What you’ll be doing?
This role has been developed to work within a specific Lambeth Primary Care Network (PCN) to support their older population access the right support and services that they need to live and age well. This is a new post, and the successful candidate will be key to ensuring the development of new partnerships and local relationships to support Lambeth’s ambitions to ensure older residents can live and age well. Your main responsibilities will be:

Undertake mapping of the local community, using existing Age UK Lambeth knowledge and resources and creating new tools for this project to identify services and support available to older adults.

Focus mapping exercise on identifying community interventions that will address health inequalities and improve wellbeing for older adults.

Using Age UK Lambeth Guides as a template, develop a specific, local guide/ directory for this project.

Organise, facilitate and deliver training sessions to PCN staff on how to use the guide, developing and building their understanding on local resources and provision in the local area.

Network to identify new services and community groups to be included within the service guide.

About the role
The Senior Case Worker will work as part of the SPOT Navigator team based alongside the Southwark SPOT Street Outreach team. The aim is to support a known cohort of clients who are stuck within the cycles of homeless and ill-health and have not been able to sustain a stable life outside of this.
This will be achieved through strength-based assessment, using creative engagement techniques and co-producing action plans aimed at linking clients with services and community groups. Your work will improve health outcomes for clients and promote harm reduction by working alongside specialist partner agencies. You will be working with clients on the streets in an outreach capacity or in various types of accommodation.
In this role you will:
- Work in partnership with local authorities, accommodation services, statutory and voluntary agencies to ensure that clients transition away from the streets and into the appropriate accommodation with the correct support in place.
- Manage a caseload of clients experiencing multiple disadvantage who may not have stayed in stable accommodation for long periods of time.
- Have a great opportunity to assist people to change their lives meaningfully for the better.
In this non-shift role, you will work mainly within normal office hours Monday – Friday, however, on occasion you may be asked to work flexibly and conduct early or late shifts starting at 6am or ending at 3am. For this role staff can claim an additional 20% pay premium for hours worked between 8pm and 8am Monday to Friday, and all hours worked on Saturday and Sunday.

What you'll do:

Peer Support Worker

The Peer Support Service operates alongside other clinical and wellbeing services to provide formalised peer support for people living with common and complex mental health needs and complex emotional needs, including personality disorder.
The service compliments and collaborates with NHS Peer Support Workers and is delivered from community-based locations and NHS Hubs across Kensington and Chelsea and Westminster to help people regain control over their lives, shaped by their own unique recovery process.
The role involves experiential sharing, connecting relationships and community building to deliver a range of peer support activities, including one to one peer support, peer support groups, social peer support, peer support training and Living Well Workshops.

Our Emergency Winter Night Shelter Service is run seven nights a week in venues across west London including: Fulham, Kensington, Hammersmith and Richmond.

We are looking for an individual who will thrive in a fast-paced, rewarding role helping to change the lives of young people by taking up a fixed-term role as a part-time Project Worker in our IntoUniversity centre in North Islington. You will work closely with our existing team, supporting the delivery and organisation of the IntoUniversity programme in the centre.
Contract: Fixed term until July 2025, part-time 7.5hrs per week
Start date: As soon as possible, as agreed with the candidate and subject to satisfactory references and an enhanced DBS check, and right to work checks.

Main duties of the role
To deliver the IntoUniversity programmem effectively to children and young people in different settings, including schools-based workshops, Academic Support sessions and educational visits. This includes positive behaviour management and adherence to the IntoUniversity safeguarding policy.

To assist the IntoUniversity Centre Leader in liaising with schools and other partner bodies, in organising the classroom and resources and in planning project delivery.

What you'll do:
- Building supportive, trusting relationships with young people and creating a positive atmosphere

About the Service
We have two refuges in Islington and provide emergency accommodation to women and children fleeing domestic abuse and sexual violence. We are there for women at a time of crisis and help families take their first steps to freedom and recovery.
About the Role
You will provide advice and support to women who have experienced domestic abuse in a refuge setting, across two sites. You will carry out risk and needs assessments, develop support plans and carry out key work with women survivors. You should have a minimum of two years’ experience of working with this or a similar client group. An in depth understanding of the needs of women and children from a diverse background is essential.
You will be expected to work across the two refuges as part of your role
About You
The ideal candidate will have in-depth knowledge of domestic abuse and its impact on women and their children, as well as experience of providing emotional and practical support to victims of domestic and sexual violence.
You will have experience of managing risk and following case management procedures to meet the needs of the client group, and the ability to work with women to help them to understand their needs, risk assess, develop safety plan and support them in linking with other agencies.
You will have a working knowledge of the available legal remedies, housing and welfare benefits, as well as experience of providing workshops.
Outside of this, you will have excellent administrative and time management skills and the ability to effectively manage a caseload.

This is a highly rewarding role for an Outreach Worker to support parents and carers of children under 5 at Westminster Children Centres. We are currently looking to recruit an Outreach Worker in Westminster area to provide accessible information to parents and carers, particularly those who are marginalised, to ensure they receive the support they need in order to increase opportunities and strengthen family life, bringing better outcomes for children.
Fluent in Arabic, the role will include developing an understanding of the local area including services and facilities. You will also need to pro-actively engage with vulnerable and isolated families and carry out home visits, as required. The role will include developing good relationships with health visitors and liaising with other agencies regarding the needs of parents and carers. In addition, the successful individual will help to organise social events to bring the community together.
We are looking for someone who has experience of community and outreach work with diverse communities in an inner-city environment. This will include experience of a wide range of methods/techniques to reach out and engage with isolated individuals/communities. Excellent communication skills, both oral and written, along with the ability to empathise with parents and carers are required. A non-judgmental approach is essential. The ability to network is also important.
