We have an exciting new opportunity for a Social Prescribing Link Worker to join our team in Merton.
The Merton Social Prescribing service is one of the most established and mature services in the UK and is celebrating its 7th anniversary this year. We are seeking an individual to join our successful service in Southwest London. This role will help to strengthen the links between Primary Care, voluntary and public services to improve patients’ health and wellbeing.
You will support patients within Primary Care in Merton, and you will support each patient to access appropriate services provided by voluntary and community organisations and other agencies. We expand the options available in Primary Care by creating a ‘managed’ way for referrals of patients with social, emotional and/or practical needs to a variety of local non-clinical services in the community. These include leisure, social activities, education, welfare, housing and employment advice and support.

About the service
Children and young people who are looked after don’t always have a trusted adult that they can rely upon for support, with many young people not having someone that they can talk to or trust.
Our independent visitor service provides looked after children and young people with an adult volunteer who spends time with them, offering support and friendship whilst being a positive role model to enable young people to build on their confidence and self-esteem, whilst helping learn new skills and raise their aspirations.
About the role
We are looking for a passionate and enthusiastic IV Coordinator, who will be responsible for the day to day running of the independent visitor service.
You will be responsible for managing and developing a pool of volunteers which includes the recruitment, training, and induction of new volunteers to become volunteer independent visitors, before matching them with a young person who has been referred to the service.
You will provide continuous support to volunteers to ensure that the service is being delivered to a high standard, and that children and young people are safeguarded at all times.
You will also be responsible for monitoring volunteer visits, budgets and ensuring that risk assessments are undertaken as required, whilst acting as a link between NYAS and the local authority by liaising with social workers, carers and other professionals.

The role:
Delivering targeted street work and outreach across the borough of Tower Hamlets. This involves engaging and building relationships of trust with young people at street level and in identified hotspot areas of risk where there is low to high anti-social behaviour, criminality and violence. Key Responsibilities
As a Building Safety Officer, you will oversee the delivery of property related safety servicing programmes ensuring that customers are safe, and that we meet and exceed our landlord statutory requirements.

The post of Group Work Facilitator will provide support to victims of domestic abuse and sexual violence within group work provision.
At FearFree we run various trauma-informed groups for victims such as: Hope2Recovery, an educational domestic abuse online support group; Rediscovering You, a face-to-face emotional wellbeing recovery group; Inspiring Families, a ‘whole family’ approach to support; and a Living Skills group supporting those having to flee domestic abuse.
We are looking for experienced and motivated staff to join us to help deliver the group work programmes we run around locations in Wiltshire. Groups run in the morning, afternoon and evening. Training for the groups will be provided.
In addition to facilitating groups, staff will be asked to attend training events and meetings, including regular supervision.
This is a fixed term role until November 2025, with the potential to become permanent subject to funding.
Main Duties and Responsibilities
- To facilitate group work provisions across the Wiltshire Service.
- Completing pre and post group assessments and required paperwork, following the procedures in place.
- Write comprehensive case notes following groups about service user involvement, any concerns raised and any action required.
- Identify and take the appropriate action on any disclosures or safeguarding concerns raised within the group.
- To work cohesively with a second facilitator both virtually and in person in locations around Wiltshire.

Working in partnership across Berkshire Healthcare Foundation Trust, you will support the testing of new and integrated models of primary and community mental health care.
Community Connectors will support patients to access a wide range of community services and resources including social care, housing, family, debt and employment counselling that support maintaining good health and wellbeing.
Key Responsibilities:
- Work jointly with the Mental Health Practitioner in conducting assessments, including risk assessment
- Work with patients to support them in identifying their socially determined needs and goals, provide self- management tools and facilitate the development of personal support plans
- Provide a range of motivational and structured psycho-social interventions
- Promote independence through an enabling asset-based approach that draws on individuals’ strengths, preferences and ‘natural’ support networks
- Assist with community resource mapping exercises and maintain a database of community resources; map where there are gaps in provision across the PCN and wider geography and work with other VCS organisations to develop resources where most needed
- Promote and support (where necessary) advised follow up actions from physical health checks for people with SMI
- Work with the service to identify opportunities to expand provision particularly in local communities where there is a lack of service provision identified
- Broker and establish new partnerships between public and voluntary sector agencies to enhance service delivery and access to services
- Develop and sustain professional relationships with service users, partner agencies and appropriate external agencies

The Project Manager will be a key member of the Charity and will play a significant part in developing and delivering the Life Chances Programme to our beneficiaries. The post holder will work closely with the CEO and is able to draw on support from the Admin Support Officer.
The Project Manager will support the success of the Charity’s core operations, the development and delivery of the Life Chances Programme, the lifecycle of projects and financial management from securing funding to reporting impact by ensuring the effective development and management of project opportunities, proposals, funding and finances, planning, delivery, evaluation and reporting.
Main duties and responsibilities
Project Management
· Develop, plan, manage, evaluate and report on diverse and inter-related externally funded and non-funded projects.
· Develop, monitor, maintain and deliver effective project management plans, liaising with project partners to ensure the completion of tasks, workstreams and overall project outputs, outcomes and aims.
· Liaise with internal departments to ensure projects are delivered in compliance with Charity procedures.
· Deliver effective assessment and processing of community grant applications, including identification of need, due diligence, assessment and reporting for decisions on grant awards.
Funding
· Liaise and develop positive relationships with Charity stakeholders and project funders.
· Fulfil funders’ monitoring and reporting requirements.
· Support the team in planning future project opportunities by contributing to the development of grant funding bids.
Finance
· Support effective financial management, decision making and value for money by developing, monitoring and reporting on project and core operating budgets.
· Maintain accurate financial records for all projects and the Charity’s core operations, including via the Charity’s internal CRM.
Other

PERSON SPECIFICATION
EXPERIENCE
· Ideal candidate will hold possess a Project Management Practitioner level qualification (e.g. APM, PMQ, PRINCE 2) or equivalent experience.
· The post holder will have experience in managing budgets, be skilled with Microsoft Office packages and be willing to learn new IT tools.
· A successful track record of delivery results within fundraising, managing programmes & partnership working or similar role for a minimum of two years.

Job Summary
Please note this is a part time role working 21 hours per week with a pro rata salary of £15,450 per year (£25,750 per year full time equivalent).
The role will involve the provision of youth engagement activities to a diverse range of LGBTQ+ young people aged 16-25 who are homeless, at risk of homelessness or living in a hostile environment. Our focus is in maintaining our ability to provide the highest service possible, giving our young people the opportunities to access peer support, community engagement and life skills. Overall, we aim to support LGBTQ+ young people into safe accommodation and work or education and prevent them from becoming homeless again in the future.
Key Tasks:
- Work as part of the Youth Engagement team to engage LGBTQ+ youth in a programme of meaningful activities designed to promote wellbeing and support young people to meet their personal goals.
- Ensure young people are meaningfully engaged in the co-creation of akt’s services and projects.
- With the support of the Youth Engagement Manager develop, manage, and grow akt’s LGBTQ+ youth work programme in Manchester.
- Facilitate group work and peer support sessions and complete risk assessments.
- Support the Trans Pathway group.
- Develop and grow our programme of LGBTQ+ national and regional youth events.
- Work with young people and colleagues to explore the potential for national/regional trans youth events and activities.

We have a large team of clinical staff who work across community, in-patient and out-patient services delivering individualised, responsive and holistic support to patients and their families/carers.
We’re looking for motivated, enthusiastic and forward thinking registered staff nurses to work within our two In-patient wards that have 17 beds each for patients. You’ll have experience of working in end-of-life care within an acute or palliative care setting. We will also consider newly qualified nurses who want to develop a career in palliative care. The successful candidates will be keen team players with good interpersonal and communication skills who enjoy working in a challenging environment whilst displaying a caring attitude at all times.
You will need:
- To be a Registered General Nurse and hold a Intra Venous administration Accreditation

- Experience of working in end of life care within an acute or Palliative Care setting.
