We’re looking for a compassionate and motivated Community Connector to join our GP Integrated Mental Health Service and Mental Health Integrated Community Service in Surrey Heath.

This is an exciting opportunity to work at the heart of local communities, supporting people with their mental health and wellbeing by helping them access the right support at the right time. Working alongside GPs, mental health practitioners, partner agencies and community organisations, you will provide timely assessments, brief psychosocial interventions and practical support that helps people build on their strengths, overcome barriers and connect with services, activities and networks that promote independence and recovery.

Key responsibilities:

To work jointly with the Mental Health Practitioner in conducting assessments, including risk assessment, and to work with patients to support them in identifying their socially determined needs and goals, provide self-management tools and facilitate the development of personal support plans

• To provide a range of motivational and structured psychosocial interventions
• To promote independence through an enabling asset-based approach that draws on individuals’ strengths, preferences and ‘natural’ support networks
• To establish effective working relationships with a range of agencies to facilitate a ‘joined up’ approach to support plans
• To liaise with, develop and maintain good relations with GPs, practice managers and other health practitioners across mental health and physical health pathways and wider support networks
• To provide continuity throughout recovery; engaging the patient with key services such as Safe Havens, accessing CMHRS, and providing support for those engaged with and leaving CMHRS, and linking into local wellbeing services and activities
• To co-ordinate and support patients to access a range of community services such as wellbeing services, housing providers, family and carer support services, debt and employer advisors where appropriate
• To assist with community resource mapping exercises and maintain a database of community resources; map where there are gaps in provision across the PCN and wider geography and work with other VCS organisations to develop resources where most needed
• To promote and support (where necessary) advised follow up actions from physical health checks for people with serious mental illness
• To support patients to engage with local peer support workers and volunteering services
• To support and supervise any peer support workers linked to PCN
• To work with the service to identify opportunities to expand provision particularly in local communities where there is a lack of service provision identified
• To broker and establish new partnerships between public and voluntary sector agencies to enhance service delivery and access to services
• To develop and sustain professional relationships with service users, partner agencies and appropriate external agencies
• To complete locally agreed quality outcome measures, with patients and to undertake regular reviews

About the role

As Fundraising Manager - Trusts & Foundations, you will play a crucial role in shaping and implementing our fundraising strategy. You will work closely with the Fundraising Manager - Philanthropy and Partnerships and the Chief Executive Officer to grow and diversify Action for Conservation’s income, taking responsibility for our trusts and foundations revenue stream. You will be the driving force behind our grant applications and reports, producing compelling fundraising content that drives donor engagement and maximises income opportunities. You will be able to distill and transform information from various sources about our projects and programmes, including impact data, into readable, persuasive narratives about our work tailored to donor interests and criteria. You will also manage the trusts and foundations pipeline, from researching new prospects and identifying the most appropriate approach for each funder to coordinating grant administration, monitoring and reporting. Through everything you do, you will maintain and build on existing funder relationships and proactively nurture new ones, enabling us to attract and retain funders year after year.

We are looking for a driven and passionate individual with a track record of securing one-off and/or multi-year five- to six-figure grants, who is comfortable managing multiple prospects, deadlines and priorities. To succeed in this role, you will be a skilled and persuasive written communicator with a keen eye for detail, excellent organisation and time-management skills and a positive, flexible and highly proactive approach to your work.

Key Responsibilities

• Contribute to the development of our fundraising strategy and annual fundraising plans in line with delivery targets, strategic goals and funding needs; monitor and report on fundraising performance.
• Grow and diversify income from trusts, foundations and statutory funding sources, taking the lead on identifying new opportunities, drafting compelling funding applications and managing the grant fundraising pipeline.

The Opportunity

This is an opportunity to take a leading role in an ambitious five-year programme working to address how structural racism and discrimination contribute to health across Bradford District. You will provide specialist leadership on structural racism, policy and system influence, working alongside communities and partners to turn lived and community insight into credible structural analysis, practical action and demonstrable system change. From early in the programme, you will build the relationships and routes into health, care and wider systems needed to ensure emerging evidence reaches the places where decisions are made and, creates opportunities to test and evaluate change that is translated into opportunities to test, evaluate and embed change.

You will also shape Bhip’s anti-racism and structural discrimination learning, strengthen community leadership and influence, and generate evidence and learning that can inform policy and practice in Bradford and contribute to the National Lottery Community Fund’s National Collective.

About You/ The Role

You will be an experienced and credible leader with significant expertise in anti-racism, structural discrimination, policy and system change, and an understanding of how these contribute to health inequities.

This is not a generic EDI, policy or anti-racism role. It is Bhip’s specialist lead for racism and structural discrimination, responsible for applying structural analysis to community insight and evidence, building structural literacy and anti-racist capability, and working with communities and system partners to translate that learning into accountable changes in policy, practice, commissioning, governance and decision-making that drive demonstrable system change and improvement.

You will be equally comfortable working alongside Global Majority communities and community leaders, white allies, VCSE organisations, practitioners, commissioners, senior system leaders and change makers. The Bradford Health Inequities Programme (Bhip)

The Bradford Health Inequities Programme (Bhip) is a five-year programme funded by the National Lottery Community Fund and delivered jointly in partnership by Race Equality Network (REN) and Happy Healthy You (HHU). As part of the National Lottery Community Fund’s National Collective, Bhip is one of nine health inequities programmes established nationally to address health inequities caused by structural racism and discrimination.

Working alongside communities, voluntary, community and social enterprise (VCSE) organisations, the NHS, public services and other partners across Bradford District, Bhip will co-design, test and evaluate community-led approaches that address health inequities driven by structural racism and discrimination, influence policy and practice, drive system change, and generate evidence and learning for Bradford while contributing to the National Lottery Community Fund’s National Collective.

As an innovative five-year programme, Bhip is expected to evolve through community engagement, co-design, learning and evaluation. We Are Survivors is a survivor-focused voluntary sector organisation that aims to create and facilitate safe spaces for male (including trans and non-binary individuals) survivors of sexual harms across Greater Manchester, providing access to quality-assured support. About you/ The Role

You will be an experienced, motivational and credible leader with a proven track record of successfully leading complex programmes, partnerships or services that improve health equity and deliver meaningful system change. You will relish an opportunity to establish and lead a new, fast-paced programme from the ground up. About the role

We’re looking for a motivated and committed policy manager to join our policy, evidence and influencing team. In this role, you’ll help deliver change for people affected by breast cancer by leading our policy work on early diagnosis (including breast screening and cancer waiting times) as well as our work on improving services for people at increased risk of breast cancer. This role manages a senior policy officer. 

You’ll use insight from patients and healthcare professionals, data and research to develop evidence-based policy positions. And you’ll build strong relationships with external stakeholders to enable you to influence to deliver policy change. You’ll work with colleagues across the team and wider organisation to take our priorities to supporters, MPs, decision-makers and the media.

About you

With experience of working in relevant policy roles to deliver significant change, you’ll be an excellent communicator, well-organised and able to work under pressure to tight deadlines.  You’ll be confident working with a wide range of people and have experience of supervising or managing people. You’ll also have an excellent understanding of the policy and influencing landscape that Breast Cancer Now works in.

This role is primarily based in our London office. Our hybrid working model allows you to work up to 3 days per week at home.

£46,000 to £49,500 per annum London based

Working closely with women with lived experience, colleagues across Beyond the Streets and our Senior Leadership Team, you will help create meaningful, safe and flexible opportunities for women to influence our services, research, training, policy, communications and organisational decision-making. This is a role with real scope to influence how our approach develops, while joining at a point where you will not be expected to build everything from scratch.

AllChild is a charity pioneering a new way of working with children and young people at the tipping point of need. Through our two-year, in-school Impact Programme, dedicated Link Workers build trusted relationships and bring together personalised support from families, schools and communities before problems escalate, helping children and young people progress socially, emotionally and academically and flourish in school and beyond.

We are seeking a Monitoring & Evaluation Manager to shape how we monitor performance, evaluate our two-year Impact Programme and use evidence to improve support for children and young people.

The Monitoring & Evaluation Manager will play a key role in strengthening AllChild’s approach to monitoring and evaluation, ensuring we generate robust evidence about programme implementation and outcomes and use it to improve our work. Reporting to the Head of Monitoring & Evaluation, they will oversee core M&E activity and support programme performance management, organisational decision-making and external reporting. They will work closely with Programme Design, Delivery, Systems and colleagues across AllChild.

We are looking for someone with strong analytical skills, excellent project management and communication, and a clear understanding of what robust, high-quality monitoring and evaluation requires. They will be confident working with complex data and solving problems, translating M&E requirements into effective systems and reporting tools, and communicating findings clearly and accurately, including what the evidence can and cannot tell us. They will line manage the M&E Officer and help build a strong M&E function that enables AllChild to understand, improve and demonstrate its impact.
