NHS England have awarded funding for NHS, Local Authorities, and the voluntary sector to increase their capacity for joint working to meet multiple needs of communities.

Working in partnership across Berkshire Healthcare Foundation Trust, you will support the testing of new and integrated models of primary and community mental health care.

Community Connectors will support patients to access a wide range of community services and resources including social care, housing, family, debt and employment counselling that support maintaining good health and wellbeing.

Key Responsibilities:

• Work jointly with the Mental Health Practitioner in conducting assessments, including risk assessment
• Work with patients to support them in identifying their socially determined needs and goals, provide self- management tools and facilitate the development of personal support plans
• Provide a range of motivational and structured psycho-social interventions
• Promote independence through an enabling asset-based approach that draws on individuals’ strengths, preferences and ‘natural’ support networks
• Assist with community resource mapping exercises and maintain a database of community resources; map where there are gaps in provision across the PCN and wider geography and work with other VCS organisations to develop resources where most needed
• Promote and support (where necessary) advised follow up actions from physical health checks for people with SMI
• Work with the service to identify opportunities to expand provision particularly in local communities where there is a lack of service provision identified
• Broker and establish new partnerships between public and voluntary sector agencies to enhance service delivery and access to services
• Develop and sustain professional relationships with service users, partner agencies and appropriate external agencies

About this key role
As our Children and Families Caseworker, you will support individuals who are affected by domestic violence, C.S.E, Mental health, grooming and exploitation, and gang and serious youth violence, all of which will include working with both victims and perpetrators.  You will carry and review thorough risk management plans with young people and their family, ensuring each family member has an age-appropriate safety plan, and provide a client-led support service that responds to each individual’s needs, including supporting with housing, finances, schooling, parenting, health, ETE and criminal justice. We will rely on you to deliver the service in a timely and targeted manner and to close cases effectively and positively, identifying a survival plan for the client that can be used for ongoing support and agencies that can be used if serious problems develop in the future.  Providing social support to children and family members, including identification of need, liaison and referral to specialist agencies, is also a key element of this role.

• To have lived experience similar to that of the client group or to have worked on an offender-led/ social care service with a commitment to utilising a peer-led approach
• Proven record of working with and supporting volunteers in a community setting
• The ability to recruit, engage and keep the target client group motivated

Working with two highly experienced Programme Coordinators, you will help to grow our existing Youth Development Team activities, currently present in Hampton, and continue the development of our newly launched Aspire project.

Romsey Mill's Youth Development Team (YDT) works with disadvantaged, at-risk and vulnerable young people (aged 10 - 21 years) across Cambridgeshire and Peterborough. YDT are passionate about supporting young people to overcome challenges, live fulfilling lives and inspire change in the world. We currently engage around 1200 young people each year. Aspire is the name of Romsey Mill’s project that supports autistic children and young people. • To assist with training events and activities held at Enfield Carers Centre including liaising with sessional       workers/trainers/speakers preparing room set-up, refreshments and set up of Zoom 
• To lead on matters relating to office management including supporting the CEO and ensuring the office adheres to health and safety compliance issues
• To assist in maintaining ECC’s electronic database, website and email system e.g. ensuring events are uploaded and attendance data kept up to date
• To act in accordance with all policies and procedures laid down by Enfield Carers Centre.

The ideal candidate must have:

• • At least 2 years previous administrative experience 
• • Exceptional organisational skills
• • Intermediate to advanced IT skills
• • Excellent interpersonal skills 
• • The ability and experience of multitasking 
• • The ability to stay calm under pressure
• • The aptitude to pay high attention to detail
