A Support Worker will help to enable vulnerable people with complex support needs to manage their licences and move towards greater independence within the community by providing a flexible and individually tailored support package.

The shift pattern of the role consist of: 40 hours 1 x scattered site 2 x Pound Lane Shift pro-rota 7.30-3.30 and 1.30 -9.30 shift pattern with weekends included.

What you'll do:
* Undertake key-working responsibilities for a caseload assigned by the Team Leader / Manager
* Oversee the daily running of the service ensuring all health and safety responsibilities are met
* Raise safeguarding alerts and complete incident reports within specified timeframe
* Undertake initial and continuous assessment of needs and potential risks and agree levels of support and actions
* Create support/action plans for providing appropriate support based on the assessment and reflecting the services and resources available
* Support customers to undertake domestic tasks wherever possible, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of living standards
* Support customers to move on from the service through their identified move on pathway liaising with our Resettlement Officer and Brent Housing team.
* Develop and maintain links with all key agencies and service providers in the local community
* Empower customers to ensure they receive the service and benefits they are entitled to

As a Floating Support Worker, you’ll support people living across Westminster to sustain their tenancies, navigate challenges and build the confidence and skills needed to live independently. This peer support project is delivered in partnership with South West London and St George’s Mental Health NHS Trust to support Carers of people with mental health conditions.  This project will work to support Carers through one-to-one support, outreach, training and peer support groups.

About the Role

We're hiring a Housing Support Worker to join our floating support service which is based in Brent. The service supports adults who have been or are at risk of homelessness, have experienced social exclusion, have previous criminal convictions, suffer with their mental health, have substance misuse, and/or could have a learning disability. You will support participants on their journey towards recovery, reablement, and independence, all within a compassionate, trauma informed environment.

In this role, you'll be at the heart of delivering high quality, person centered support through tailored support plans which provide practical interventions, to empower our participants to overcome personal challenges, develop life skills, and achieve their personal goals. You will also work closely in partnership with various organisations to support the different needs of our participants, which may include signposting. Key Responsibilities Include:

• Support participants will all aspects of their housing needs to support them ensure sustainable housing.
• Co-create tailored support plans which are centered towards the participants needs.
• Signpost participants with local providers to support with their independence such as with Job Centres, employment support hubs, community support centres, mental health trusts and other as relevant.
• Recognise support needs in an individual and initiate appropriate interventions to prevent further deterioration.
• Complete safeguarding and other referrals where required.

In this role, you will provide support to all the women, including running co-designed activities and accompanying women to appointments. You will work closely with Specialist Support Workers who manage individual resident support and safety plans. The post holder will undertake 8-hour shifts on a 7-day rota, covering the hours 07:45 am to 20:15 pm. Please be aware that the building has several floors and no lifts.

As part of our learning and development Anne Harris Skills Development Programme, we aim to provide a high level of training and development opportunities for all staff, so you are able to perform to the best of your ability, achieve individual and team objectives aligned to Rainbow Trusts strategic plan, supporting staff to be their best and feel a valued member of a high performing organisation.

Our Family Support Teams are given the opportunity to complete a number of diverse training courses in their first 12 months, including but not limited to: Mental Health First Aid, Makaton, introduction to play, drawing and talking training.
