You will be part of the MYcommunity Linkworkers team. You’ll be responsible for:
Provide personalised brief intervention and social prescribing support to individuals, their families and carers.

Provide person-centred support to enable clients to identify their health and wellbeing needs and goals. Co-produce support plans with clients to discuss what’s important, where they would like to be in the future and how we can support them to achieve this.
Manage and prioritise a caseload to ensure clients' needs, priorities and urgent support are met.
MYcommunity Lambeth works in partnership with a large number of statutory and community services and organisations across the borough. You will link clients with appropriate services to meet their needs. It is vital to have a strong understanding of when it is necessary to refer someone back to health care professionals/ agencies when the client’s needs are beyond the scope of the MYcommunity team.

International Nepal Fellowship (INF) is a Christian health and development charity. Since 1952 they have had a huge impact serving the most disadvantaged people and communities in Nepal.
As Nepal’s longest serving international NGO they continue to have ambitious plans. It is an exciting time to join INF as they enter a bold new strategic phase for 2026 to 2031.
The most disadvantaged communities in Nepal face a lack of access to healthcare, often walking for days to their nearest health post, or being unable to afford even basic treatment.
Kopila, who was diagnosed with leprosy several years ago, is a patient at INF’s Green Pastures Hospital in Pokhara, Nepal.

We are looking for a Trusts Officer to join their small, passionate team. This is a fantastic opportunity to ensure INF UK’s key supporters feel valued and connected with the charity. The Role
To provide high-quality, person-centred support and housing management services to residents living within Causeway's Mental Health Needs Service. Building on the success of the 12-week MS Warrior programme, you'll help create opportunities for people to remain active, connected and supported long after the programme ends through local activities, online communities and volunteer-led peer support.
Working alongside volunteers, people living with MS, healthcare professionals and community partners, you'll create opportunities that encourage ongoing physical activity, wellbeing and peer support across Scotland.
A key part of the role will be recruiting, supporting and developing a network of MS Warrior Alumni Champions. Using their own lived experience, these volunteers will encourage others to build confidence, overcome barriers to physical activity and become part of a supportive, community-led network.
You'll also develop partnerships with leisure providers, community organisations and healthcare professionals, creating more inclusive opportunities for people living with MS to be active in their local communities.
This is an opportunity to shape an innovative model that has the potential to influence how community-based physical activity and peer support are delivered for people living with MS across Scotland.
No two days will be the same. One day you might be facilitating a community conversation with people living with MS, the next you'll be supporting volunteers, developing new partnerships or helping launch a local wellbeing activity.

This is a unique opportunity to play a pivotal leadership role in coordinating a community-led regeneration programme, ensuring local voices are at the heart of decision making and helping to create a lasting legacy for the area.
The successful candidate will provide visible place leadership, supporting the Neighbourhood Board to develop and deliver a 10-year Regeneration Plan and a 4-year Investment Plan. SIFA Fireside’s Support Centre, based in Digbeth, delivers a range of services to people experiencing homelessness or who are vulnerably housed. The Peer Support Worker Service (PSWS) is a team of individuals who work with rough sleepers across the city of Birmingham, aiming to facilitate connection, motivate and inspire, and provide a non-judgemental listening ear. The service supports individuals facing Multiple Exclusion Homelessness (MEH), empowering them to make informed choices about their lives.
The Peer Support Worker role includes street outreach across Birmingham, building trust and rapport with the rough sleeping community. Many individuals have had negative experiences with services, leading to a lack of trust and reluctance to engage. Some may also have limited experience of managing accommodation or navigating support services. The role therefore centres on developing supportive, persistent, and compassionate relationships.
Using a trauma-informed approach, the Peer Support Worker Service provides relational, collaborative, assertive, and personalised support to individuals facing complex and multiple disadvantages. At the heart of this work lies the relationship between the Peer Support Worker and the service user – characterised by non-judgemental support at the individual’s pace and by advocacy where needed.
Key Tasks
1. Support people through street outreach, building trust and rapport with rough sleepers in Birmingham.
2. Build and maintain trust with individuals facing Multiple Exclusion Homelessness through social and community-based activities.
3. Support individuals in their positive behavioural change journeys using trauma-informed, strengths-based approaches that build confidence and wellbeing.
4. Work collaboratively with other frontline services across the city, engaging in day-to-day liaison to ensure a multi-disciplinary response.
5. Assist in crisis intervention and provide support in challenging situations, connecting individuals with appropriate professional help when required.
6. Participate in training and development, contributing to discussions on multiple disadvantage, trauma-informed practice, and homelessness.
7. Prioritise safety for yourself, colleagues, and clients by completing thorough risk assessments and applying dynamic risk-assessment skills.
8. Participate in effective safeguarding practice, including identifying and responding to adult safeguarding concerns and self-neglect.
9. Recognise the emotional demands of the role and prioritise self-care to maintain personal wellbeing and effectiveness.
10. Engage in supervision, reflective practice, and wider wellbeing support opportunities.
11. Record and monitor client interactions and support sessions accurately to contribute to service impact reporting.
12. Undertake any other duties as required and appropriate to the post.
Working conditions:
· Working hours are between 8:30 am and 4:00 pm.
· Travel expenses will be paid for all work-related journeys.
· There may be occasions when flexible working outside of core hours is required to meet client needs.

Quartet Community Foundation is looking for an experienced, collaborative and forward-thinking Head of Programmes to lead our grant-making and programmes work across Bristol, Bath & North-East Somerset, North Somerset and South Gloucestershire.
This is an exciting time to join Quartet. We recently launched our new three-year strategy 2026-9, containing ambitious plans to increase our impact through grant giving, strengthen our understanding of local communities and raise the profile of place-based giving. The strategy outlines how we aim to diversify and strengthen our donor base, driving increasing philanthropic giving, so that we can distribute £6.5 million in grants annually by 2029. As Head of Programmes, you will play a key role in turning that ambition into action.
You will lead and develop our grant-making programmes, ensuring they are informed by community need, insight and evidence and reflect best practice in equitable and accessible funding. You will work closely with local communities, voluntary and community organisations, funders and partners to understand emerging needs and identify opportunities for collaboration and innovation. You will be able to administer funds directed by local authorities and other partners to their specification, bringing a high quality service and repeat “business” to our revenue funding through high quality grant assessment and distribution.
