Shannon Trust are delighted to be working with His Majesty’s Prison & Probation Service (HMPPS) to provide peer-led learning programmes at HMP Rochester/HMP Cookham Wood and HMP Elmley. Working closely with the prison and their staff, people in prison, Shannon Trust volunteers and mentors, this post will lead the delivery and development of our prison-based reading and numeracy programme, maximising opportunities for people in prison to learn to read.  

Ideally you will have some experience of prison settings underpinned by the ability to build relationships and personal qualities that include resilience, determination and a problem-solving approach. 

This role is a fixed term role until 31st March 2027 with the opportunity to be made permanent subject to contract renewal.

The role will be prison-based. PCN Cancer Care Coordinator

Clifton & The Meadows PCN 8, Nottingham (Hybrid)
30–37 hours per week | Permanent

About the Role

We are excited to recruit a PCN Cancer Care Coordinator to support people living with cancer across the Clifton and The Meadows Primary Care Network in Nottingham.

This role is central to delivering a holistic, person‑centred cancer care coordination service, supporting patients beyond clinical treatment and ensuring their wider emotional, practical and social needs are met. Working closely with GP practices, Primary Care Network colleagues, and the voluntary and community sector, you will help improve experiences and outcomes for people affected by cancer.

You will work in direct partnership with Self Help UK’s Macmillan Beyond Diagnosis Service, carrying out Cancer Care Reviews and Holistic Needs Assessments (eHNA), developing care and support plans, and helping people to access the right support at the right time.

What You’ll Be Doing

• Supporting people newly diagnosed with cancer through Cancer Care Reviews and eHNAs
• Creating individualised care and support plans based on each person’s needs
• Acting as a single point of access for non‑clinical cancer support
• Referring and signposting patients to appropriate voluntary and community services
• Working closely with GP practices to ensure reviews are recorded on NHS systems
• Supporting work to increase cancer screening uptake, especially within targeted communities
• Building strong relationships across health, social care and the voluntary sector
• Ensuring safeguarding, confidentiality and data protection standards are upheld

You’ll work flexibly across GP practices, community settings, patients’ homes and the Self Help UK office in Nottingham, with some home working available.

About You

You will be someone who is compassionate, organised and confident working with people facing complex and emotive situations. You will bring:

• Experience in a people‑facing role within health, community or voluntary settings
• Experience supporting people with long‑term health conditions or vulnerabilities
• Strong communication, organisation and relationship‑building skills
• Confidence working both independently and as part of a multi‑disciplinary team
• Emotional resilience and a genuinely person‑centred approach

Experience of working with people affected by cancer, delivering holistic needs assessments, social prescribing or working across NHS–voluntary sector partnerships would be an advantage, but is not essential.

A full UK driving licence and access to a car is required for this role.

This role leads our work across Birmingham at an exciting moment. We are developing a participatory, community-centred approach to analysing the city's economy, identifying opportunities for change and coordinating alliances to act on them. This means bringing together mixed groups, including grassroots changemakers, researchers, funders and other partners, and facilitating processes that help people make sense of complex information together, find common ground and make decisions about collective priorities. The Senior Programme and Network Lead will develop and deliver initiatives that support a growing movement for economic justice across the city, with a particular focus on building support and engagement amongst grassroots changemakers and communities experiencing economic injustice. It will manage projects and resources, conduct programme development and delivery, oversee outreach and partnerships, changemaker recruitment and contribute to fundraising, ultimately playing a key role in shaping our regional impact. This position is crucial in coordinating our work across Birmingham and driving meaningful collaboration with local and national stakeholders, in particular working collaboratively with Economic Justice Brum, a long-standing initiative working on local economic systems change.

Villagers Connect Project – a great opportunity to join our team at Action in rural Sussex

Villagers Connect is a community development project which is working to sustain a strong and vibrant community in the villages of Colgate, Faygate and Rusper. It aims to build connections and relationships which increase confidence, independence and empowerment, particularly for the over 65’s.

We are seeking a part-time community development worker to work as part of a small team that is active in this rural location in the Horsham district. This role involves a good deal of community-based activity, engaging with older people and the wider community. This does therefore require someone who is understanding of older people and their interests, abilities and needs.

We champion an asset based approach to community development, which is about building on existing strengths and supporting local people to flourish; undertaking community led projects that create community cohesion and have lasting impact.

Key Objectives

· Support older people to access services and support locally that will meet their needs and improve their quality of life

· Improve the lived experiences for older people in these rural communities

· Build connections and relationships across and within the three parishes to reduce social isolation

· Develop new and support existing activities to enable a vibrant community

· Increase confidence, independence and empowerment amongst individuals, groups and community based organisations.

· Strengthen existing community assets, knowledge and skills in line with our asset based approach to the Villagers Connect Project

· Support community resilience by building community networks which are strong and sustainable into the future

Two years in, the project is well established and has gone from strength to strength. We are now seeking a new member of the team to help drive this project forward and enable this community to celebrate what can be achieved when people come together and support one another to age well and live full and active lives, despite the challenges of rural living.
