The Outreach Support Worker is responsible for delivering practical and emotional support, developing and delivering person-centred care, and working in partnership with local agencies to promote positive outcomes.

About the role:

As an Outreach Support Worker, you will provide flexible, community-based support to adults with learning disabilities who live in their own homes, with family, or in supported living settings. Your focus will be on enabling each person to live as independently as possible while achieving their individual goals and aspirations.

The Outreach Support Worker will build positive, professional relationships based on trust and respect, taking time to understand each person’s strengths, preferences, communication style, and support needs. Support may include developing independent living skills such as meal planning and preparation, budgeting and money management, travel training, maintaining a tenancy, and accessing healthcare services.  It may also involve accompanying individuals to social groups, volunteering opportunities, college courses, employment placements, faith groups, or recreational activities. The post holder will actively promote social inclusion, confidence-building, and the development of meaningful relationships.

The role requires accurate record-keeping, maintaining clear communication with your team, and following safeguarding, health and safety, and medication policies at all times. You will be expected to respond calmly and appropriately to challenging situations, using positive behaviour support approaches where required.

This is a community-based role, so flexibility is essential. You may be required to work earlies, evenings, weekends, and bank holidays on a rota basis to meet the needs of the people we support.  You will work in partnership with families, carers, social workers, healthcare professionals, and other agencies to ensure a coordinated and consistent approach to support.

Above all, this role is about empowerment, enabling people to make choices, take positive risks, build skills, and live fulfilling lives on their own terms.

About the roles 

These are new roles focused on bringing lived experience into everyday inpatient care.This is a pilot project, hence the one-year contract term. 

You will work alongside ward teams to support recovery focused and therapeutic activity. The role is about being present on the ward, building relationships, and contributing to a positive and engaging environment for patients. 

You will use your lived experience appropriately to support hope, participation and recovery, while working within clear professional boundaries.

Hours and contracts

We are recruiting a small team of Lived Experience Recovery Workers to work across inpatient settings in West Sussex. 

Contracts will be offered flexibly, likely across 4 to 5 roles, with a mix of full time and part time hours. 

All roles will be place based, working within a single inpatient setting. There is no requirement to travel between sites. 

We welcome applications from people looking to work between 3 and 5 days per week. 

Final hours and working patterns will be agreed based on service needs and the strengths of the successful applicants. Our aim is to build a balanced team that provides consistent ward based presence across all sites.

About the role

As a Housing Rights Worker, you will deliver high quality housing advice and advocacy to individuals and families with the ultimate aim of helping them thrive in their communities. Your work will be in line with London Hub’s priorities of improving the practice of local authorities and Registered Social Landlords and focussing on households disproportionately affected by the housing emergency. Working alongside people who are experiencing homelessness and bad housing to identify issues facing local communities, you will deliver casework to families to resolve their housing situation as well as engaging with community groups, local organisations and individuals to understand and address the housing issues in London.

In your community work you will ensure that people with lived experience of homelessness have opportunities to share their stories, give their views and have their say in the design and delivery of Shelter services. Offering day to day support to volunteers, providing learning, shadowing and mentoring and being a consistent role model for our values are also key aspects of the role.

About you

You will have demonstrable experience of working with families and vulnerable people and/or people with multiple and complex needs, within a safeguarding framework. You have experience and knowledge of housing and homelessness advice and advocacy and are able to carry out casework related interviews, maintain detailed case records, advise and support your clients to make informed decisions. You are able to communicate clearly to a variety of audiences and collaborate with others to get the job done.

 About the team

Our London hub specialises in providing housing advice and emergency homelessness work, intensive support to families and individuals, people experiencing domestic abuse and people experiencing multiple disadvantage. We provide front line support to over 5,000 people a year and work to bring about systemic change with the ultimate aim of people being able to live securely in suitable, safe, affordable homes.

Based in Old Street, the Shelter London Hub services specialises in providing housing advice and emergency homelessness work, intensive support to families, people experiencing domestic abuse, and people experiencing multiple disadvantage, preventing homelessness and helping them to access safe and affordable homes. The team also support the community to address systemic issues that may prevent families or individuals from sustaining a home, providing targeted advice sessions in community settings, with the aim of building capacity within communities to campaign on housing issues.

About the role:

This role is based at our National Contact Centre in Cardiff.

As a Caseworker you will be: -

• Effectively manage a caseload of self-referrals or referred service users, delivering excellent services which support service goals to victims. Where relevant to undertake initial proactive contact with victims, explaining VS services and promoting the benefits of accessing support.
• Ensuring each victim receives an individually appropriate tailored support and information service that fully meets their needs, keeping complex needs central to all processes and decisions.
• Undertaking risk and needs assessments to address the specific needs of the client in line with VS operating procedures. To follow relevant safeguarding policies and procedures, working with safeguarding leads and other agencies to ensure an integrated approach to address any safeguarding concerns.
• Maintain accurate and confidential case records, complying with data protection and all other legislation connected to your work and sharing protocols where necessary. Contribute to monitoring information as required by Victim Support.
• Developing and maintaining effective working relationships and promote effective communication between staff and other stakeholders.

You will need:

• An understanding of the impact of crime and traumatic incidents on victims.
• Experience of managing a caseload including complex case management.

Overview

The Premier League Foundation is seeking to appoint an experienced Senior Grants and Compliance Manager on a permanent basis. As a key member of the Central Operations team, you will help ensure the Foundation operates as an effective and efficient charity by maintaining robust, high‑quality grant‑management processes. Working closely with the Head of Governance and Operations and the Senior Impact and Insight Manager, you will oversee all grant‑making activity, from application through to monitoring, reporting and end‑of‑grant procedures. You will ensure that grantees comply with funder requirements, and that our grant‑making practices reflect, promote and continue to evolve in line with sector best practice.

You will collaborate closely with the Programmes Team and the Impact and Insights Team to ensure they have the tools and frameworks needed to monitor grant delivery and performance in line with Premier League programme expectations and funder requirements. Supported by the Governance and Grants Executive, you will track how funding is used, ensure grant conditions are met, and lead the ongoing improvement of grant‑making systems and processes.

A core part of the role will be leading the development and implementation of the Capability Code of Practice — a governance framework designed specifically for the unique needs and context of professional football club charities. You will ensure the Code continues to evolve with governance best practice and will work directly with Premier League club charities to create regular opportunities for sharing good practice, supporting them to achieve full compliance with the Code and to strengthen their effectiveness and efficiency as charitable organisations.

Working alongside the Head of Governance and Operations, you will also help ensure the Foundation maintains strong governance and operational excellence, including our own compliance with the Code and ongoing improvements to internal systems and processes.

Who we are

The Premier League Foundation was established in 2010 to distribute funding provided by the Premier League and its partners to professional football club charities across England and Wales, supporting them in their delivery in thousands of community venues and schools across the country. A total of 106 club charities are supported across the Premier League, English Football League, and National League.

More than 2.5 million individuals have benefitted from the charity, which aims to foster positive sporting, health, personal and education opportunities for children, young people, and the wider community.

The role

• Oversee the Premier League Foundation’s grant management function, ensuring timely, high-quality and consistent processes for grant assessment, approval, delivery, monitoring and reporting.
• Lead the development, implementation and continuous improvement of grant-making processes to enhance operational efficiency and service delivery.
• Provide advice and support on the design and development of application and grant-making processes for new programmes and funds.
• Work with the Head of Governance and Operations and the Finance team to support grant payment processes, including quarterly grant payments as required.
• Oversee the management of grant agreements, supported by the Foundation Governance and Grants Executive, and establish processes to ensure all grant-related documentation is quality assured and appropriately maintained.
• Contribute to reports relating to governance and grant management including mid-year and end of year monitoring updates for Premier League Foundation Board meetings.
• Support the Programmes Team in producing engaging and informative reports on programme performance and impact for our funding partners and other relevant stakeholders as required.

• Work with impact and insight colleagues to develop and implement monitoring and evaluation frameworks for grant-funded projects.

• Oversee the development and implementation of the Capability Code of Practice and the Capability Audit process, working collaboratively with funding partners and key stakeholders to assess the governance and management arrangements of funded organisations, and to embed a culture of continuous improvement across the network
• Develop and maintain strong relationships with the 20 Premier League football club charities to ensure compliance with the Capability Code of Practice, providing tailored support, advice and guidance to aid the effective implementation of their Capability Action Plans
• Lead the Premier League Foundation’s internal self‑assessment against the Capability Code of Practice, monitoring progress and evaluating performance against the actions set out in our Capability Action Plan.
• Lead the delivery of the bi‑annual Capability Code of Practice Conference, as well as other grant‑making or governance‑related networking opportunities, events and training sessions
• Line-manage the Governance and Grants Executive, providing regular supervision, workload management, wellbeing support and opportunities for professional development.
• Ensure the Foundation is well connected to other initiatives and organisations in the sector and stay informed of trends in grant funding practice to inform operational improvements.
• Represent the Premier League Foundation in a professional and positive manner when engaging with internal and external stakeholders, always reflecting the organisation’s values.
• Undertake any other duties that may reasonably be required, and are commensurate with the job role, in furtherance of the objectives of the Premier League Foundation.

Requirements for the role

• Strong written and analytical skills, with the ability to produce high-quality, impactful board reports.
• Experience working in governance, grant management or compliance within the voluntary, community, and/or sports sector.

Key responsibilities include:

· Delivering a coordinated family support service across both hospice sites

· Working closely with the Senior Social Worker and wider MDT to ensure holistic care

· Championing best practice in palliative and end‑of‑life care with internal and external partners

· Applying your expertise in safeguarding, the Mental Capacity Act, DoLS and related legislation

· Completing assessments and applications for Social Care and Fast‑Track CHC packages to enable timely support

Baca is dedicated to providing support and care to 16 to 18 year old unaccompanied asylum seeking children who are newly arrived in the country, helping them rebuild their strength, dignity, and hope for the future. We work closely with social workers and representatives from other agencies to benefit the young people in our care.

Role Overview:

We are seeking an experienced and values‑led Accommodation & Facilities Manager to take responsibility for the safe, compliant and welcoming management of all Baca accommodation, offices and service delivery spaces.

This is a critical role within our service model. You will ensure that young people are provided with homes they can truly call their own, while staff and volunteers have safe, functional, culturally respectful environments in which to work. You will also lead our premises team and work closely with contractors, landlords and partners to deliver high‑quality, compliant properties across our services.

Key Responsibilities

• Oversee the day-to-day management of all Baca accommodation, offices, and service delivery spaces applying practical and technical knowledge and experience to diagnose maintenance issues, oversee and complete repairs.
• Ensure properties are safe, welcoming, culturally appropriate, and fully compliant with health & safety and regulatory standards
• Lead maintenance planning to support short-, medium-, and long-term property sustainability
• Line manage and support three Maintenance Workers, working collaboratively with service teams and leaders
• Ensure compliance with all relevant legislation, inspections, and audits, maintaining accurate documentation
• Manage maintenance budgets, monitor spend, and ensure value for money
• Build and maintain strong relationships with landlords, contractors, and suppliers

Purpose of the role

To deliver the Day One Casework bedside model to patients, or those closest to them, impacted by serious or multiple injuries which could result in life changing consequences, such as disability or death, within Leeds General Infirmary Hospital, providing support on some of the wards (such as rehabilitation), on-site clinics and other settings (e.g. rehabilitation centres). 

Have a visible physical presence within the Trust, becoming embedded into clinical teams focussing on major trauma pathways, facilitating outpatient and in-person legal clinics.

Key Responsibilites

The post holder’s primary duties and responsibilities are as follows: 

• Work closely, and in partnership, with NHS clinical staff and relevant community and voluntary sector organisations, to address the needs of patients, and those closest to them, affected by serious and life-changing injury. 

• Operate a case management approach to individuals, assessing and identifying needs, putting support in place including signposting, making referrals and direct support. 

• Provide consistency in assessment of all patients and their loved ones, irrespective of injury cause, age, status, giving access to the earliest possible specialist legal advice which supports rehabilitation and NHS cost recovery. 

• Maintain detailed case records, including accurate records of activity and intervention, using Day One’s Customer Relationship Management (CRM) system, capturing and storing data in line with Day One policies and procedures and regulatory requirements. 

• Contributing towards report writing through the writing case studies and narrative to support data collection. 

• Implement processes and procedures to collect patient and family feedback and contribute to ongoing monitoring and evaluation of Day One services through providing relevant information, case studies and reports. 

• Deliver awareness raising presentations to clinical colleagues and departments. 

• Organise and facilitate outpatient and in person legal clinic drop-in sessions. 

• Build awareness of the role and charity’s purpose through building strong and effective relationships across the regional major trauma network, ensuring those who need it have access to Day One support. 

• Work closely with local authority, statutory and voluntary organisations to provide patient and family support and advocacy. 

• Help establish other support mechanisms once a person is no longer within the hospital setting, supporting the patient discharge process in conjunction with leadership from NHS staff. 

• Identify and support the recruitment and supervision of Day One volunteers, where appropriate. 

• Work closely in partnership with our Peer Support Service, focussed on promoting awareness and uptake within the region. 

• Work closely with the wider team on performance, quality, safety and effectiveness of all services, ensuring appropriate safeguarding policies are followed. 

• Work closely with our Fundraising and Communications team, supporting national activity, as well as regional initiatives to raise awareness and fundraise. 

• Be prepared to travel across the region and, on occasion, to other sites and national meetings as required. 

• Willingness to undertake continuous development and training for the role, including mandatory Day One and NHS Trust specific training. 

• Participate in external clinical supervision and monthly caseworker reflective practice to sessions to effectively explore and uphold professional boundaries within a safe, structure and supportive environment.

To work closely with those impacted by serious and life-changing injury, taking referrals and carrying out initial assessments of need, expertly navigating, signposting and putting services in place in the immediate aftermath of major trauma, including talking about and facilitating timely access to legal support to aid rehabilitation.

To establish and develop relationships with key stakeholders to promote Day One and address the needs of those affected by serious and life-changing injury. 

To work as part of the wider Day One Service’s team and organisation, taking responsibility for own record keeping and data collection in line with regulatory requirements.
