About the role
Case Work-Navigators undertake an exciting role providing the link between formal health and social care and wider community forms of support. You will have the opportunity to work with clients aged 16 years of age and above, helping to identify ways to improve health, sustain wellbeing and quality of daily living.

PURPOSE OF POSITION
This role is responsible for delivering a high-quality, trauma-informed and gender-informed support service for women living in the Complex Needs service. The post-holder will hold a caseload of women residents who may be experiencing multiple disadvantage, including homelessness, rough sleeping, domestic abuse, mental ill health, substance use, exploitation, offending histories, social isolation and complex trauma.
The Women’s Support Worker will provide persistent, compassionate and practical support to women who may find it difficult to trust services, sustain engagement or accept support consistently. The role requires someone who can build safe, respectful relationships at the woman’s pace, while maintaining clear professional boundaries and supporting women to stabilise, feel safer, reduce risk and work towards their own goals.
The post-holder will work as part of the wider frontline support team, leading on support planning, advocacy and multi-disciplinary working for women residents. They will also support colleagues’ day to day to apply gender-informed practice, strengthen engagement with women and ensure support is coordinated, responsive and centred on women’s safety, choice and wellbeing.
This role forms part of a seven-day rota and will include some evening and weekend working.

St Mungo’s Bristol Street Outreach Team will be recruiting to a number of Outreach Worker roles as part of an increased number of opportunities across rough sleeping services. This could include working with focused groups of clients including those residing in vehicles on the roadside or people who have been rough sleeping for a particularly long time, some of whom are part of the ‘Target Priority Group (TPG)’.
Our belief is that rough sleeping is an intrinsically harmful and dangerous activity both for the individual and for the wider community. Therefore, we take a proactive and assertive approach to supporting people away from this activity. Our aim is to support people to recover from the effects of rough sleeping and move away from a street lifestyle permanently.

The Place Impact Researcher will gather, analyse and apply insight, research and evaluation evidence to support programmes, partnerships and place-based activity. As a Peer Support Worker, you will manage a caseload of individuals requiring early intervention and preventative mental health support, using your knowledge and experience to empower them on their recovery journey. You will deliver group activities and programmes, promote peer support networks, and help service users actively participate in co-producing their support and achieving their goals. Working collaboratively with community mental health teams, social workers, housing providers, health professionals, and employment services, you will encourage engagement with wider support networks and opportunities. You will monitor wellbeing and safety, maintain accurate records, complete assessments and reviews, support colleagues, and contribute to the effective, inclusive, and recovery-focused delivery of the service.

Strong listening, assessment, risk awareness, and signposting skills are essential, alongside knowledge of recovery tools, local wellbeing services, and safeguarding procedures. You should be confident working under pressure, managing competing priorities, and collaborating with colleagues and volunteers. Good IT, literacy, and numeracy skills are also required, together with the ability to work independently and as part of a team.

* Building supportive, trusting relationships with customers and creating a positive atmosphere
* Carrying out holistic assessments of new customers which incorporate relevant statutory referral information
* Conducting regular key work sessions that are innovative and engaging in order to achieve Support Plan goals
* Create support plans for providing appropriate services based on on-going assessment and reflecting the services and resources available to enable customer to progress in their recovery

* Provide support and access to relevant services to enable customers to continue their recovery and empower customers to self-manage their medication regimes through individual re-enablement programmes and informed decision making
