The Role
Our Practitioner Led Assessment Service is the entry point for our beneficiaries seeking support from the Charity. Our Assessment Practitioners work in partnership with our beneficiaries to identify goals, individual needs and appropriate solutions.
The team provide advice, information and support and work collaboratively with the multi-professional team to ensure our beneficiaries are offered the right support at the right time.
Reporting into the Beneficiary Experience and Continuity Lead you will be responsible for day-to-day management of the practitioner led assessment service, holding a small case load and reviewing and resolving complex cases with the team.
You will work collaboratively with key stakeholders across health improvement and rehabilitation services, to ensure services are integrated, person centred, outcome focused and based on an ethos of recovery.

About You
An excellent communicator, with a can-do-attitude, you will be a registered health or social care practitioner with a broad skill and knowledge base and will be experienced in managing a multi-professional team.

- Be a registered health or social care practitioner, with membership of a relevant professional body
- Have experience of managing a multi-professional team within a health or social care setting
- Have experience of conducting telephone assessments (health and wellbeing), service user risk assessments and delivering advice and information to improve health and wellbeing

About the role
In this role, you will offer clinical supervision to the Macmillan Support Line teams including advisers, nurses, line managers, service knowledge specialists and other groups and individuals as needed. As part of their continuing professional development, you will enable colleagues to reflect on their practice in supporting people affected by cancer while maintaining their own wellbeing.
You will be part of a team of 3 part-time clinical supervisors and will be given opportunities to feedback on recurrent themes to the organisation. You will learn about organisational wellbeing and be a key part of a team capable of working in an emotionally challenging, fast paced, live contact centre environment.
You will be running regular small group supervision sessions (approximately 3 per day) for 1hr 15’ each, managing a caseload of approximately 25 groups monthly. You will also offer individual supervision where needed.
About you
The successful candidate will demonstrate the following skills and experience:
- You must hold one of the following qualifications: Clinical Supervision Level 6 or above; accredited Counsellor or Psychotherapist (British Association of Counselling and Psychotherapy, UK Council for Psychotherapy, Health Professionals Council)
- Your experience of delivering both 1-1 and group supervision virtually will be essential
- Experience of delivering supervision to colleagues who provide a live, frontline, fast paced health related service is desirable

CDAS (Stoke on Trent Community Drug and Alcohol Service) is a new, all age service supporting individuals, their families and communities. Our busy team currently helps around 1500 people at any one time and we want to help even more by transforming our service to deliver from multiple venues and make more effective use of the resources we have.
You will lead on and be responsible for the provision of clinical care in our service, ensuring that our standards of care allow clients to achieve their recovery goals. A dynamic leader, you will guide and support clinical staff working within the service, working hand in hand with the service manager to foster a continuous improvement mindset and ensuring that evidence based best clinical practice is delivered in line with national, organisational and local standards.Working in co-production with operational managers, you will lead the continuous development and governance of high quality, effective and safe services within the budget and the contract. It will be vital for you to develop strong and positive relationships with key stakeholders and commissioners for the continuity of service, as well as leading on delivering opioid substitution and detoxification treatment, alcohol detoxification and relapse prevention treatment and other clinical interventions.
This is a full time and permanent post. Working hours will cover Monday to Friday 9.00am to 5.00pm, late shifts from 12.00pm to 8.00pm and some Saturdays.
You will be a compassionate and motivated registered nurse or pharmacist who holds a prescribing qualification, with the ability to connect quickly with the people we support, treating them respectfully and with dignity. We’re looking for substantial experience working within the substance misuse arena, as well as experience working in a supervisory role within a multidisciplinary team. You will be highly organised, with excellent communication skills and able to manage your time effectively. The Role
The Safeguarding Lead will bring their extensive knowledge and experience to this role to offer the highest level of support for LE staff and all of our client groups (including children, young people and adults). There would be a requirement to take lead responsibility for safeguarding and child protection across the Organisation and take part in strategy discussions and inter-agency meetings.
They will advise, train, and support all members of staff on risk, incident, safeguarding, and child protection matters, and liaise with relevant third-party agencies such as the local authority and police.

Main areas of responsibility include:
- To act as a source of support, advice, and expertise in managing safeguarding and clinical issues for LE staff.
- To be fully equipped to independently manage clinical risk safely and in line with all LE policies and procedures, in consultation with the member of staff raising a safeguarding concern.
- Coordinate the safe check-in and check-out of all clinical staff scheduled to work during the shift.
- Oversee the management of any risk/safeguarding concerns ensuring that relevant paperwork is completed and signed off within adequate timeframes.

- Support staff during the referral, triage and assessment process
- Liaise with the SMT and the local authority designated officer(s) (LADO) for child protection concerns in cases which concern a staff member

- Report cases to the police where a crime may have been committed
- Communicate relevant safeguarding issues to the SMT

Job Title: Community Nurse

Job purpose
The post holder will be responsible for providing holistic care and support to children and young people (CYP) who have life-limiting conditions and their families as a key member of the community team. This care is provided in the family’s own home, in the community, and support into the hospices and acute settings.
The post holder will be the first point of contact for families following acceptance to the service and the nurse is responsible for undertaking a holistic assessment of the child and family’s needs. The post holder will provide high quality palliative care to CYPs under the care of Shooting Star Children’s Hospices, using their skills and expertise in a diverse range of situations, autonomously and as a member of the community team. The post holder will have excellent communication skills, be a sound educator, and work proficiently.
They will work closely with the SSCH in house team, family support team and symptom management services to ensure a seamless and comprehensive service to CYP and their families.
SSCH care is provided 24 hours a day, 365 days a year, the post holder will be required to work flexibly to meet the needs of planned and emergency care, including on calls overnight and weekends in addition to contracted hours.
Key tasks and Responsibilities
• Providing professional clinical leadership in the provision of palliative care across care settings for CYP under the care of SSCH.
• To work in partnership with the SSCH care teams and wider MDT, demonstrating excellent communication skills to ensure high quality seamless care.
• To ensure that holistic care is family centred, research based, and meets with all statutory requirements and adheres to clinical governance initiatives and best practice.
• Provide support to CYPs and their families where required through community visits to hospital, home or hospice alongside relevant external professionals from statutory services.
• Maintain an awareness of the multi-cultural nature of the SSCH catchment area, adapting care culturally as required.
• To ensure all documentation is maintained in accordance with SSCH policies, guidelines and standards.
• To act in accordance with the SSCH and statutory Safeguarding Children Policy and Procedure.
Clinical
• To be actively involved with the on-going development of the SSCH care service, in particular the community team, contributing to development through research, audit and quality improvement.
• Provide clinical support and continuity of care to CYPs with palliative needs.
• Work alongside specialist symptom management teams to assess symptoms and provide clinical feedback to form part of decision making under the lead of the specialist services.
• Ability to follow an agreed symptom management plan and liaise with teams to offer high level care in the home as needed.
• Working in partnership with all members of the MDT, encouraging and promoting effective team working from all disciplines within SSCH.
• Attend professional meetings as a representative for SSCH and provide proactive input to ensure high level care for CYPs and families.
• Coordinating transition to adult services when appropriate in conjunction with the transition coordinator.
• Ensure written communication and correspondence is used to communicate with external professionals and services.
• Form part of the community on call service in conjunction with statutory services to endeavour to offer a responsive 24/7 service in end of life care.
Quality / Audit / Research
• Enhancing the interface between the community team and user involvement into the development of the service.
• To share professional expertise with Care Team colleagues, providing support and training where appropriate.
• To work to the SSCH medicine policy and adhere to the NMC guidelines at all times when giving medication.
• Participating in audits with the clinical governance facilitator to maintain and improve clinical care.
• In collaboration with the team, producing and reviewing operational policies and procedures specific to the team.
• Attending the Morbidity and Mortality meeting and having a proactive input.
• To take responsibility for your own lone worker safety, support the team and comply with SSCH policy/practices.

Leadership

The opportunity
We are seeking to recruit a Suicide Prevention Therapist to support men in suicidal crisis at our centre in Liverpool. We are looking for an experienced mental health professional or therapist with demonstrable interest in suicide prevention. We will work with you and train you to develop your therapeutic skills to offer our collaborative semi-structured psychologically informed intervention. You will support the centre user and their supporter(s) to recover from the suicidal crisis and maintain their safety. You will report to a Senior Suicide Prevention Therapist.
This appointment is part of our growth as a charity. The successful candidate will join an experienced supportive team and will gradually build a caseload supporting four men in suicidal crisis per day to meet the growth of referral partnerships and increased awareness raising in Liverpool.
The Charity
James’ Place exists to stop men dying by suicide. We opened our first centre in 2018 in Liverpool, the first of its kind in the UK, in 2022 we launched our second centre in London, and this year we opened our third centre in Newcastle. To date, we have supported over 1500 men and delivered over 7000 sessions of our life-saving intervention. James’ Place is a therapy centre for men who are experiencing a suicidal crisis, and it was set up to make the experience of finding help as easy as possible. It offers men a brief, intensive, therapeutic intervention in a safe environment. Men who walk through the door at James’ Place will be in a space where they feel valued and respected.
Building on the success of our centres in Liverpool, London, and Newcastle, we are now expanding our model to reach more men in suicidal crisis. Our aim is to have five centres open across England by 2026.

At Bluebell Wood Children’s Hospice we care for children and young people whose lives are just too short. Our aim is to reach every family that needs our help and to offer them the very best choice of care. Did you know that every year we need to raise £5m to keep our doors open and only around 20 percent of our income comes from government sources? Our services include short overnight stays and community breaks at home or in the hospice. You will work with women living in refuge within 7 Pan London boroughs, to devise a support plan that meets their immediate practical and emotional needs and start the process of recovery, linking women into more specialist support services where required. The Multiple Disadvantage Worker will undertake risk assessments, safety plans and identify support needs of women such as accessing benefits, budgeting, and explore re settlement pathways from the refuge. You will promote social inclusion, encourage independence and development of personal resilience. You will build up support network for women, promoting and enabling engagement with other services.

Relevant experience and direct work within specialist support services to women/families with multiple needs. Case work experience, providing proactive and emotional support to women who have experienced domestic abuse and provide the support to enable them to recover and rebuild their lives. The National Paediatric Diabetes Audit (NPDA) Project Coordinator is a crucial role for the College in which you will support the delivery of a high-quality national clinical audit along with supporting improvements in clinical care for children and young people with diabetes, and their families.
As Audit Project Coordinator, you will coordinate the collection of high-quality audit data, and be responsible for coordinating sub-projects and spotlight audits.
Acting as the first point of contact for audit process and data enquiries received from hospital units, regional network leads and other stakeholder organisations, you will take responsibility for responding to each enquiry using your detailed knowledge of the audit methodology.
You will also respond to queries relating to the NPDA methodology and support paediatric services within NHS Health Boards, Trusts and Networks to understand and use NPDA results to inform quality improvement activities, as well as monitoring NPDA participation rates to define, execute and promote improvements to data submission processes.
Reporting to the NPDA Project Manager, you will conduct testing and quality assurance of audit software systems, liaising with software developers to evaluate, improve and update online data submission and reporting processes, whilst preparing updates to project webpages, user guidance and promotional materials for audit submission and participation. In addition, you will coordinate project webinars, workshops and national events for the NPDA and represent the NPDA at conferences to help promote NPDA activities.
You will also prepare and disseminate audit outputs and publications for different audiences, including audit commissioners and funders, children, young people and families, and NHS services participating in the audit.
Degree qualified or with equivalent experience in a mathematical or numerate discipline, you should have proven experience of producing high-quality written reports and promotional information, suitable for a range of audiences.
With strong critical thinking and problem-solving skills, a high level of numeracy and the ability to interrogate large, complex datasets in Excel or statistical packages, you should have a background of working in a health-related or research setting.
Your project management experience along with your proven organisational skills and your ability to adopt an orderly and precise approach to work should ensure you are able to take personal responsibility for your own projects.
Demonstrable experience of engaging and communicating with stakeholders effectively, along with a proven understanding of information governance, data protection and confidentiality processes are essential.
