About Us
Bridges Outcomes Partnerships (BOP) is a not-for-profit social enterprise that works alongside Government, community groups and specialist Delivery partners to design and deliver vital services that support people to improve their lives, in areas ranging from housing and employment to education and health & wellbeing.
The role of BOP spans project development (working with Government and others to design and launch the service), project management (coordinating the delivery phase, managing performance, and liaising with the various stakeholders) and project finance (funding the project until it starts to earn outcomes payments). Our own funding comes from a group of pioneering social investors, including The Office for Civil Society, Big Society Capital, Pilotlight, Trust for London, who, like us, are motivated by improving lives and changing the system for the better.
About the role
In 2018 Bridges Outcomes Partnerships co-designed the Single Homelessness Prevention service with Brent Council following the Homelessness Reduction Act (2018). The service provides early intervention to prevent or relieve homelessness for single people who previously would not have received support, aiming to prevent the mental trauma associated with homelessness and stop individuals reaching crisis point.
Our services continue to evolve into models that combine homelessness prevention, refugee support, and family stability interventions to improve life chances for vulnerable people and reduce the long-term impacts of housing instability and child poverty.
The Referral & Assessment Officer is a key role within the London Partnership, it is the first point of contact for people who have experienced trauma or are struggling to find a home.
As part of the role, you will be responsible for the following activities within this element of the service

Receive and process referrals from Brent Council, acting as the central point of access for the service.

Review each referral against the service eligibility criteria, completing an initial triage assessment to determine the most appropriate support pathway.

Contact clients where required to gather any outstanding information or supporting documentation needed to progress their referral.

Develop and maintain positive working relationships with Brent Council, delivery partners and other stakeholders to support effective referral pathways and timely allocation of referrals.

Liaise regularly with Brent Council and delivery partners to resolve referral queries, gather outstanding information and ensure referrals progress efficiently

Identify, record and escalate any immediate safeguarding concerns through the appropriate safeguarding procedures.

Record client support needs, risks and relevant information on the case management system to ensure delivery partners have the information required to provide safe and effective support.

Maintain timely, accurate and comprehensive records on the case management system in line with service standards

Work closely with the Programme Manager to monitor referral activity, identify issues or opportunities, and recommend improvements where appropriate.

Ensure the service is accessible and inclusive, recognising individual communication, language, cultural and accessibility needs.

Produce regular reports on referral activity, triage performance and emerging trends for the Programme Manager and commissioners.
This is a hybrid role. The postholder will be expected to work across a combination of home working, Bridges offices and partner locations. This will include regular attendance at Turning Point in Brent to work alongside Brent Council and partner organisations, supporting the effective management of referrals and the development of local referral pathways.

Job Requirements
Our work is place-based, and people focused. To be responsive to the community and to be able to support Shoreditch Trust activities, we do not offer remote working contracts. JOB PURPOSE
This is a pivotal role within The Passage’s Fundraising Team, created to help support the transformation of our supporter data systems and ensure we become a more data informed, insight driven organisation and fundraising team. You will be the main contact for the fundraising CRM working with the fundraising team and other teams within The Passage such as finance and IT to ensure the smooth running of all systems and processes.
As the dedicated data and CRM Manager for Fundraising and Communications, you will support colleagues with reporting, analysis, campaign set-up and selections, and data strategy. Your work will ensure our data is accurate, accessible, and GDPR-compliant, enabling smarter decision-making, improved supporter journeys, and to help grow income. You will also be working with the Marketing and Communications team to implement email soft opt-in for our supporters. This is a unique opportunity to shape how we use data to engage, retain, and grow our supporter base at a time of exciting change.
MAIN DUTIES
1. Fundraising CRM Project Leadership
-
Manage and maintain the current Salesforce system, ensuring it remains functional, accurate, and fit for the purposes of stewarding donors and maintaining accurate records of donations

Act as the day-to-day lead for the CRM system within the Fundraising and Communications team, ensuring it meets team needs and supports strategic goals.

Document workflows, integrations and processes to ensure the long-term sustainability and usability of the system for vital processes such as donation processing and gift aid

Work with IT and the fundraising team to evaluate onboard 3rd party platforms to support fundraising initiatives. Ensure any changes are approved with senior stakeholders and documented.

Be a custodian of GDPR within the Fundraising team and be the main point of contact with The Passage’s Data protection lead and ensure all policies and procedures are up to date and in line with current fundraising regulations.
2.CRM and Data Management

Work collaboratively with the IT team and procurement team as the fundraising representative to recommend improvements to the current CRM and any other related projects.

Maintain and improve the quality of supporter data through regular cleansing, deduplication, and tagging to ensure accuracy and GDPR compliance.

Ensure ongoing system performance and data integrity by embedding data hygiene routines and best practices.

Work with the Finance team to support integration between the CRM and finance systems for seamless donation and income reporting.

Working with data flows and setting up integrations, ensuring the system runs smoothly on an ongoing basis, making updates, troubleshooting issues whilst maintaining data consistency.

Work on system migrations and leading on maintaining proper process structures and documentation during those transitions. Keep track of installed packages, security keys, and their passwords and related administrative processes.

Be responsible for all data imports from our 3rd party platforms, ensure that data is uploaded in line with regular upload schedule in an accurate manner.

3. Data Selections, Segmentation and Campaign Support

Collaborate with the Fundraising team to perform targeted data selections for appeals, newsletters, stewardship journeys, and events.

4. Reporting and Insight

Overview
The role will provide high-quality administrative, systems, and research support to the Quality Team, enabling consistent quality assurance across the grant-making cycle. Key Responsibilities
Salesforce administration and systems support
- Administer the Quality/Projects components of Salesforce (and associated tools), including user support, data standards, fields/picklists, basic configuration tasks within delegated permissions, and documentation of processes.
- Maintain data quality rules and routines (validation, required fields, definitions, and guidance) and run regular data quality checks, working with colleagues to resolve issues.

• Sub-contracted to TOGS Mind, Family Action is a VCSE delivery partner for the 111 press 2 / Mental Health Urgent Triage team located within the Northwest Ambulance Service call centres.
Listening Lounge Services in Bolton and Wigan operate between the hours of 3pm and 10pm 7 days a week. There is an expectation that the post holder will work to a flexible rota to meet the needs of the service and to provide onsite management support. Typically, this will include at least 2 weekend days per month from 2pm to 10pm on site – one in Bolton and one in Wigan, and two evenings per month during the week from 2pm to 10pm. These to be arranged in collaboration with the Service Managers from Bolton Listening Lounge and Wigan Crisis Space.
The Operational Manager will also be a member of the North Operational Management team that reports to the Deputy Director of Services & Innovation.
