• Act as first point of contact for any requests for information and support from the public or professionals (drop-in, appointment, phone, email), triaging more complex requests appropriately.

• Manage professional and self-referrals to the service and triage appropriately, including to internal colleagues and services and partner agencies, ensuring people referred receive timely and coordinated, information and support.

• Meet with or provide digital or telephone support to people using the service (patients/clients, carers, relatives, professionals), providing high quality information and support within defined level of competence in relation to cancer. This will include assessment of service users’ information and support needs (Holistic Needs Assessment/Cancer Care Review), provision of written or verbal information, supportive listening and action planning to facilitate self-management.

• Act as single point of contact for any on-going support services liaising with the individual being supported and any professionals and agencies involved in their care.

• Work proactivity with primary and secondary care and voluntary sector partners to promote Weston Park Cancer Charity and build awareness of the support available for people living with and beyond cancer

Family Support & Group Coordinator

About the role

As Family Support and Group Coordinator, you will be responsible for:

• Ensuring allocated caseload of families receive the most appropriate form of support, monitoring and reviewing from referral to completion
• Implementing good safeguarding practice in all areas of work, promoting children’s welfare at all times
• Contributing to the recruitment and ongoing training, and providing regular support and supervision of home-visiting volunteers
• Planning and running Home-Start Elmbridge group/s as required
• Ensuring effective communication and working in partnership with other professionals

We’re looking for someone with:

• Proven experience of supporting parents and young children in a paid or voluntary capacity
• A solid understanding of safeguarding practices and experience working with vulnerable families
• Knowledge of current legislation and policies relating to children and families
• Knowledge of other agencies providing services for children and families
• Ability to supervise and manage volunteers and to work as part of a team
• Excellent written and verbal communication skills and relationship building abilities

The Role

We’re looking for a curious, compassionate and committed individual with excellent communication and organisational skills to join the Stroke Recovery Service in the Stockport area. This is an exciting opportunity to work directly with stroke survivors and their families, supporting them in rebuilding their lives after stroke.

The service operates in fast-paced, person-centred environment, where Coordinators identify and respond to the evolving needs of stroke survivors and their carers. This includes delivering structured reviews at six months post-stroke, supporting stroke survivors in their ongoing recovery and helping them achieve their personal goals.

Key responsibilities will include:

• Work as part of a team to plan and deliver engaging peer support and communication groups
• Attend multi-disciplinary meetings with the local stroke therapy team, contributing to joined-up care
• Recruit, train and manage volunteers to enhance service delivery (experience in volunteer management is an advantage)
• Support stroke survivors and their carers from hospital discharge into the community, offering personalised advice and guidance
• Provide resources for stroke survivors to make informed lifestyle changes to reduce the risk of further strokes
• Deliver Stroke Reviews at six months post-stroke

Wolfram Syndrome UK (WSUK)

WSUK is a small national charity based in West Sussex that supports children, young people and adults affected by Wolfram Syndrome (WS) and their parents/wider families. WS is an ultra-rare genetic disorder which causes a complex range of symptoms, including diabetes mellitus, vision problems, renal problems, deafness, and neurological problems. WSUK Adult Support Co-ordinator Role

WSUK is seeking a part-time adult support co-ordinator (2.5 days per week, working over 4-5 days, 0.5 FTE, £14.1k-£15.6k per annum), based in the Midlands area. This home-based role will provide support to WS affected adults and their families to help improve the quality of their lives. Through liaising with professionals (including the WS expert clinical team at Queen Elizabeth Hospital (QEH), Birmingham), external or advocacy organisations, this role will help facilitate access to the services to which WS affected adults and their families are entitled. The adult support co-ordinator will help adults and their families to increase their confidence and independence in living with their condition. This role will also act as an important point of contact for WS affected adults, providing advice and support in confidence.

Skills and experience

Applicants should have several years’ experience in a similar position, working with people affected by sensory loss, long-term medical / genetic condition, or disability. Applicants should also have a strong desire to help improve the lives of adults living with a long-term condition or disability and be confident in talking with people with a range of abilities and diverse cultural backgrounds.

Applicants should have experience in working effectively as part of a remote team, be confident in working independently and in managing conflicting priorities.

Some limited UK travel will be required (particularly to meet WS affected adults during their WS clinic visit at QEH, Birmingham (6 clinics per year) and the annual WS conference. An enhanced DBS check will be required.

We are seeking a proactive and organised Hub and Helpdesk Coordinator to oversee the day-to-day operation of the URC Resources Hub and deliver an efficient, user-focused helpdesk service.

In this role, you will be the first point of contact for enquiries, triaging and tracking queries through to resolution and escalating issues as needed. You will monitor Hub performance, flag content and technical issues, maintain service documentation and coordinate closely with the relevant teams and users to ensure a consistent and reliable service.

The role also involves preparing regular performance reports and escalating risks or development needs, helping to support continuous improvement of the Hub’s operations.

The successful candidate will demonstrate excellent judgement, strong risk awareness and confident use of IT tools. You will have outstanding communication and interpersonal skills, the ability to manage a complex workload within the tight deadlines and the confidence to act on your initiative when appropriate.
