The Counselling Outreach Worker is key to SEL Mind's exciting new counselling service for Black residents of Southwark, delivered as part of the wider Southwark Wellbeing Hub. This part-time role will shape the Wellbeing Hub's provision to Black communities, expanding equitable access to culturally-appropriate support.
As Outreach Worker, you will work closely in partnership with community organisations to ensure relevant, de-colonial approaches, including maintaining a delivery partnership with Black Psychotherapy. The main working site is a community centre in SE5, and the community outreach work involves regular travel across the borough. You will play a vital role in building community trust, ensuring that we are creating routes into the service and continually seeking and implementing feedback.

Job Purpose
This role offers an opportunity to play a vital part in sustaining our Helpline service, which sits at the heart of the support we provide to people living with PSP & CBD. As Helpline Care Navigator (HCN), you will act as the first point of contact for individuals, families and health professionals across the West and South West.

The post-holder will play an important part in creating policy impact to promote the rights of children in the areas of immigration and education policy. Working with the Head of Policy and Practice Change, the Policy Managers and teams across Coram, the key objective of the role is to deliver policy work that has impact for children and centres the views and experiences of children and young people. We are part of One Bexley; a partnership of 8 local charities who have come together to support local people with care and support needs in new and creative ways. Working as a Trusted Assessor based in Bexley Mencap you will enable people to find the right support services that meet their needs and fulfil their personal goals, supporting them to maintain their independence for as long as possible and helping them to plan ahead. You will do this by building trusting relationships with individuals and their families, identifying the strengths that they have as well as their support needs and linking people with existing services and networks in their local neighbourhood.
About the role
The Trusted Assessor supports adults with care and support needs through strengths-based assessment, support planning and review. You will work alongside adults with a learning disability and neurodiverse people to understand what is important to them, identify their strengths, assess eligible needs and develop personalised support plans that promote independence, wellbeing and positive risk taking. You will also help people understand Care Act processes, personal budgets and Direct Payments where appropriate, while working closely with health, social care and voluntary sector partners.
About You

You will ideally have experience of assessment, support planning or care coordination and be confident building trusting relationships with people from a wide range of backgrounds. We are seeking a motivated and organised candidate with experience in social care, support planning or a related advice-based role. You will be someone who understands person-centred support and believes in helping people have real choice and control in their lives. We are looking for someone who genuinely cares about improving outcomes for people with care and support needs.
You will need to have excellent interpersonal and communication skills to build trusted relationships with residents. You will also need the confidence to explain complex information clearly, support people to understand their responsibilities, and manage a varied caseload effectively.
You will need to be approachable, solutions-focused and able to work independently. Strong organisational skills and good IT skills are essential to ensure accurate record keeping and effective case management.

About the Role
The key objective of this role is to support Trustees and the Senior Leadership Team with strategic development and income growth, by identifying opportunities, presenting business cases, leading on tendering activity, and project mobilisation. Building strong relationships with external commissioners and funders is also key to this post.

About the role:
Support individuals through the transition into adult integrated services
Support individuals through the transition in to paid employment and/or relevant training, education or occupation/diversionary activities that can best support there ongoing needs
Deliver a range of treatment interventions to the clients utilising a child and young person centric approach guided by the 10 key policy principles of working with children and young people. This may include both individual and group based client work.
