Shine supports a community of over 15,000 members living with spina bifida and/or hydrocephalus, including 5,000 children and young people (0–25).

You will be delivering high-quality support and creating opportunities for children and young people living with spina bifida and/or hydrocephalus and their families/carers. Supporting Shine members to lead healthy, independent, and fulfilling lives by improving condition management and fostering connections within the Shine community. 

This role will focus on children and young people (0–25), you will be primarily working within the Children, Young People and Families team. However, there will be occasions when you will work across age groups to ensure the best outcomes for our members. 

The role is home-based but you will be required to attend regular clinics in London including GOSH (Great Ormond Street Hospital). Other travel across Southern England including Bristol, Devon and Hampshire may be required. There will be occasional travel required across wider areas and nationally including attendance at events, conferences and meetings at our head office in Peterborough.

About the Role

Headway East London is looking for a compassionate and motivated Community Support Worker to support adults living with brain injury across our 13 London borough catchment area. You will support members in their own homes and local communities. This is a varied, people-focused role where you will work one to one in the community, helping individuals live with greater independence, confidence and choice.

Principal Duties and Responsibilities

1. Conduct support sessions to a regular caseload of members in the community as a lone worker with remote office assistance.
2. You will work to maximise and maintain their autonomy and independence in their home environment or within the local community.
3. You will work collaboratively with the member to plan, organise and implement daily activities by developing daily and weekly structure and wellbeing plans.
4. You will be responsible for being the first responder, with remote manager support, in the event of any incidents, safeguarding issues or behaviour that challenges during support sessions.
5. Accompany and support members to appointments with health and statutory care providers and provide accurate notes for future reference.
6. Accompany and support members to access social, leisure, educational and vocational activities in the community.
7. Maintain a maximum income for members through completing benefit and grant entitlement and assist with financial budgeting.
8. Provide appropriate and relevant advice, information and advocacy to members and their families. Where appropriate liaise with other professionals and statutory and non-statutory organisations to make referrals or signpost to appropriate support services.
9. Maintain accurate and factual monitoring notes on our online database and report any incidents.
10. Conduct and monitor specific therapy / support / rehabilitation and risk management plans in accordance with case managers, therapists and other cprofessionals as required.

• Make referrals to hostels and supported housing
• Assist clients to access grants and with benefit claims

The role of a Children and Family Worker will support children and young people under the age of 18 on a one-to-one basis in a confidential space. The role will deliver a wide range of interventions/ education though activities, such as games, arts and crafts and emotional wellbeing resources. The role will utilise a young person-centred approach, empowering them to explore situations which they feel they have little control over to support them to lead a safe, happy, and healthy life.

The role will work in partnership and liaise with other young people services throughout Buckinghamshire, to provide holistic support to young people accessing the service. We need to record information about what we do, so enjoying a bit of admin work and attention to detail is important, but most of the time you’ll be enjoying working with young people from all backgrounds, making a difference to their lives. You will be required to work flexibly across operational sites as required so must hold a full UK driving license and have access to a car. Responsibilities

About the role: 

• To holistically support CYP affected by parental substance misuse, using a range of interventions including group work, 1:1 work, whole family interventions, safety planning and safeguarding working closely and proactively with the family unit to support positive and sustainable outcomes.
• Identify families in conjunction with other agencies, who will benefit from a Think Family / Whole family approach, negotiating participation and engagement with the programme of care.
• Identifying and responding effectively to potential safeguarding issues. 
• Reducing drug and alcohol related harm to young people and the wider community. 
• Working with young people to support positive, holistic outcomes in relation to their health and wellbeing, enabling them to lead safe, healthy, and purposeful lives: reducing risk and increasing resilience

About you: 

• Experience of working with young people and knowledge of the issues they face 
• Knowledge of safeguarding concerns in relation to children and young people and the Fraser Competence framework 
• Have an excellent understanding of drug and alcohol issues and experience of working within a related field

One of two Outreach and Project Workers pivotal to the delivery of an exciting new Women at Risk (WaR) project which will support women at risk of rough sleeping, homelessness and exploitation. Working across key areas of Enfield and Haringey you will, through a combination of nighttime outreach and daytime service provision, be critical to supporting a coordinated response across multiple agencies already engaged in this work.

Crisis Skylight Oxford is located in the Old Fire Station in the centre of the city. Our team provide a one-to-one specialist housing led service to help people find and secure affordable housing. About the role 

In this role you will carry your own caseload and be supporting people who are at risk of homelessness or experiencing homelessness into both the private rented sector and social housing.

You will be part of a supportive and established team who have knowledge and experience of accessing the PRS sector, completing housing assessments and budgeting plans.

The key to success will be your ability, as part of the Crisis team, to make collaborative, consistent and persistent relationships with each person you support and have good partnership working skills

About you, to be successful in this role you will have;

1. Experience of working within a relevant sector (e.g. homelessness, mental health, drug and alcohol treatment) and have good knowledge of relevant housing, homelessness and welfare legislation

2. Excellent understanding of supporting individuals experiencing multiple disadvantages

About the Role

The primary role of a Family Support Worker is to provide effective, efficient, and supportive service to children and their mothers residing in refuge with Solace Women’s Aid. • Develop and deliver a range of trauma-informed services to children and young people (0-16 years) and their mothers within the refuge and when they are resettled into the community
