About the role:
No one should leave hospital with nowhere safe to go.
As Homeless Pathway Coordinator, you’ll be a consistent and trusted presence for patients experiencing homelessness, helping them navigate what can feel like an overwhelming mix of health, housing and support systems. You’ll spend time with patients on wards and in A&E, listening to what matters to them, understanding the barriers they face and making sure their voice remains at the heart of every decision.
Day to day, you’ll manage a caseload, complete assessments, develop clear discharge and support plans and make referrals to housing, healthcare, social care, mental health and substance use services. You’ll follow up actions, keep plans moving and advocate firmly when someone is at risk of being discharged without the right accommodation or support in place.
You’ll work closely with the Homeless Pathway Lead, hospital teams, Newham Council and SHP’s wider Health Team, bringing professionals together and helping everyone stay focused on the person behind the case. You’ll also build links with community services, identify gaps in existing pathways and share learning that helps the hospital respond more effectively to people experiencing homelessness.
This is a role where strong relationships can change the course of someone’s recovery. You’ll join an experienced and supportive team, with opportunities to deepen your knowledge of inclusion health, homelessness and psychologically informed practice through training, reflective practice and shared learning.
About you:
- You have experience supporting people experiencing homelessness, multiple disadvantage or significant barriers to accessing services.
- You know how to build trust with people who may feel let down by services, keeping their voice and choices at the centre of your work.
- You can manage a caseload, assess what support is needed and turn complex situations into clear, practical next steps.
- You’re a confident communicator and advocate who can bring health, housing and support services together around an individual.
- You work calmly and compassionately, understand safeguarding and risk, and are keen to keep developing your knowledge through training and reflective practice.
About us:
We’re London’s leading homelessness charity – and we get things done.
In a city where hundreds are forced into homelessness every day, our work has never been more needed or more challenging. And we’re not shying away. We’re rolling up our sleeves to make change and helping over 10,000 Londoners every year. We prevent homelessness, provide safe places to live and give people the opportunity to rebuild their lives and transform their futures. And we never give up.
We’re here for Londoners wherever they are on their journey. We start with trust, building relationships that help people feel safe, supported, and ready to move forward. Every day, we put people first in everything we do, challenging injustice and barriers that keep people from the safety, stability and opportunity they deserve. We stand alongside people as they rebuild and shape a future that feels their own.
Joining Single Homeless Project means joining a team that’s bold, compassionate and determined to do better for the people we support and for each other. You’ll work alongside colleagues with lived experience, in a space that’s trans-inclusive, disability-friendly, and actively striving to be anti-oppressive and equitable.
We’re not perfect, but we’re real. We listen. We learn. And we push forward, together. Because this isn’t just a job. It’s a chance to lead with empathy, spark change, and help build a London where no one is left behind.

As a Project Worker, you’ll hold a caseload and build purposeful, trusting relationships with clients. - You understand that progress rarely happens through one service alone, and you’re ready to work closely with health, social care, housing and criminal justice partners to help people get the right support.
- You believe recovery looks different for everyone, and you’re motivated by helping people build confidence, independence and a more stable life at their own pace.

Are you an experienced support worker with a passion for helping young people achieve independence? Do you have experience supporting vulnerable young people with complex needs while providing guidance and support to colleagues?
If so, St Giles Trust is looking for a highly motivated Senior Caseworker to join our Lambeth Floating Support Service, helping young people secure and sustain safe accommodation while supporting the wider team to deliver outstanding person-centred support.
About St Giles Trust
An ambitious, well-established charity that helps people facing adversity to find jobs, homes and the right support they need. Central to our ethos is our belief that people with first-hand experience of successfully overcoming issues such as an offending background, homelessness, addictions and gang involvement, hold the key to positive change in others.
About the service
Our Lambeth Floating Support Service helps young people successfully move into independent living by providing practical, tailored support to secure and sustain accommodation. Working closely with Social Workers, Personal Advisors, housing providers and other partner agencies, the service supports young people to overcome barriers to independence, build resilience and achieve positive long-term outcomes.
About the role
As Senior Caseworker, you will manage a caseload of young people with a range of housing and support needs while acting as the day-to-day lead within the team. You will support colleagues with complex cases, provide guidance and mentoring, and work closely with the Team Manager to ensure the service delivers high-quality, outcome-focused support.
You will develop holistic support and risk management plans, work collaboratively with statutory and voluntary sector partners, and provide practical assistance to help young people establish and maintain successful tenancies. This may include supporting access to education, employment and training, benefits, debt advice, health services and community resources, as well as accompanying young people to appointments and advocating on their behalf.
What we are looking for
- Experience supporting young people with housing, tenancy sustainment and independent living.
- Experience supervising, mentoring or supporting staff and/or volunteers.
- Proven experience working with vulnerable young people or adults with multiple and complex needs.
- Experience developing support plans, carrying out risk assessments and providing advice and advocacy to help young people access accommodation and support services.
- Experience working effectively within multi-agency partnerships to achieve positive outcomes for young people.
- Knowledge of housing legislation affecting young people and an understanding of the barriers faced by people with lived experience.
- Strong communication, organisational and IT skills, including experience maintaining accurate case records.
- The ability to work flexibly across London, including occasional evenings and weekends, and to manage competing demands within a busy service.
In return, you can expect a competitive salary, generous leave allowance, staff pension, flexible working, a mentoring programme, an advice and counselling service, access to clinical supervision, season ticket loan and much more.
We are an equity and inclusion-confident employer. We welcome all applications, and we particularly encourage applications from people of the global majority (black, brown, multi-heritage) and those who identify as disabled, neuroexpansive, or neurodiverse, with any protected characteristics and/or social barriers or challenges. We value the empowering and informative impact that all lived experiences and diversity of thought can offer the organisation.
Please note St Giles will guarantee to interview all disabled applicants who meet the minimum criteria set out in the Job Description for the vacancy.
Please note this role requires an Enhanced DBS check with Adult Barred List.
Closing date: 3rd August 2026 at 9am.
