups across East London.
•  Ensure all volunteers complete appropriate DBS checks, Volunteer Agreements and any other required safeguarding processes.
•  Deliver volunteer inductions and provide practical training, supporting the learning, development and wellbeing of volunteers, and identifying any further training needs as they arise.
•  Network and conduct desk research to identify wellbeing, learning and CPD opportunities for volunteers, particularly those with lived experience of migration or seeking sanctuary.
• Administer volunteer travel expenses and maintain accurate records on shared systems.
• Attend sessions regularly alongside volunteers to build relationships, provide support and understand their experiences.
•  Maintain regular communication with volunteers and respond promptly to questions and concerns.
•  Organise volunteer thank-you events and other opportunities to recognise and celebrate their contributions.
•  Monitor and report on volunteer numbers, recruitment, retention, successes and challenges.
•  Attend relevant staff meetings as required.
• Report safeguarding concerns immediately and follow organisational safeguarding procedures.

Community Officer
Shrewsbury, Shropshire

Role Title: Communications and Engagement Officer PT, 3 days per week

(Fixed Term Contract – 21 months June 2026- March 2028)

Reports to: Assistant Curator

This is an exciting part-time position to help develop our new Seeing Nature, Shaping Culture: Art and Identity in Prehistory project. The Communications and Engagement Officer will play a key role in delivering this new project, a major two-year exhibition project funded by the National Lottery Heritage Fund. The role will involve the coordination and promotion of a community activity programme to reflect the objects and themes in the two main exhibitions ‘Iron Age Farmsteads’ and ‘Shaping Britain: the influences of European Cultures on Prehistoric Britain.’

The post-holder will help to design, deliver and promote an inclusive community programme that connects new audiences with Prehistoric Britain through creative, hands-on activities and interpretation of the planned exhibitions, working closely with the Assistant Curator to create partnerships with local cultural organisations and recruit visitor experience volunteers. The post will involve working weekends and some bank holidays.

Celtic Harmony is a charity that aims to improve the well-being of children, young people and adults through engaging, interactive experiences with the visual heritage of Prehistory and through the lifelong learning programme, creating a more sustainable future in harmony with the natural world.

Location: Celtic Harmony Camp has been awarded the Hidden Gem accolade from Visit England and is a reconstructed Iron Age settlement nestled in acres of Hertfordshire woodland - with plenty of clean air! The office is located in The Prehistory Centre with all modern amenities, own transport is recommended as we are in a rural setting.

Relationships 

• The Communications and Engagement Officer is accountable to the Assistant Curator in all matters. 
• The Communications and Engagement Officer develops and delivers community activities that speak to the themes of the exhibitions.
• The Communications and Engagement Officer works in collaboration with the Assistant Curator and CEO to develop and market community activities.
• The Communications and Engagement Officer supports and reports to the Assistant Curator and CEO for the collaboration with community groups.
• Proactively contribute in team meetings and initiatives for new initiatives and events.

Key deliverables of the role:

• Design, coordinate, and deliver accessible community activities and events such as after school clubs, society tours, U3A talks, Saturday clubs for young people, in accordance with the exhibition programme.

Opportunity to contribute to a specialist think tank's leading-edge approach to integrating lived experience into reseach and policy influencing and support a growing migrant community of practice in Scotland.Migration Policy Scotland (MPS) is an independent charity. Our mission is to build a better migration future for everyone. We offer principled, effective solutions to confront the challenges and harness the benefits and opportunities migration brings. Our work combines learned and lived experience grounded in the realities of migration in Scotland.

MPS pioneers an approach that integrates migrant lived experience into research-driven policy influencing. With funding from The National Lottery Community Action Fund we are working to further develop and embed this area of activity and to grow our migrant community of practice. We are committed to support the work of our Migrant Lived Experience Panel. We will shortly be launching the first year of our new Capacity Building and Leadership Development Programme for migrants and migrant community organisations, alongside continuing to develop of our migrant community of practice.

We are seeking a part-time participation officer (0.3 FTE) to support our senior staff in the delivery of this important and ambitious work.

Potential candidates for this role are likely to have a background in community engagement and/or volunteer support and familiarity with social research methods.
